In depth
Cholinergic signaling — the acetylcholine system central to attention, learning and memory.
What drives it
Gut barrier increases it.
Gut-ACh finding 1977, Appl Environ Microbiol 34:237 — gut bacteria such as Lactobacillus plantarum synthesize acetylcholine, contributing to the host cholinergic pool. doi:10.1128/aem.34.2.237-239.1977. Convergent: acetylcholine is the most common cholinergic neurotransmitter in the intestinal tract, synthesized from choline and acetyl-CoA by choline acetyltransferases and released by sympathetic preganglionic and parasympathetic pre- and postganglionic fibers. Microbial ACh production was first identified in L. plantarum. much of the intestinal ACh is host-derived from enteric neurons; the microbial contribution is a component, not the source. Shared caveats on this whole row: (1) the route to the brain is not free — gut-derived neurotransmitters reach targets via the bloodstream and must cross the blood-brain barrier; elevated plasma 5-HT can itself raise BBB permeability, temporarily letting plasma 5-HT enter the CNS, so the barrier is a variable not a constant. (2) much of this literature is association — the reviews state plainly that the association studies linking microbiota to depression did not provide evidence on aetiology. (3) The enteric nervous system hosts more than 30 neurotransmitters; any single arrow is one strand of a dense web. doi 10.1128/aem.34.2.237-239.1977.
Sleep pressure reduces it.
Hub edge,Adenosine A1 receptor agonists inhibit the release of acetylcholine. doi 10.1016/j.neubiorev.2016.09.001
Lowers wake signal increases it.
Hub edge,Orexin projections excite basal forebrain and brainstem acetylcholine neurons, with dense innervation of the laterodorsal and pedunculopontine tegmental nuclei. Lesion evidence again separates the arms: ablating basal forebrain cholinergic neurons attenuated an orexin antagonist's effect on nrem sleep without affecting REM, while LC ablation did the opposite — the cholinergic and noradrenergic arms carry different parts of the signal. Calm reduces it.
Not a static wire — A receptor population that moves with sleep pressure. The source studies dynamic changes in GABA-A receptors on basal forebrain cholinergic neurons following sleep deprivation and recovery. The inhibitory receptors sitting on the arousal cells are themselves regulated by how long you have been awake. The homeostatic logic is explicit: prolonged activity decreases excitability and prolonged inactivity increases it, mediated by reciprocal plastic changes in inhibitory transmission. This sits directly under two edges already drawn: the GABA → Sleep link and the Cholinergic → Sleep link, and the Cholinergic → Sleep link carries the NREM/REM conditional showing the basal forebrain is where the NREM effect is delivered. This edge names the receptor doing it. PMC1805759
What it drives
It increases Moves everything along.
In a randomized placebo-controlled trial, raising synaptic acetylcholine with the cholinesterase inhibitor neostigmine produced prompt colonic decompression and passage of flatus or stool in 10 of 11 patients versus 0 of 10 on placebo, demonstrating that cholinergic drive accelerates colonic transit. doi 10.1056/NEJM199907153410301 · PMID 10403850
It increases Memory & focus.
Bartus 1982, Science 217:408 — the cholinergic hypothesis: acetylcholine signaling underpins memory; its loss drives cognitive decline. PMID 7046051. Causal in humans, not just correlational: cholinergic antagonists impair memory in animals and humans; scopolamine given to neurosurgical patients during a memory task disrupted the amplitude and timing of hippocampal theta oscillations. Reversible lesion: cholinergic hypofunction from 192-IgG-saporin cut hippocampal Arc and BDNF and worsened spatial acquisition — and GALANTAMINE reversed both the deficit and the Arc/BDNF levels (PMID 20865740). The therapy limit is explicit: a practice-guideline update found NO level a evidence that cholinesterase inhibitors give symptomatic improvement at the MCI stage, while donepezil benefits more severe AD. And the effect is phase-specific: scopolamine disrupted memory encoding, with retrieval a separate question. PMID 7046051
It increases Sustained focus.
Noradrenaline and dopamine tune gain and are drawn as balance. Acetylcholine is permissive — it is what allows the networks to connect at all, so it is drawn as raise. Cortical acetylcholine released from cells in the basal forebrain facilitates CUE detection and improves attentional performance — cue detection is the elementary operation sustained attention is built from. Human structural evidence in a non-clinical cohort: volumes of the CH4 basal-forebrain cell group (nucleus basalis of Meynert) were measured by MRI in 38 people from a population-based cohort and tested against the Trail-making test and the Stroop task — standard attention and executive measures, not a diagnostic instrument. The permissive role, stated mechanistically: in deep sleep we may be unconscious due to the absence of acetylcholine stimulation of the nicotinic alpha-7 receptor, which is permissive for NMDA actions; when awake, cholinergic stimulation allows networks to connect. Without it there is nothing for gain-tuning to act on — which is also why this edge points at sleep as an upstream dependency. PMC4834944
It increases Default mode network.
The question was whether cholinergic tone gates default-mode suppression or merely runs alongside it. It gates. Chemogenetic and causal: DREADDs were used to selectively increase cholinergic activity in the rat basal forebrain with resting-state fMRI before and after, and unilateral activation of right basal forebrain cholinergic neurons produced decreased INTRA- and interhemispheric functional connectivity in the default mode like network. A modeling paper states the relationship in its title: cholinergic modulation supports dynamic switching of resting state networks through selective DMN suppression. And the basal forebrain behaves like the network it modulates: BF neurons show robust activity suppression when animals transition from rest to a novel task environment — mirroring the DMN deactivation seen in humans during externally focused tasks — and BF signaling drives medial frontal cortex activity, especially during rest. Human 7-TESLA fMRI mapped BF involvement across both DMN deactivation and DMN engagement states. This completes a chain already half-drawn here: the Cholinergic → Sustained Attention link is the permissive arm (cue detection, letting networks connect) and the Default Mode Suppression → Sustained Attention link is about attenuation on CUE. This edge shows they are the same act seen at two levels — the cholinergic signal that permits task engagement is the signal that stands the default network down. Two complications recorded: unilateral stimulation produced differential effects between the two hemispheres, so this is not a uniform global dial; and the basal forebrain has itself been suggested to be an inherent part of the DMN, which blurs modulator and member. Rodent chemogenetics with human imaging support. doi 10.1371/journal.pcbi.1012099 · PMID 34974620 · PMC7452182
It increases Sleep.
Conditional, because the experiment that establishes it also splits sleep in two. Cholinergic cells in the basal forebrain and pontomesencephalic nuclei are wake-active. The dissociation is exact: ablation of basal forebrain cholinergic neurons attenuated the orexin-antagonist-induced increase in NREM sleep time without affecting REM sleep, while ablating locus coeruleus norepinephrine OR tuberomammillary histamine neurons attenuated REM promotion without affecting NREM. One drug, two sleep stages, two separate cell populations delivering the effect. A claim about improving sleep that does not say which stage is under-specified. PMC4801942
