Type 2 diabetes
Insulin resistance with sustained hyperglycemia↓
Also searched as Insulin resistance with sustained hyperglycemia.
It is the end of a long sum, not a sudden event — which is why every term above it (body weight, insulin sensitivity, beta-cell reserve) is worth correcting early.
In depth
Sustained high blood glucose arising when insulin stops working and the pancreas can no longer make up the difference.
What drives it
Insulin sensitivity reduces it.
Insulin resistance is the major pathophysiological feature of type 2 diabetes; obesity-driven release of non-esterified fatty acids, glycerol, hormones and pro-inflammatory cytokines from adipose tissue is what produces it. PMID 17167471
Protects insulin cells reduces it.
Type 2 diabetes never develops unless beta-cells fail to compensate for insulin resistance: a deficit of beta-cell functional mass is an essential component of the pathophysiology, present at diagnosis and worsening with duration (review, PMC4317303). Abnormalities in beta-cell function are therefore critical in defining risk. PMID 17167471 · PMC4317303
Supports weight management increases it.
Obesity is the dominant modifiable risk factor: insulin resistance may be mostly explained by the presence of obesity, and it accelerates progression to diabetes in those predisposed to beta-cell failure (PMID 15561919). Diet and exercise prevent or delay diabetes in impaired glucose tolerance. PMID 15561919
Slows carb absorption reduces it.
The same class that blunts the post-meal spike also lowers incident disease: alpha-glucosidase inhibitors improve glycemic control and reduce type 2 diabetes risk (PMC7930031). Note: long-term cardiovascular outcomes for this class have been inconsistent — the glycemic and incidence findings are the defensible ones. PMC7930031
Mood reduces it.
Depression increases the risk of diabetes at RR 1.60 in initially disease-free subjects. Consistent with the metabolic dysregulation named as a mechanism, and with obesity at RR 1.58 in the same series. Depression and anxiety behave differently on the same endpoint, and the map shows it. PMC3661358

Fat-burning hormone reduces it.
insulin, fasting insulin, type 2 diabetes and blood lipids share one literature: heart, kidney, liver, and metabolic diseases share interlinked pathological mechanisms, including inflammation, oxidative stress, insulin resistance, and endothelial health issues. These four overlap by nature: fasting insulin is the level, insulin is the signaling, type 2 diabetes is the diagnosis and blood lipids are the substrate. The same driver legitimately reaches all four, and a reader should not mistake four arrows for four independent findings (doi 10.1038/s41581-026-01066-0; doi 10.3390/ijms26146962; PMC11725408; NBK279115; PMC12257864). REFUTED ON A SECOND READING. This link was shown by linked steps until a source was opened that does not support it: “These results do not provide any consistent evidence that interventions aimed at increasing adiponectin levels will improve insulin sensitivity or risk of type 2 diabetes.” — Mendelian Randomization Studies Do Not Support a Causal Role for Reduced Circulating Adiponectin Levels in Insulin Resistance and Type 2 Diabetes. Diabetes, 2013, 62(10), 3589-3598. doi 10.1038/s41581-026-01066-0 · PMID 23835345 · PMC11725408

What it drives
It increases Nerve protection.
Neuropathy is where sustained hyperglycemia is eventually felt. In UKPDS, intensive glucose control in type 2 diabetes cut overall microvascular complications by 25%, and post-hoc analysis found each 1% fall in HbA1c gave a 35% reduction in microvascular complications (PMC12855444). The dose-response is what makes this an edge rather than an association. PMC12855444

It reduces Vision.
Why the retina is the most oxidatively stressed tissue in the body, stated directly: the retina is highly susceptible to oxidative damage due to its elevated oxygen consumption, abundant polyunsaturated fatty acids, and continuous exposure to light. Three risk factors that cannot be separated from its function. And the aging failure is specific and double: in the elderly both the mitophagic process and Nrf2 activation ARE impaired, thus causing retinal cell death. The carotenoid case is unusually mechanical: lutein and zeaxanthin ARE selectively taken up by the RPE, preferentially accumulated in the human macula, and transferred to photoreceptors - a targeted delivery route, not a general antioxidant claim. The Eye Disease Case-Control Study found high plasma levels of lutein and zeaxanthin associated with reduced risk of neovascular AMD. By contrast the reviewers say the relationships between dietary and serum levels of the antioxidant vitamins and age-related macular disease ARE less clear - with alpha-tocopherol the exception, convincingly demonstrated. Lipofuscin is itself a photoreactive substance - the waste product becomes a new damage source. doi 10.3390/antiox14101251 · PMID 11033038 · PMC12026614
