In the context of excess: supporting a lean, healthy weight is foundational to metabolic health.

In depth

How much body fat you carry, shown by body weight, BMI and waist size. Higher means more fat. Where fat sits matters too: fat around the belly and organs (visceral fat) carries more health risk than fat under the skin, and muscle is tracked separately.

What drives it

Fat cell clearance reduces it.

Terminal deoxynucleotidyl transferase-mediated dUTP-biotin nick endlabeling (TUNEL) DNA fragmentation analysis revealed that fat-mass decrease by CLA was mainly due to apoptosis. doi 10.2337/diabetes.49.9.1534

Daily energy burn reduces it.

In this eight-month randomised controlled trial in sedentary overweight adults asked not to change their diet, raising exercise energy expenditure produced dose-dependent loss of body mass and fat mass (high-amount group -2.9 kg body mass, -4.8 kg fat mass) while the non-exercising controls gained both. doi 10.1001/archinte.164.1.31 · PMID 14718319

Parasite burden reduces it.

Cochrane screened-infected arm, with the adversarial IPD network meta-analysis retrieved in full alongside it. Verbatim: “a single dose of deworming drugs may increase weight (0.58 kg, 95% CI 0.40 to 0.76, three trials, 139 participants; low quality evidence).”. PMC8356492

Curbs appetite reduces it.

Body-weight/appetite review 2008, J Clin Endocrinol Metab — satiation signals (CCK, GLP-1, gastric distension) terminate meals and, integrated with adiposity signals, regulate energy homeostasis; enhancing satiety lowers food intake and body weight. PMID 18987269

Feeding drive increases it.

Stanley 1986, Peptides — chronic hypothalamic neuropeptide Y injection overrides satiety, driving hyperphagia and body-weight/fat gain. PMID 3470711. Two mechanisms at once, which is why these neurons are so powerful: NPY/AgRP neurons — found only in the hypothalamic arcuate nucleus — stimulate feeding both by raising NPY signaling and by releasing AgRP, an endogenous melanocortin-3/4 receptor antagonist that removes the satiety brake. They push the accelerator and cut the brake cable in the same action. And they move both sides of the energy equation: when activated they stimulate food intake and reduce energy expenditure. Regulation is explicit: inhibited by insulin and leptin, activated by ghrelin — so a fall in leptin or insulin signaling activates them, which is why fasting, uncontrolled diabetes and genetic leptin deficiency all raise food intake. The controlled experiment that changes the advice: in pair-fed mice — matched for caloric intake and weight gain — high-fat diet still produced a significant increase in AgRP neuronal spiking. The authors conclude diet composition may be more important than either calorie intake OR body weight for electrically remodeling these neurons. Even brief high-fat feeding caused persistent activation, and leptin-insensitivity was not a prerequisite for the raised firing rate — the circuit changes before leptin resistance appears. these neurons are not essential for all feeding — they are stated to be not required for the feeding response to glucoprivation. PMID 3470711

Thyroid reduces it.

The same conversion failure shows on the scale: reduced deiodinase activity lowers T3 and manifests as weight gain alongside fatigue and cold intolerance, again with T4 apparently normal (PMC12672807). This is why a thyroid complaint and a weight complaint can share one upstream lane rather than being two separate problems. PMC12672807

Body clock reduces it.

The title is a causal claim the paper earns: genetic and environmental circadian disruption induce weight gain through changes in the gut microbiome. Two independent models agree: mice with BMAL1 deficiency in the suprachiasmatic nucleus and wild-type mice under simulated shift work both gained weight, both showing desynchronisation between peripheral gastrointestinal clocks and reduced microbial rhythmicity. And the transfer experiment makes it mechanistic: germ-free colonization with shift-work-associated microbiota mechanistically links body weight gain to the microbial changes. The recipients never did shift work. The recipients also showed altered peripheral clock gene expression — the transplanted microbiota reset the host clock, which is the loop closing. Bidirectional and the authors say so: conversely, body weight changes induce microbiota dysbiosis. doi 10.1016/j.molmet.2022.101628 · PMC8291023

Lowers cortisol increases it.

