You want it protected: shielding the retina from oxidative damage preserves vision with age.
In depth
Sight as a processing chain, not an organ: optics, retina, and the cortical interpretation that turns signal into scene. Most age-related complaints arise downstream of the eye itself.
What drives it
Oxidative stress reduces it.
AMD oxidative-stress review 2018 — excess reactive oxygen species damage the retinal pigment epithelium, driving macular degeneration and loss of central vision. PMC6230566. The mechanism: the retinal pigment epithelium is highly susceptible to oxidative damage because every one of its jobs -- photoreceptor outer-segment phagocytosis, retinoid cycling, blood-retinal barrier transport, light absorption by melanin and lipofuscin -- requires active oxidative metabolism. Excess ROS oxidize DNA, proteins and lipids; lipid oxidation degrades membrane and lipoprotein function and triggers inflammation; mitochondria and lysosomes break down, killing RPE and then photoreceptor cells. The dose-dependence is confirmed independently here: reactive oxygen intermediates are described as mainly byproducts of physiological processes and part of a normal feedback loop in redox signaling -- damage arises from OVEREXPOSURE plus impaired antioxidant defense with aging, not from ROS as such. AMD is multifactorial -- oxidative stress, chronic inflammation, immune dysregulation and genetic predisposition. ROS is one of four named contributors, not the account. And the 2025 review states plainly that effective therapeutic options remain limited and there is NO definitive cure for AMD. Verbatim: “This imbalance contributes to cellular damage and neurodegeneration, ultimately leading to vision loss”. doi 10.3390/ijms26083463 · PMC6230566
Blood-sugar support reduces it.
The same trial that settled neuropathy settled the eye: intensive glycemic control reduced the incidence of diabetic retinopathy alongside nephropathy and neuropathy, and the long-term benefit persisted into EDIC follow-up (PMC5084623, PMC12855444). Retinopathy remains a leading cause of preventable sight loss in working-age adults, which is what makes this pathway worth drawing rather than assuming. Diabetic retinopathy is described as one of the most serious microvascular complications induced by hyperglycemia. PKC activation induces VEGF and RAGE expression, which is the direct link to the neovascular step. The mechanism is five pathways, not one — Brownlee's unifying framework (Diabetes 2005;54:1615-25): the polyol pathway (aldose reductase converting glucose to sorbitol, consuming NADPH), the hexosamine biosynthesis pathway, protein kinase C activation, angiotensin II signaling, and advanced glycation end product accumulation — all converging on oxidative stress. An arrow from glycemic control to an outcome collapses five interacting routes. PMC5084623

Why the retina is the most oxidatively stressed tissue in the body, stated directly: the retina is highly susceptible to oxidative damage due to its elevated oxygen consumption, abundant polyunsaturated fatty acids, and continuous exposure to light. Three risk factors that cannot be separated from its function. And the aging failure is specific and double: in the elderly both the mitophagic process and Nrf2 activation ARE impaired, thus causing retinal cell death. The carotenoid case is unusually mechanical: lutein and zeaxanthin ARE selectively taken up by the RPE, preferentially accumulated in the human macula, and transferred to photoreceptors - a targeted delivery route, not a general antioxidant claim. The Eye Disease Case-Control Study found high plasma levels of lutein and zeaxanthin associated with reduced risk of neovascular AMD. By contrast the reviewers say the relationships between dietary and serum levels of the antioxidant vitamins and age-related macular disease ARE less clear - with alpha-tocopherol the exception, convincingly demonstrated. Lipofuscin is itself a photoreactive substance - the waste product becomes a new damage source. doi 10.3390/antiox14101251 · PMID 11033038 · PMC12026614
