You want it in balance: healthy noradrenergic tone sustains vigilance and steady mood.

In depth

The arousal dial. Noradrenaline decides how sharply the world comes into focus — too little and attention drifts, too much and it narrows into anxiety. Performance sits on an inverted U, and the peak is narrower than most people assume.

What drives it

Sympathetic drive increases it.

Noradrenergic-synapse physiology (StatPearls) — norepinephrine is the principal neurotransmitter released by sympathetic postganglionic neurons onto effector organs (the sympathetic fight-or-flight response). NBK540977. Norepinephrine is the sympathetic effector: plasma norepinephrine is the most widely reported marker of sympathetic nervous system activity, and regional norepinephrine spillover together with microneurographic recording of sympathetic nerve traffic are the reference techniques for measuring sympathetic neural function. NE acts through multiple adrenoceptor subtypes with different and sometimes opposing consequences — alpha-1 and alpha-2 drive vasoconstriction, beta-2 drives renin release, beta-1 modulates renal sodium transporters. 'Raising noradrenergic tone' is not one action.

Feeding drive reduces it.

NPY is colocalized and co-released with norepinephrine in most sympathetic nerves and in brainstem noradrenergic cell groups — and it inhibits the transmitter it travels with. NPY inhibits the firing rate of locus coeruleus neurons and inhibits release of norepinephrine through the presynaptic Y2 receptor, and potentiates norepinephrine's own inhibitory effect on LC neurons via Y2. A built-in brake on the system that carries it. The release rule is a duration rule, which matters for a stress atlas: NPY co-stored with norepinephrine is preferentially released by intense and prolonged sympathetic activation — it is not a first-response signal, it arrives when the alarm has been running a while. The same co-transmission is not benign everywhere: in 78 patients after ST-elevation myocardial infarction, higher venous NPY predicted sustained VT or VF despite beta-blockade, with a threshold of 27.3 pg/mL (sensitivity 0.83, specificity 0.71) — the peptide beta-blockers do not touch (PMC3602986; PMC4641560; doi 10.1093/eurheartj/ehz852). A peri-LC NPY population exerts inhibitory control over the LC via NPY-Y1 receptor signaling, giving bidirectional control of anxiety-like behavior in naive animals. Stress recruits the brake: stressful experiences recruit peri-LC NPY neurons with local NPY release measured in vivo, and enhanced peri-LC NPY activity curbs anxiety after stress. The authors state they establish a causal role — this is no longer inference from receptor pharmacology. doi 10.1093/eurheartj/ehz852 · PMC3602986

Endocannabinoid tone increases it.

They accelerate it: systemic administration of the CB1 agonist WIN55,212-2 increases the firing frequency of noradrenergic neurons and enhances forebrain norepinephrine release, acute intravenous WIN55212-2 or delta-9-THC raised locus coeruleus firing rate while the CB1 antagonist SR141716A had the opposite effect, and blocking anandamide breakdown with URB597 during acute swim stress increased norepinephrine release in the prefrontal cortex and basolateral amygdala. Agonist up, antagonist down — both directions tested. And a genuine tension is recorded rather than smoothed: CB1 deletion also increased LC-NE excitability and input resistance, and CB1 loss leads to enhanced age-related neuronal loss in the locus coeruleus. Acute pharmacological activation and lifelong genetic absence do not give mirror-image results, so this edge should not be read as a simple dial. doi 10.3389/fnmol.2017.00411 · PMC7792602

Calm reduces it.

10.1177/0022034513505771 — ,the central amygdala provides substantial GABAergic projections to the locus coeruleus — a direct anatomical brake from the fear nucleus onto the noradrenergic alarm, with a large number of CeA neurons projecting to the LC. The routing is segregated, which matters: a large number of CeA neurons project to the LC whereas only a small number of medial amygdala cells project to the dorsal raphe, and none of the CeA OR MeA cells provided dual projections to both — the amygdala addresses norepinephrine and serotonin through separate cell populations, not one broadcast (S0006899318301732). doi 10.1177/0022034513505771

Sleep pressure reduces it.

10.1523/JNEUROSCI.20-20-07760.2000 — hub edge,Orexin projections excite locus coeruleus noradrenergic neurons, and the LC expresses orexin receptor 1 specifically. Lesion evidence confirms the arm carries traffic: ablating LC noradrenergic neurons attenuated the effect of a dual orexin-receptor antagonist on REM sleep. doi 10.1523/JNEUROSCI.20-20-07760.2000

Cold exposure response increases it.