The most useful correction on this pathway: the blood test is the wrong place to look. Cushing’s syndrome produces centripetal fat distribution with visceral adiposity playing a leading role — raise cortisol enough and the body reorganises where it stores fat. But ordinary obesity is not a mild version of that: obesity and insulin resistance ARE not states of sub-clinical glucocorticoid excess — circulating cortisol levels ARE either normal OR even slightly reduced. Anyone measuring serum or salivary cortisol to explain someone’s weight is measuring the compartment where the signal is absent. The signal is tissue-local and the enzyme is named: 11-beta-hydroxysteroid dehydrogenase type 1 converts inactive cortisone to active cortisol inside the tissue, and its activity is elevated in adipose tissue in obesity while hepatic activity is reduced. The fat is making its own cortisol. And the causal test dissociates cleanly: mice overexpressing 11-beta-HSD1 in adipose tissue develop visceral obesity, insulin resistance, dyslipidemia and hypertension — while liver-specific overexpression produces insulin resistance and hypertension but not obesity. Same enzyme, different tissue, and the obesity follows only one. The enzyme is also expressed in the hypothalamic arcuate nucleus, and selective inhibitors reduce food intake and weight gain. Glucocorticoids do both things at once: they simultaneously increase lipolysis and adipogenesis through separate pathways, and the net effect depends on the physiologic context including circadian variation relative to meal timing. Lowers wake signal reduces it.

10.3389/fendo.2020.00145 — the peptide is named for appetite — orexin, from the Greek — and acutely it does promote feeding. Yet losing it makes animals and people fat while they EAT less. The human natural experiment: narcolepsy type 1 is caused by loss of more than 90 percent of orexin-producing neurons, and those patients show significantly higher BMI and larger waist circumference — despite normal to decreased food intake and comparable physical activity. The comparison group is chosen well: patients with idiopathic hypersomnia, who share the sleepiness but have normal orexin, do not show it. Gain of function goes the other way: transgenic orexin overexpression confers resistance to high-fat-diet-induced obesity by promoting energy expenditure and reducing consumption — and the phenotype depends on an intact OX2R gene, which the authors note proves physiological signaling rather than a transgene artefact. Another, in the same knockout, states: we did not detect abnormalities in brown adipose tissues of orexin-deficient mice. The weight phenotype is not in dispute; the brown-fat explanation is. doi 10.3389/fendo.2020.00145

Endocannabinoid tone increases it.

The hypothesis came from an observation anyone could have made: appetite rises sharply when CB1 receptors are activated. It worked: a meta-analysis of four rimonabant trials showed weight loss of 4.7 kg over placebo after one year — larger than sibutramine (4.5 kg) or orlistat (2.7 kg). The mechanism is described as primarily affecting energy intake by dampening the reward aspect of feeding behavior. And it was withdrawn, because the European committee for medicinal products for human use found that rimonabant doubled the risk of psychiatric disorders — depressed mood, anxiety, irritability, insomnia and suicidality. In November 2008 all further research halted, and the clinical development of three other CB1 antagonists was terminated: taranabant, otenabant and SLV-319. The MAP already draws the Endocannabinoid Signaling → Mood (Positive Affect) link and the Endocannabinoid Signaling → Anxiety link, both on evidence that CB1 activity is protective. Rimonabant is a CB1 inverse agonist. The drug that moved weight in the desired direction moved those two nodes in the opposite one — and the psychiatric harm was not an unrelated toxicity, it was the same receptor action read at a different outcome. The atlas predicts this collision from edges evidenced independently, in different reviews. And separation looks unlikely rather than merely difficult: the therapeutic effect and the psychiatric effects occur over the same dose range, and the side effects ARE most pronounced at the most efficacious weight-reducing dose. Only about 50 percent of the benefit to triglycerides, fasting insulin and insulin resistance could be accounted for by the weight loss itself. PMC3083904

Mood (serotonin) reduces it.

The mechanism is explicit: fenfluramine and DEXFENFLURAMINE ARE serotonergic drugs which cause the release of serotonin to suppress appetite and reduce food intake. Both were individually FDA-approved. The mixed reuptake inhibitor sibutramine achieved 4.5 kg weight reduction in meta-analysis. And the failure mode was cardiac and structural: fenfluramine and DEXFENFLURAMINE were withdrawn because of heart valve damage. Not a tolerability issue — irreversible organ injury from a weight-loss drug. The pattern across this pathway is the point: every centrally-acting appetite SUPPRESSANT that produced meaningful weight loss also produced a serious off-target harm, and each failed in a different organ — serotonergic (fenfluramine: heart valve damage), mixed serotonin-norepinephrine (sibutramine: major adverse cardiovascular events in SCOUT), and endocannabinoid (rimonabant: psychiatric disorders). And none changed the SETPOINT: patients were unable to retain weight loss once therapy was discontinued. PMC3083904

Alertness chemical reduces it.

