Sustained focus
Vigilance and distractor filtering

This is a balance node, not a raise node. Attention follows an inverted U: too little arousal and nothing registers, too much and you cannot hold a thread. Healthy older adults show tonic noradrenergic hyperactivity and their attention is worse for it — more of the signal is not more focus.
In depth
Sustained attention — the capacity to keep focus on one task, shift it deliberately, and filter distraction over time.
What drives it
Alertness chemical balances it.
Focus here is evidenced on normal-range attention in healthy people, not on trials in diagnosed populations. Human, healthy, preregistered: in 63 adults over 50 from the Cambridge Center for Aging and Neuroscience cohort — none with a diagnosis of dementia or mild cognitive impairment, all scoring above consensus thresholds for normal cognition — better response inhibition on a stop-signal task correlated with locus coeruleus integrity measured by magnetisation-transfer MRI, and with connectivity between pre-supplementary motor area and right inferior frontal gyrus, but not with the volumes of those prefrontal regions. The nucleus, not the cortex it drives. And it holds at autopsy: neuronal density in the locus coeruleus was linked to higher baseline functioning and slower cognitive decline before death, which is why the noradrenergic system is proposed as a component of cognitive reserve. Why this link is drawn as balance and not as raise: the LC operates by adaptive gain with distinct tonic and phasic modes, and the relationship to performance is an inverted U. Cognitively healthy older adults sustain tonic LC hyperactivity, and the consequence is worse attention, not better: arousal elicited weaker behavioral responses, weaker pupil dilation and weaker P300 responses in older adults than young. A system already running hot cannot mount a phasic response to what matters. The practical reading: raising noradrenergic tone is not the same as improving focus, and in an already-elevated person it is the opposite. The fatigue signature is measurable: over a 2-hour 2-back task, performance, self-reported attention-to-task and stimulus-evoked pupil dilation all declined together — the pupil tracks the noradrenergic drive going flat. doi 10.1093/geroni/igaf122.3817 · PMID 40412462 · PMC5522184
Motivation balances it.
D1 receptor stimulation in prefrontal cortex produces an INVERTED-U dose response, whereby either too little OR too much D1R stimulation impairs spatial working memory (PMID 17277774). A meta-analysis reviewed 646 papers and included 75 to quantify it (PMC9364670). The physiology says what each end costs you: low-dose D1 blockade produces noisy firing patterns; optimal D1 stimulation gates out noise but leaves signals intact; high-dose D1 stimulation suppresses all cell firing. Too little and you cannot separate signal from background; too much and there is no signal left to separate. And the peak moves with the task, which is why NO single direction is correct: low levels of D1 sculpting may be optimal for creative thinking when a broad range of inputs ARE needed, while moderate levels may be optimal for tasks requiring precise focus. Stress pushes you off the peak in the impairing direction: high levels of norepinephrine and dopamine released during stress suppress neuronal firing and impair prefrontal function, through alpha-1 and D1 receptors respectively. And region matters: manipulating dopamine has paradoxical consequences for distinct cognitive control processes, depending on the basal or optimal level in different brain regions — prefrontal cortex and striatum do not share an optimum, so a systemic change cannot be optimal for both at once. Memory chemical increases it.
Noradrenaline and dopamine tune gain and are drawn as balance. Acetylcholine is permissive — it is what allows the networks to connect at all, so it is drawn as raise. Cortical acetylcholine released from cells in the basal forebrain facilitates CUE detection and improves attentional performance — cue detection is the elementary operation sustained attention is built from. Human structural evidence in a non-clinical cohort: volumes of the CH4 basal-forebrain cell group (nucleus basalis of Meynert) were measured by MRI in 38 people from a population-based cohort and tested against the Trail-making test and the Stroop task — standard attention and executive measures, not a diagnostic instrument. The permissive role, stated mechanistically: in deep sleep we may be unconscious due to the absence of acetylcholine stimulation of the nicotinic alpha-7 receptor, which is permissive for NMDA actions; when awake, cholinergic stimulation allows networks to connect. Without it there is nothing for gain-tuning to act on — which is also why this edge points at sleep as an upstream dependency. PMC4834944
Sleep increases it.
The dose-response is clean, in 48 healthy young adults, in a laboratory, for 14 days: participants were randomized to 4, 6 or 8 hours time in bed, against a group given 3 days of total sleep deprivation, with a 10-minute psychomotor vigilance test administered repeatedly. At 8 hours, lapses of attention were relatively rare and stable. At 6 and 4 hours there was a steady build-up of vigilant attention deficits in a sleep dose-response manner — and impairment in the 4-hour condition reached levels equivalent to two to three days of acute total sleep deprivation. Two weeks at four hours performs like two days awake. And here is the part that matters most: the subjective signal decouples from the objective one. There was NO steady build-up of self-reported sleepiness and NO systematic dose-response effect on it — subjective sleepiness in the 4- and 6-hour conditions stabilized at a much lower level than in the total-deprivation group. Performance kept falling; the feeling of impairment did not. You cannot detect your own attention deficit on chronic short sleep, which means self-report is worthless as a guide and anyone judging a focus product by how alert they feel is reading the wrong instrument. The mechanism was then seen directly in human neurons: intracranial electrodes in neurosurgical patients performing a vigilance task after a night of sleep deprivation showed selective neuronal lapses preceding the behavioral lapses, with regional sleep-like slow and theta waves occurring during wakefulness. Parts of the brain go offline while the person is awake and does not know it. doi 10.1038/nm.4433 · PMID 31176308
Sleep pressure reduces it.