Verbatim: 'Plasma norepinephrine concentrations were likely to very likely increased after PBC (+57.4%) and WBC (+76.2%), respectively.' (10.1371/journal.pone.0072658). Tier: human randomized trials. human · single 3-min whole-body (WBC) or partial-body (PBC) cryostimulation session · n=15 WBC, n=15 PBC, n=10 non-exposed control · parallel-group controlled trial, magnitude-. doi 10.1371/journal.pone.0072658

Monoamine breakdown reduces it.

Noradrenaline is a substrate of MAO-A; MAO-A is highly expressed in noradrenergic neurons such as the locus coeruleus. Degradative, therefore inhibitory. StatPearls NBK539848 — Monoamine Oxidase Inhibitors ; Sci Rep 2022, PMC8742005 — Phenethylamine is a substrate of monoamine oxidase B in the paraventricular thalamic nucleus. PMC8742005

Dopamine clearance reduces it.

10.5455/cap.20131025120419 — Nbk540977 — the enzyme that shreds the signal — and the actual mechanism behind one of this formula's entries. 'Le plus important est l'inhibition de la CATÉCHOL-o-MÉTHYLE-TRANSFÉRASE (comt) par l'EGCG' = the most important action is the inhibition of catechol-o-methyltransferase by the green-tea catechin (Médecine & Nutrition — Les récepteurs adrénergiques de l'adipocyte — FRANÇAIS). ⇒ this explains the condition already on the green tea card: that it works 'in the presence, but not the absence, of noradrenaline'. It does not create the signal. It stops the signal being destroyed. NO signal, nothing to preserve — which is why the timing rule is not optional. doi 10.5455/cap.20131025120419

What it drives

It increases Mood.

Norepinephrine/mood-disorders review 1999 — locus coeruleus-norepinephrine dysregulation contributes to depression and is a target of antidepressants. PMID 10560027. A systematic umbrella review — the highest level of evidence synthesis — concluded there is NO consistent evidence of an association between serotonin and depression (Moncrieff, Cooper, Stockmann, Amendola, Hengartner & Horowitz 2022, Molecular Psychiatry, doi 10.1038/s41380-022-01661-0). The specifics: a meta-analysis of plasma serotonin showed NO relationship with depression (n=1,869) and found that lowered serotonin was associated with antidepressant use; 5-HT1A receptor-binding (largest n=561) and SERT-binding (largest n=1,845) meta-analyses showed weak and inconsistent evidence with prior antidepressant use not reliably excluded; and tryptophan depletion produced NO effect in most healthy volunteers (n=566), with weak evidence only in those with a family history (n=75). Separately, a meta-analysis of monoamine-depletion studies concluded monoamines alone ARE not sufficient to cause depression and depression has NO direct causal relation with monoamine depletion (Ruhe 2007, Mol Psychiatry 12:331). The dispute is live and both sides ARE cited here: Jauhar and colleagues replied under the title 'A leaky umbrella has little value: evidence clearly indicates the serotonin system is implicated in depression' (doi 10.1038/s41380-023-02095-y), and methodological critiques argue Moncrieff et al. mixed study types inappropriately and used unclear primary-study selection criteria (PMC9957868). One finding both sides accept: depletion did lower mood in patients with MDD in remission — the system matters in the vulnerable, not in everyone. doi 10.1038/s41380-022-01661-0 · PMID 10560027 · PMC9957868

It increases Calm under stress.

NBK441870 — Noradrenaline is the fight-or-flight transmitter and is implicated across anxiety disorders; SNRIs sit alongside SSRIs in first-line guidance. A provocation test shows the receptor is altered, not just the level: clonidine, an alpha-2 agonist, normally stimulates growth-hormone release, and the growth-hormone response to clonidine is blunted in GAD patients. The source states its own ambiguity: it is not yet known whether this reflects alpha-2 receptor down-regulation from chronic central noradrenergic overactivity, or a different abnormality as in depression. The finding is solid; its interpretation is not settled.

It increases Mood (serotonin).

10.1038/s41386-024-01824-3 — ,In vivo the levels of norepinephrine and serotonin in the dorsal raphe covary positively, because noradrenergic input drives the serotonin neurons to fire action potentials and release serotonin via Gq-coupled alpha-1 adrenergic receptors. Channelrhodopsin circuit mapping shows sufficiency: activation of locus-coeruleus-derived noradrenergic axon terminals was sufficient to produce an alpha-1-receptor epsc in dorsal raphe serotonin neurons. Presynaptic brake: alpha-2 adrenergic receptors inhibit that epsc by cutting norepinephrine release. doi 10.1038/s41386-024-01824-3

It balances Sustained focus.