The anatomy is named: PHENTERMINE is a noradrenergic drug which stimulates the release of norepinephrine and reduces food intake by acting on beta-adrenergic receptors in the perifornical hypothalamus. Sibutramine, inhibiting reuptake of both serotonin and norepinephrine, produced 4.5 kg of weight loss. Its failure mode was cardiovascular rather than VALVULAR: sibutramine suppresses appetite and increases heart rate and blood pressure, and in the sibutramine cardiovascular outcomes trial an increase in major adverse cardiovascular events prompted its withdrawal in Europe and the united states. The appetite benefit and the pressor harm are the same pharmacology read at two outcomes — structurally identical to the rimonabant collision on the Endocannabinoid Signaling → Weight & Body Fat link. PMC3083904

Supports BDNF signaling reduces it.

WAGR syndrome is an accidental experiment nobody could have run deliberately: it is caused by contiguous deletions on chromosome 11p13 hitting WT1 and PAX6, and in some patients the deletion extends far enough to take out one copy of BDNF at 11p14.1 — about 4 megabases away. That splits the syndrome into two natural groups differing in essentially one gene. A study of 33 patients found BDNF HAPLOINSUFFICIENCY associated with lower serum BDNF and with childhood-onset obesity, and 100 percent of the WAGR patients rendered BDNF-HAPLOINSUFFICIENT were obese by 10 years of AGE. Two independent single-patient confirmations: an 8-year-old girl with a de novo chromosomal inversion leaving one functional BDNF copy, with monoallelic expression confirmed; and a boy with a de novo Y722C missense in TrkB, the BDNF receptor, at a key tyrosine in the kinase activation loop. Ligand and receptor, two patients, same phenotype. And the mouse work closes the loop three ways: re-expressing Bdnf-e2 in the ventromedial hypothalamus rescued the hyperphagia and obesity; deleting TrkB in VMH neurons of wild-type mice produced them; and infusing A TrkB agonistic antibody into the VMH alleviated the phenotype. An unplanned convergence: the Thyroid Hormone (T3) → Weight & Body Fat link was drawn on TR-beta knockdown in the ventromedial hypothalamus. Two independent drivers of weight, evidenced separately from separate literatures, both land in the same nucleus. Rare monogenic and syndromic obesity — WAGR prevalence about 1 in 500,000. Nothing here shows ordinary BDNF variation explains ordinary body weight. doi 10.1056/NEJMoa0801119 · PMC4898883

Training load reduces it.

The interesting part is not that it works but how much your body takes back. Dose matters: a 6-day-per-week group expended 2753.5 kcal and exercised 320.5 minutes weekly versus 1490.7 kcal in the 2-day group, and achieved greater fat loss than either the 2-day or control groups. But energy compensation is real and dose-independent: the exercise groups did not differ in the percentage OR total kilocalories compensated. Exercising six days a week rather than two does not buy a better compensation ratio — it buys a bigger absolute expenditure that the same fraction is clawed back from. This is why the gap between expected and actual exercise weight loss is so consistently disappointing. And the predictor of success was a hormone, not the workout: greater decreases in ACYLATED ghrelin area-under-curve predicted greater fat loss, regardless of group, energy expended, duration OR intensity. Metabolic adaptation compounds it, attenuating both total daily energy expenditure and resting metabolic rate. PMID 33064415 · PMC12908587

What it drives

It increases Type 2 diabetes.

Obesity is the dominant modifiable risk factor: insulin resistance may be mostly explained by the presence of obesity, and it accelerates progression to diabetes in those predisposed to beta-cell failure (PMID 15561919). Diet and exercise prevent or delay diabetes in impaired glucose tolerance. PMID 15561919

It increases Joint wear.

Body weight acts on the joint metabolically as well as mechanically: obesity raises pro-inflammatory cytokines and degrading enzymes that inhibit cartilage matrix synthesis, and metabolic syndrome and osteoarthritis share inflammatory-aging, oxidative-stress and lipid-metabolism mechanisms. PMC4842398

It increases PCOS.

