Attention regulation

Attention-deficit/hyperactivity disorder (ADHD)

The Clearance Problem

1 blend on this site modulates it.

Also searched as Attention-deficit/hyperactivity disorder (ADHD).

This pathway is Built on Evidence from Diagnosed Populations and is deliberately separate from the focus node, which is built on healthy-cohort attention data. The two are not interchangeable and neither inherits the other's evidence. The catecholamine story is about clearance, not production.

In depth

Attention-deficit/hyperactivity disorder — a developmental condition of inattention, hyperactivity and impulsivity with high heritability.

What drives it

Motivation reduces it.

Focus is evidenced in healthy people; this link is evidenced in patients, and the two are kept apart. The finding is about clearance, not production: patients with ADHD have depleted levels of dopamine and norepinephrine thought to be largely the result of dysfunction of their respective transporter systems, with a higher density of the dopamine transporter reported in patients — more pumps removing it from the synapse, not less being made. Three drug classes, one converging endpoint, which is the strongest argument in this literature: DAT-selective (METHYLPHENIDATE), DAT/NET-SELECTIVE (amphetamine) and NET-selective (atomoxetine) medications have one pharmacological effect in common — to elevate extracellular dopamine and norepinephrine in the prefrontal cortex. Different molecules, different transporters, same destination. And the genetics disappoint, recorded honestly: heritability is high, but studies have failed to indicate a single gene responsible, implying complex polygenic mechanisms and gene-environment interactions. The DAT1 3-prime UTR VNTR is a candidate marker for executive-function performance, not a cause. doi 10.1007/s44192-022-00030-1 · PMID 18480676 · PMC2443797

Alertness chemical reduces it.

In the prefrontal cortex, where DAT density is low and NET density is higher, it is unlikely that blockade of the dopamine transporter is a significant contributor to elevated dopamine there — dopamine is transported by the norepinephrine transporter in the prefrontal cortex, because dopamine has a higher affinity for NET than for DAT. In the region that matters most for attention, the dopamine system is cleared by the norepinephrine machinery. That is why a purely noradrenergic drug raises prefrontal dopamine. The noradrenergic case is also clinical: atomoxetine, a highly selective noradrenergic reuptake inhibitor, is clinically effective in ADHD — efficacy without any direct dopaminergic action. And imaged in patients: 20 adults with ADHD and 20 healthy controls underwent PET with the NET radioligand (S,S)-[18F]FMeNER-D2 alongside genotyping, finding genotype-dependent differences in NET binding potential between groups in thalamus and cerebellum. The function it serves is signal-to-noise: norepinephrine is described as improving attention by increasing the signal-to-noise ratio of dopamine-containing neurons in the prefrontal cortex. Candidate-gene work largely failed and is recorded as such: family-based and case-control analyses of the SLC6A2 -3081(A/T) and G1287A polymorphisms found NO significant association with ADHD, and the null for G1287A was independently reproduced. Default mode network reduces it.

PMC11325164 — Beyond chemistry (transporters, catecholamines), network timing. The default mode interference hypothesis proposes that inattentiveness in ADHD may be caused by insufficient down-regulation of the default mode network during goal-oriented processes — the inward-facing network fails to stand down when a task begins, and reduced suppression may disrupt task-positive networks and lead to lapses of attention and excessive mind-wandering. Mega-analysis of multiple large samples, with the drug confound excluded: ADHD diagnosis was associated with less anticorrelation between the default mode and the dorsal attention (d=0.17, p-FDR<0.001), SALIENCE/VENTRAL attention (d=0.14) and SOMATOMOTOR (d=0.14) networks — and these results were robust to sensitivity analyses considering comorbid internalizing problems, externalizing problems and PSYCHOSTIMULANT medication. Not a medication artefact. The effect sizes ARE small and must travel with the d = 0.14 to 0.17, and for ADHD traits the largest was partial-r = 0.06. Statistically robust across big samples; nowhere near a diagnostic. And the direction is disputed: studies ARE inconsistent about whether ADHD shows hyperconnectivity OR hypoconnectivity of the DMN — which is why this edge is drawn on the anticorrelation (the failure to attenuate) rather than on connectivity magnitude. Sleep reduces it.

The numbers ARE not marginal: insomnia and sleep disturbances affect up to 80 percent of adults with ADHD and up to 82 percent of children. Delayed sleep-wake timing occurs in up to 78 percent, and delayed sleep phase disorder is reported in 73-78 percent of children and adults with the diagnosis. Up to 44 percent have restless legs syndrome or RLS-like symptoms. And the direction is unresolved, which is recorded rather than papered over: ADHD is either the cause OR the consequence of sleep disturbances, OR they may have a shared aetiological and genetic background. Note also: sleep disruption itself may produce ADHD-like symptoms, and daytime RLS features such as restlessness and inattention mimic ADHD — so some of the diagnosed population may be carrying a sleep disorder wearing an attention label. doi 10.1007/s12402-018-0271-z · PMC12728042

Sleep hormone reduces it.

