It starts at the longest nerves and works inward, which is why the feet go first. The trial evidence is unusually clear: tightening glucose control cut the risk by nearly two-thirds, and the benefit outlasted the trial by more than a decade. Prevention here is worth far more than treatment.
In depth
Nerve damage from sustained high blood glucose — numbness, burning or pain, beginning in the feet.
What drives it
Testosterone increases it.

Blood-sugar support increases it.
10.1002/14651858.CD007543.pub2 — honest scale: this is prevention and delay, not reversal — 33% of DCCT/EDIC participants developed neuropathy anyway. Neuropathy is one of the three classical microvascular complications alongside retinopathy and nephropathy, named as leading causes of blindness, end-stage renal disease and various painful neuropathies; AGE formation is directly implicated in its pathogenesis. The mechanism is five pathways, not one — Brownlee's unifying framework (Diabetes 2005;54:1615-25): the polyol pathway (aldose reductase converting glucose to sorbitol, consuming NADPH), the hexosamine biosynthesis pathway, protein kinase C activation, angiotensin II signaling, and advanced glycation end product accumulation — all converging on oxidative stress. An arrow from glycemic control to an outcome collapses five interacting routes. Metabolic memory — the caveat that changes the practical claim: the literature explicitly frames non-enzymatic glycation of proteins, lipids and nucleic acids as producing a persistent effect. AGEs accumulate preferentially in tissues with low protein turnover, particularly the extracellular matrix, which is why damage outlives the exposure. Restoring glucose control does not immediately unwind what has already been cross-linked. doi 10.1002/14651858.CD007543.pub2
Type 2 diabetes increases it.
Neuropathy is where sustained hyperglycemia is eventually felt. In UKPDS, intensive glucose control in type 2 diabetes cut overall microvascular complications by 25%, and post-hoc analysis found each 1% fall in HbA1c gave a 35% reduction in microvascular complications (PMC12855444). The dose-response is what makes this an edge rather than an association. PMC12855444

What it drives
It reduces Sexual function.
A reordering of cause that matters: neural injury often precedes, and then amplifies, vascular dysfunction, supporting the view of ED in diabetes as a neurovascular manifestation of systemic neuropathic disease rather than an EXCLUSIVELY vasculogenic condition. The standard account treats erectile dysfunction as a plumbing problem; the nerve damage came first. doi 10.3390/jcm15041621 · PMID 33923709 · PMC11988752
It reduces Physical performance.
10.1007/s13300-017-0295-y — This is what the symptom costs rather than what it causes physiologically. It is shown at reduced weight because no separate effect size has been reported for this arm. The asymmetry is deliberate and stated. Sensory and motor loss limits function. doi 10.1007/s13300-017-0295-y
It increases Calm pain signaling.
The defining feature is that the pain is real and the tissue is not damaged: central sensitization manifests as hyperalgesia, allodynia, expanded receptive fields, and impaired endogenous pain inhibition, and in osteoarthritis the same phenomenon does not correlate with radiographic findings - severe pain may persist despite stable joint structure. The mechanism is a balance failure in both directions at once: in the dorsal horn there is excessive release of glutamate, ATP, and substance P, leading to activation of NMDA receptors - while at the same time descending inhibitory mechanisms, notably those mediated by GABA, ARE weakened. Too much excitation and too little inhibition. And mast cells carry a specific hypothesis: thalamic mast cells releasing histamine, IL-1-beta, IL-6, TNF, CGRP and substance P, either stimulating thalamic nociceptive neurons directly or via microglia. Stated by the authors as a hypothesis, not an established mechanism. The reviewers ARE blunt about the state of knowledge: the pathophysiology and etiopathogenesis remain largely unknown, and there is NO definitive pathogenesis except for altered central pain pathways. doi 10.3390/biomedicines12071543 · PMC6687840