Obesity aggravates the insulin resistance that drives the syndrome, and lifestyle improvement is first-line in every current treatment framework (PMC9832677, PMC9030414). PCOS affects 6-20% of women of reproductive age. PMC9832677

It reduces Joint comfort.

Body weight acts on the joint chemically as well as mechanically: obesity raises pro-inflammatory cytokines and degrading enzymes that inhibit cartilage matrix synthesis, and osteoarthritis shares inflammatory-aging, oxidative-stress and lipid-metabolism mechanisms with metabolic syndrome (PMC4842398). The load you carry and the inflammation you carry reach the same tissue by two different routes. PMC4842398

It reduces Fat-burning hormone.

10.1186/s42506-020-00060-z — the one adipokine that runs backwards. Adiponectin is secreted only by fat tissue, yet its levels are paradoxically lower in obese than in lean people — the opposite of every other adipocytokine, which rise in proportion to fat mass (Diabetes Care 2003;26:2442). And the depot matters more than the weight: secretion from visceral fat falls as adiposity rises (r=−0.31, p=0.01 against BMI) while subcutaneous secretion is unchanged — which is why central obesity, not body mass as such, tracks with low adiponectin, and why lower-body fat associates with higher levels (PMC6010237). The limit: the relationship is not universal — a positive correlation appears in liver cirrhosis and in some obese individuals ('the adiponectin paradox'), so this is a strong tendency, not a law. doi 10.1186/s42506-020-00060-z · PMC6010237

It increases Calms inflamed fat tissue.

The uncontested half of a relationship whose other half is disputed. That adiposity produces adipose tissue inflammation is not in question anywhere in this literature: early during high-fat feeding, adipocytes undergo hypertrophy, releasing chemokines that induce recruitment of M1-polarized macrophages. With increased adiposity, recruited CCR2-positive macrophages form crown-like structures and overwhelm the protective effects of M2 macrophages. The chemokines ARE named: CXCL12 produced by adipocytes interacts with CXCR4 to mediate macrophage recruitment, with haptoglobin and C3a also reported. And the trigger is adipocyte death — the crown-like structures form around necrotic adipocytes, which is why the histology looks the way it does. Drawing this direction explicitly matters: it shows what obesity does to fat tissue without depending on the disputed direction between fat-tissue inflammation and insulin. One arrow is settled and drawn; the other is contested and labeled. doi 10.3389/fimmu.2023.1153915 · PMC1716210

It reduces Insulin sensitivity.

The authors state the problem plainly: reverse causality has made it difficult to establish the causal directions, and direct evidence of obesity causing systemic insulin resistance in humans is still lacking. So they used genetic instruments, in UK BIOBANK and METSIM, restricted to non-diabetic individuals so that established disease could not explain the result: both prediabetes and systemic insulin resistance ARE caused by obesity, p = 1.2 x 10^-3 and p = 3.1 x 10^-24 respectively. That second p-value is not a marginal finding. And they did not stop at the association — they went to the tissue: bulk and single-nucleus RNA sequencing on frozen human subcutaneous adipose biopsies showed that adipose mitochondrial gene expression and body fat percent ARE independently associated with insulin resistance, and together with adipose cell-type composition these three factors explain 44.39 percent of the variance and predicted it across three independent human cohorts. Here is the same tissue named as the mediator between body fat and systemic insulin resistance, from an entirely separate literature. Note the direction: the insulin pathway here stands for insulin sensitivity, so this link is drawn inhibitory — obesity reduces sensitivity. doi 10.1530/JOE-24-0269 · PMC7515203

It increases Restores fullness signal.

10.3389/fendo.2023.1172060 — The loop that makes the resistance story coherent. Leptin is synthesized and secreted by adipocytes in proportion to fat mass — so adiposity raises leptin, and in common obesity leptin is high precisely because there is more fat reporting. This link explains why people with obesity have plenty of a hormone that lowers weight. doi 10.3389/fendo.2023.1172060

It increases Calms body-wide inflammation.

The drivers mirror neuroinflammation’s where the same evidence supports both, and the inflammaging reviews name them jointly: nutritional perception dysregulation and inflammatory aging ARE the core key elements linking upstream molecular damage to downstream organ function decline. Where the driver is shared with neuroinflammation the evidence is the same evidence - the split was about what these nodes drive, not about what drives them, and that asymmetry is stated rather than hidden. doi 10.1016/j.molmet.2023.101755 · PMC12293478