Adenosine is not a peripheral addition — it sits upstream of all four other drivers of focus. The proof is the most widely self-administered drug on earth: caffeine exerts its effects by blocking adenosine receptors — the only likely mechanism at physiological concentrations — and following low to moderate doses, alertness, vigilance, attention and reaction time improve. Every cup of coffee is an uncontrolled experiment confirming this link. Why it is a hub and not a detail: adenosine inhibits the release of many neurotransmitters in the central nervous system — A1 receptor agonists inhibit release of glutamate, serotonin, acetylcholine, norepinephrine and dopamine. Three of those five are the other drivers of focus. Adenosine does not sit beside them; it sits on them. The same literature states the limit on caffeine: caffeine increases energy metabolism throughout the brain but simultaneously decreases cerebral blood flow, inducing a relative brain HYPOPERFUSION; and effects are less consistent on memory and higher-order executive function such as judgment and decision making than on simple vigilance. It sharpens the watchman, not the strategist. doi 10.1016/j.neubiorev.2016.09.001
Lowers wake signal increases it.
Orexin is the wake signal, and it is upstream of every other driver of this pathway: orexin projections excite wake-promoting systems, including locus coeruleus norepinephrine, dorsal raphe serotonin, tuberomammillary histamine and basal FOREBRAIN/BRAINSTEM acetylcholine neurons — and the LC expresses orexin receptor 1 specifically. It also projects to the VTA. Noradrenaline, acetylcholine and dopamine are three of the five drivers of focus on the map; orexin sits on all three. Causal, and fast: orexin neurons are more active during wake than sleep, fire just before waking, and cause waking within several seconds when optogenetically stimulated. Orexin administration lengthens and orexin disruption shortens continuous periods of wakefulness. Human loss-of-function settles necessity: a selective loss of orexin neurons is found in narcolepsy, in humans, dogs and rodents — remove these neurons and wakefulness cannot be held. And it opens a further dimension: orexin neurons ARE regulated by peripheral metabolic cues including ghrelin, leptin and glucose concentration, providing a link between energy homeostasis and arousal states. Attention has a fuel dependency that the neurotransmitters alone could never have shown. Inputs from the LIMBIC system also regulate orexin neurons to evoke emotional arousal and fear-related responses; that possible orexin-anxiety link is not drawn here. doi 10.3389/fphys.2017.00357 · PMC4335648
Default mode network increases it.
In non-clinical subjects, reduced deactivation of the precuneus — the posterior default mode hub — preceded attentional lapses during a goal-oriented task (Weissman 2006). Two independent literatures, two different methods, same sequence: the brain disengages before the person notices. the dmn is not a fault to be suppressed — it is active at rest by design and the measure that matters is whether it attenuates on cue, which is why this edge is drawn on suppression and not on the network. Mitochondrial energy increases it.
The split of labor is the finding, and it is drug-resolved: DA may be more essential to attaching attention; NE may contribute more to executive function. Methylphenidate blocks DA reuptake; amphetamines block reuptake and increase production of both DA and NE, with the noradrenergic effects contributing to increased energy and enhanced executive function; atomoxetine acts primarily on NE with secondary effects of increasing prefrontal cortical DA. Four drugs, four profiles, one dissociation. And the reviewers refuse the simple story: a hypothesis suggesting too much OR too little of a single neurotransmitter will NO longer suffice. Anything selling a dopamine-deficit account of attention is selling what the field explicitly abandoned. Micronutrient status increases it.
Iron is the rate-limiting cofactor for tyrosine hydroxylase in catecholamine synthesis, and the reviews place ADHD’s causes squarely in brain catecholamines. The cofactor role is established biochemistry but NO retrieved source measures iron repletion improving attention as an endpoint - and this is the same bar that refused the Iron Status → Hair Greying link. Shown as weaker evidence: just as graying needed a repletion study with a pigment endpoint, this needs one with an attention endpoint. Named among the mechanisms affecting high-demand cortical neurons. The weakest of the six drawn here: the catecholamine reviews retrieved do not measure oxidative stress against attention endpoints. 10.1007/s10802-017-0318-1 — This is what the symptom costs rather than what it causes physiologically. It is shown at reduced weight because no separate effect size has been reported for this arm. The asymmetry is deliberate and stated. Sustained attention capacity and mood co-vary. Weaker evidence - association from the shared catecholamine architecture rather than a measured effect. This link rests on the reviews of this outcome as a whole, and no effect size specific to it has been reported. doi 10.1007/s10802-017-0318-1