Focus here is evidenced on normal-range attention in healthy people, not on trials in diagnosed populations. Human, healthy, preregistered: in 63 adults over 50 from the Cambridge Center for Aging and Neuroscience cohort — none with a diagnosis of dementia or mild cognitive impairment, all scoring above consensus thresholds for normal cognition — better response inhibition on a stop-signal task correlated with locus coeruleus integrity measured by magnetisation-transfer MRI, and with connectivity between pre-supplementary motor area and right inferior frontal gyrus, but not with the volumes of those prefrontal regions. The nucleus, not the cortex it drives. And it holds at autopsy: neuronal density in the locus coeruleus was linked to higher baseline functioning and slower cognitive decline before death, which is why the noradrenergic system is proposed as a component of cognitive reserve. Why this link is drawn as balance and not as raise: the LC operates by adaptive gain with distinct tonic and phasic modes, and the relationship to performance is an inverted U. Cognitively healthy older adults sustain tonic LC hyperactivity, and the consequence is worse attention, not better: arousal elicited weaker behavioral responses, weaker pupil dilation and weaker P300 responses in older adults than young. A system already running hot cannot mount a phasic response to what matters. The practical reading: raising noradrenergic tone is not the same as improving focus, and in an already-elevated person it is the opposite. The fatigue signature is measurable: over a 2-hour 2-back task, performance, self-reported attention-to-task and stimulus-evoked pupil dilation all declined together — the pupil tracks the noradrenergic drive going flat. doi 10.1093/geroni/igaf122.3817 · PMID 40412462 · PMC5522184

It reduces Attention regulation.

In the prefrontal cortex, where DAT density is low and NET density is higher, it is unlikely that blockade of the dopamine transporter is a significant contributor to elevated dopamine there — dopamine is transported by the norepinephrine transporter in the prefrontal cortex, because dopamine has a higher affinity for NET than for DAT. In the region that matters most for attention, the dopamine system is cleared by the norepinephrine machinery. That is why a purely noradrenergic drug raises prefrontal dopamine. The noradrenergic case is also clinical: atomoxetine, a highly selective noradrenergic reuptake inhibitor, is clinically effective in ADHD — efficacy without any direct dopaminergic action. And imaged in patients: 20 adults with ADHD and 20 healthy controls underwent PET with the NET radioligand (S,S)-[18F]FMeNER-D2 alongside genotyping, finding genotype-dependent differences in NET binding potential between groups in thalamus and cerebellum. The function it serves is signal-to-noise: norepinephrine is described as improving attention by increasing the signal-to-noise ratio of dopamine-containing neurons in the prefrontal cortex. Candidate-gene work largely failed and is recorded as such: family-based and case-control analyses of the SLC6A2 -3081(A/T) and G1287A polymorphisms found NO significant association with ADHD, and the null for G1287A was independently reproduced. It increases Sleep hormone.

NBK550972 — ,Noradrenaline is the alerting transmitter here, driving the Noradrenergic Tone → Sustained Attention link and the Noradrenergic Tone → ADHD link. It is also what makes melatonin. Norepinephrine, through activation of beta receptors, stimulates the synthesis of melatonin from serotonin in the pineal gland, which is characterized by a rich noradrenergic innervation. The suprachiasmatic clock controls the release of norepinephrine from the dense pineal sympathetic afferents at night. Mechanism to the enzyme: cAMP/PKA signaling phosphorylates AANAT, the rate-limiting enzyme of melatonin synthesis. And the off-switch is the same lever: during daylight, when norepinephrine stimulation ceases, degradation of that enzyme occurs rapidly. So a single transmitter runs both daytime alertness and night-time melatonin synthesis, in different tissues, on different schedules.

It reduces Sleep.

The noradrenergic locus coeruleus is a core wake-active nucleus of the ascending arousal system, sending arousing projections to thalamus and neocortex. The GABAergic VLPO neurons inhibit both the orexin system and the ascending arousal system; the arousal nuclei inhibit the VLPO back. A flip-flop has no comfortable middle — which is why sleep onset is abrupt rather than gradual, and why these arrows are drawn inhibitory as a class. And the causal test is cell-type-specific: ablating locus coeruleus norepinephrine neurons with SAPORIN conjugates attenuated the promotion of REM sleep by a dual orexin antagonist. That same experiment shows the the LC ablation affected REM only, and neither lesion altered the efficacy of ZOLPIDEM on any sleep measure. This edge is about the orexin/monoamine arm of arousal, not all sedation. PMC4801942

It reduces Supports weight management.

