Urinary Flow

2 blends on this site modulate it.

You want it easy: healthy urinary flow reflects sound prostate and hormonal balance.

In depth

Lower urinary tract function — flow, frequency, urgency and nocturnal waking. Quietly one of the strongest predictors of sleep quality in men over fifty, which is why it appears here.

What drives it

Testosterone modulates it.

Gormley 1992, NEJM — benign prostatic hyperplasia is androgen-dependent; testosterone converted to DHT via 5α-reductase drives prostate enlargement and urinary-obstruction symptoms, reversed by finasteride. doi:10.1056/NEJM199210223271701. The earlier note here rested on a patent claiming DHT tracks urinary symptoms while blood testosterone does not. A POST-HOC analysis of the randomized reduce trial — 3,009 asymptomatic and 2,145 symptomatic men — found NO evidence that serum testosterone OR serum dihydrotestosterone was related to LUTS incidence in asymptomatic men (P=0.9 and P=0.4) or to LUTS progression in symptomatic men (P=0.9 and P=0.7), with results alike in both placebo and dutasteride arms (all P-interaction >=0.3). Neither serum androgen predicts urinary symptoms (J Urol doi 10.1097/JU.0000000000003738; PMC10842939). Yet blocking DHT works, which is the whole point: 5-alpha-reductase inhibitors reduce serum and intraprostatic DHT — dutasteride by more than 90% — causing involution of prostate tissue, and randomized placebo-controlled trials show reduced LUTS, increased maximum urinary flow, and lower absolute risk of urinary retention and BPH surgery over 24 months (PMID 14627182; PMC5407420; finasteride: prostate volume -18% vs +14% on placebo over 4 years). The reconciliation is the circulating androgen level is not the variable — intraprostatic DHT and tissue volume ARE. A blood test does not predict who has symptoms; lowering DHT inside the gland still relieves them. Implication for this edge: it should be read as running through prostate tissue DHT, not through serum testosterone. doi 10.1056/NEJM199210223271701. · PMID 14627182 · PMC10842939

Blood flow (nitric oxide) increases it.

The same NO/cGMP pathway relieves the urinary symptoms, which is why one drug class treats both: PDE5 inhibition and the consequent change in NO/cGMP signaling relaxes prostatic smooth muscle and reduces urethral pressure, and lower urinary tract symptoms secondary to benign prostatic hyperplasia is now an approved indication for PDE5 inhibitors (PMC4563010, PMC6003652). The overlap is not coincidence — in men scheduled for surgical management of LUTS/BPH, 36% with moderate and 94% with severe symptoms also had erectile dysfunction. PMC4563010

Estrogen balance increases it.

The genitalia and lower urinary tract share common estrogen receptor function. The receptor change is the estrogen receptors alpha and beta ARE present in the vagina, vestibule, urethra and bladder TRIGONE - while both receptors can be found in premenopausal women, only estrogen-beta receptors can be detected in postmenopausal women. The tissue does not merely see less hormone; it loses a receptor subtype. And a pH loop with a clinical endpoint: estrogen maintains vaginal glycogen, lactobacilli convert it to lactic acid holding pH at 3.5-5.0; loss of estrogen is responsible for the reduction of Lactobacillus, changing the vaginal fluid to an alkaline pH of 5.0 OR above, which promotes overgrowth of gram-negative rod fecal flora, inducing vaginal infection and UTI. Scale and underdiagnosis: GSM affects 40-54 percent of postmenopausal women and 15 percent of premenopausal women, and around 60 percent of these women have never had a diagnosis of GSM before. doi 10.3389/frph.2021.779398 · PMC10692865

It increases Prostate health.

The finding that unifies all three nodes is that the same androgen does opposite things in different tissues: testosterone acts directly, and indirectly through intracellular conversion to dihydrotestosterone by 5-alpha reductase, as in prostate cells, and separately through estrogen effect when converted to estradiol by aromatase. Raise testosterone and you also raise DHT and estradiol - which is why the same intervention supports male hormonal health and can enlarge the prostate. And the clinical data shows both arms at once: in 183 men over 50 with BPH, testosterone was significantly lower in patients with severe LUTS (285.94 ng/dL) than mild (544.09 ng/dL) - so low testosterone tracks with worse urinary symptoms, while DHT drives the gland. That is not a contradiction to resolve, it is two different molecules. Bidirectionality is explicit: the relationship between hypogonadism and metabolic disorders is bidirectional, and a vicious circle between the two components has been documented. PMC6669361