DIM-light melatonin onset is delayed by approximately 45 minutes in children and 90 minutes in adults with ADHD, with up to 75 percent of adults with childhood-onset ADHD showing delayed DLMO. This is a clock reading, not a questionnaire. It travels with other rhythm markers: blunted and delayed cortisol rhythms, reduced pineal volume, and attenuated peripheral clock-gene rhythms (BMAL1, PER2), with genetic studies implicating per and clock polymorphisms. And it is the only arm on this pathway with a non-drug intervention signal: melatonin and bright light therapy have advanced DLMO in both children and adults with ADHD, and emerging data correlate phase advancement with ADHD symptom improvement. Scope, stated by the authors: that correlation is emerging, and the proposed clinical pathway is behavioral-first — fixed wake times, morning bright light, evening light restriction — with melatonin selective and low-dose for confirmed or probable DLMO delay. It is a phase-shifting tool, not a sedative, and not a treatment for the disorder. PMC12728042

Body clock reduces it.

Distinct from the Sleep → ADHD link and the Melatonin → ADHD link because the measurements are of the clock itself. Delayed sleep phase disorder is present in 73-78 percent of children and adults with ADHD, and delayed sleep-wake timing in up to 78 percent. The molecular clock is measurably altered: attenuated peripheral clock-gene rhythms of BMAL1 and PER2, alongside blunted and delayed cortisol rhythms and reduced pineal volume. Genetics: clock-gene polymorphisms including per and clock are linked to ADHD. Same direction caveat as the Sleep → ADHD link: ADHD is either the cause OR the consequence of these disturbances, or they share aetiology. The arrow is drawn this way because the intervention evidence — bright light and melatonin advancing DLMO with correlated symptom improvement — runs this way. PMC12728042

Memory chemical reduces it.

Cholinergic arousal. MODAFINIL may enhance attention predominantly by increasing arousal and alertness in patients with low-energy ADHD - a separable arousal route distinct from the catecholamine one. Mitochondrial energy reduces it.

The split of labor is the finding, and it is drug-resolved: DA may be more essential to attaching attention; NE may contribute more to executive function. Methylphenidate blocks DA reuptake; amphetamines block reuptake and increase production of both DA and NE, with the noradrenergic effects contributing to increased energy and enhanced executive function; atomoxetine acts primarily on NE with secondary effects of increasing prefrontal cortical DA. Four drugs, four profiles, one dissociation. And the reviewers refuse the simple story: a hypothesis suggesting too much OR too little of a single neurotransmitter will NO longer suffice. Anything selling a dopamine-deficit account of attention is selling what the field explicitly abandoned. Micronutrient status reduces it.

Iron is the rate-limiting cofactor for tyrosine hydroxylase in catecholamine synthesis, and the reviews place ADHD’s causes squarely in brain catecholamines. The cofactor role is established biochemistry but NO retrieved source measures iron repletion improving attention as an endpoint - and this is the same bar that refused the Iron Status → Hair Greying link. Shown as weaker evidence: just as graying needed a repletion study with a pigment endpoint, this needs one with an attention endpoint. Oxidative stress increases it.

Named among the mechanisms affecting high-demand cortical neurons. The weakest of the six drawn here: the catecholamine reviews retrieved do not measure oxidative stress against attention endpoints. Drawn on the shared prefrontal-metabolism argument, which is an inference. doi 10.3389/fncir.2014.00033 · PMC4033238

Sympathetic drive increases it.

The peripheral epinephrine system may be a critical factor in the response of individuals with ADHD to stimulant medication, and MODAFINIL may enhance attention predominantly by increasing arousal and alertness in patients with low-energy ADHD. Drawn activating on the arousal arm; the relationship is almost certainly an INVERTED-U rather than monotonic, and no retrieved source gives the shape. doi 10.3389/fncir.2014.00033 · PMC4033238

Omega-3 status reduces it.

All nine included studies reported a negative correlation between vitamin D level and chronic rhinosinusitis; vitamin D deficiency also causes airway hyperresponsiveness. doi 10.7759/cureus.55955 · PMC4002254

What it drives

It reduces Mood.

10.1371/journal.pone.0277175 — 10.3389/fpsyt.2025.1597559 — This is what the symptom costs rather than what it causes physiologically. It is shown at reduced weight because no separate effect size has been reported for this arm. The asymmetry is deliberate and stated. Comorbid mood difficulty is consistently reported alongside ADHD in the reviews. Association, not established direction. This link rests on the reviews of this outcome as a whole, and no effect size specific to it has been reported. doi 10.1371/journal.pone.0277175

It reduces Sleep.

19625983 — 10.3390/brainsci11101361 — This is what the symptom costs rather than what it causes physiologically. It is shown at reduced weight because no separate effect size has been reported for this arm. The asymmetry is deliberate and stated. Sleep disturbance is consistently comorbid. Direction contested - the Sleep → ADHD link is also drawn and may be the stronger arm. doi 10.3390/brainsci11101361
