You want it slowed: blunting α-glucosidase flattens the glucose surge after meals.

In depth

α-glucosidase — the intestinal enzyme that breaks dietary carbohydrate into absorbable glucose, setting the post-meal sugar spike.

What it drives

It increases Insulin sensitivity.

By decreasing postprandial hyperglycemia and improving insulin sensitivity, acarbose therapy reduces fasting and postprandial serum insulin, fasting plasma glucose and HbA1c (PMID 12086760; doi 10.1016/S0140-6736(02)08905-5; PMID 14703948; doi 10.1002/14651858.CD005061.pub2; PMC8771266). doi 10.1016/S0140-6736(02 · PMID 12086760 · PMC8771266

It reduces Blood-sugar support.

Alpha-glucosidase sets the post-meal glucose spike, and inhibiting it is the proof: a systematic review and meta-analysis found postprandial glucose fell by 1.5 mmol/l with acarbose and 1.6 with miglitol in people with diabetes (0.4 and 0.6 in those without) — a relative reduction of 43-54% in both groups — by slowing the rate of carbohydrate digestion and gut glucose uptake (PMC7930031, DOI 10.1038/s41387-021-00152-5). In 1,996 patients on acarbose, 2-hour postprandial glucose fell 74.4 mg/dl and HbA1c 1.0%. doi 10.1038/s41387-021-00152-5 · PMC7930031

It reduces Type 2 diabetes.

The same class that blunts the post-meal spike also lowers incident disease: alpha-glucosidase inhibitors improve glycemic control and reduce type 2 diabetes risk (PMC7930031). Note: long-term cardiovascular outcomes for this class have been inconsistent — the glycemic and incidence findings are the defensible ones. PMC7930031

It increases Appetite & sugar hormone.

A counterintuitive lane, and the most useful kind. The L-cells that make GLP-1 live in the distal small intestine — so when α-glucosidase does its job efficiently, carbohydrate is absorbed high up and never reaches them. Inhibit the enzyme and large amounts of undigested carbohydrate arrive at the ileum, producing a long-lasting increase in GLP-1 secretion (PMC3653752; Expert Rev Endocrinol Metab 2014, DOI 10.1586/17446651.2014.931807). Acarbose also delays gastric emptying and augments CCK, GLP-1 and PYY, mainly by retarding carbohydrate absorption (PMID 11518688), and causality was tested directly with the GLP-1 receptor antagonist exendin(9-39) in a double-blind randomized crossover (PMID 33449919). Honest INCONSISTENCY: at least one trial found α-glucosidase inhibition failed to enhance GLP-1 or delay gastric emptying in type 2 diabetic patients (Diabet Med 2005;22:470-6), so the effect is well supported but not universal. Note also the contrast with DPP-4 inhibitors: α-glucosidase inhibitors raise GLP-1 while reducing plasma insulin and C-peptide — incretin enhancement without hyperinsulinemia. doi 10.1586/17446651.2014.931807 · PMID 11518688 · PMC3653752