The anatomy is named: PHENTERMINE is a noradrenergic drug which stimulates the release of norepinephrine and reduces food intake by acting on beta-adrenergic receptors in the perifornical hypothalamus. Sibutramine, inhibiting reuptake of both serotonin and norepinephrine, produced 4.5 kg of weight loss. Its failure mode was cardiovascular rather than VALVULAR: sibutramine suppresses appetite and increases heart rate and blood pressure, and in the sibutramine cardiovascular outcomes trial an increase in major adverse cardiovascular events prompted its withdrawal in Europe and the united states. The appetite benefit and the pressor harm are the same pharmacology read at two outcomes — structurally identical to the rimonabant collision on the Endocannabinoid Signaling → Weight & Body Fat link. The pattern across this pathway is the point: every centrally-acting appetite SUPPRESSANT that produced meaningful weight loss also produced a serious off-target harm, and each failed in a different organ — serotonergic (fenfluramine: heart valve damage), mixed serotonin-norepinephrine (sibutramine: major adverse cardiovascular events in SCOUT), and endocannabinoid (rimonabant: psychiatric disorders). And none changed the SETPOINT: patients were unable to retain weight loss once therapy was discontinued. PMC3083904

It increases Sexual function.

Norepinephrine is the primary adrenergic transmitter in the penis and controls penile DETUMESCENCE by inducing penile smooth muscle contraction. The default state is contracted - erection is the removal of a brake, not the addition of a push, which is why sympathetic arousal opposes it. doi 10.21037/tau-2025-419 · PMID 19138376 · PMC11988752

It increases Female libido.

Named across five reviews of erectile and female arousal physiology. Key contributors include reduced nitric oxide bioavailability due to endothelial oxidative stress and inflammation, diminished cavernosal smooth muscle content leading to impaired veno-occlusion, and increased arterial stiffness compounded by metabolic disorders. doi 10.21037/tau-2025-419 · PMID 19138376 · PMC11988752

It increases Women's hormone balance.

The genitalia and lower urinary tract share common estrogen receptor function. The receptor change is the estrogen receptors alpha and beta ARE present in the vagina, vestibule, urethra and bladder TRIGONE - while both receptors can be found in premenopausal women, only estrogen-beta receptors can be detected in postmenopausal women. The tissue does not merely see less hormone; it loses a receptor subtype. And a pH loop with a clinical endpoint: estrogen maintains vaginal glycogen, lactobacilli convert it to lactic acid holding pH at 3.5-5.0; loss of estrogen is responsible for the reduction of Lactobacillus, changing the vaginal fluid to an alkaline pH of 5.0 OR above, which promotes overgrowth of gram-negative rod fecal flora, inducing vaginal infection and UTI. Scale and underdiagnosis: GSM affects 40-54 percent of postmenopausal women and 15 percent of premenopausal women, and around 60 percent of these women have never had a diagnosis of GSM before (NBK559297; PMC10692865; PMC7212735; PMC11595908; doi 10.3389/frph.2021.779398). It modulates Calm pain signaling.

The defining feature is that the pain is real and the tissue is not damaged: central sensitization manifests as hyperalgesia, allodynia, expanded receptive fields, and impaired endogenous pain inhibition, and in osteoarthritis the same phenomenon does not correlate with radiographic findings - severe pain may persist despite stable joint structure. The mechanism is a balance failure in both directions at once: in the dorsal horn there is excessive release of glutamate, ATP, and substance P, leading to activation of NMDA receptors - while at the same time descending inhibitory mechanisms, notably those mediated by GABA, ARE weakened. Too much excitation and too little inhibition. And mast cells carry a specific hypothesis: thalamic mast cells releasing histamine, IL-1-beta, IL-6, TNF, CGRP and substance P, either stimulating thalamic nociceptive neurons directly or via microglia. Stated by the authors as a hypothesis, not an established mechanism. The reviewers ARE blunt about the state of knowledge: the pathophysiology and etiopathogenesis remain largely unknown, and there is NO definitive pathogenesis except for altered central pain pathways. doi 10.3390/biomedicines12071543 · PMC6687840
