Hormone-binding protein
Sex hormone-binding globulin (SHBG)
Also searched as Sex hormone-binding globulin (SHBG).
You want it high: abundant SHBG sequesters DHT, and because insulin suppresses SHBG, protecting insulin sensitivity is how you keep this buffer full.
In depth
The liver-made carrier protein that binds circulating sex hormones — the buffer that decides how much androgen is free to reach the follicle.
What drives it
Stops fat storage reduces it.
This — not insulin by itself — is the proximate mechanism. Monosaccharides (glucose and fructose) reduce human SHBG production by hepatocytes by downregulating HNF-4α, the transcription factor that drives the SHBG promoter; and the causal step is lipogenesis, because inhibiting lipogenesis prevented the monosaccharide-induced fall in HNF-4α and in SHBG (Selva et al., PMID 17992261). Hepatic insulin resistance and dietary sugar raise monosaccharide delivery to the liver, stimulate SREBP-1c and de-novo lipogenesis, and lower HNF-4α and SHBG with it. PMID 17992261 · PMC7663738
Fasting insulin reduces it.
Clinically the inverse relationship is consistent — low SHBG is a recognized marker of hyperinsulinemia, and interventions that lower basal insulin raise SHBG alongside improved insulin sensitivity and androgen profiles (PMC12829335, PMC7663738). But the honest nuance, stated because it changes what may be claimed: in HepG2 hepatocytes cultured with high glucose, insulin alone did not alter SHBG production OR hnf-4α levels (Selva et al., PMID 17992261). The insulin association appears to run through hepatic lipogenesis rather than through insulin acting directly on the SHBG promoter — so lowering insulin plausibly raises SHBG by removing the lipogenic drive, not by a direct effect. Calms inflammation reduces it.
Inflammation lowers it by the same lever: TNF-α both inhibits expression of the HNF4A gene and suppresses its transcriptional activity via NF-κB, reducing SHBG synthesis in hepatocytes — which is why chronic low-grade inflammatory disease travels with low plasma SHBG. doi 10.1016/j.biopha.2022.113261 · PMC7663738
Phytoestrogen receptor activity increases it.

What it drives
It reduces Blocks the hormone that shrinks follicles.
Free-hormone-hypothesis review 2022, Cell Mol Life Sci — SHBG binds circulating androgens (testosterone and, with even higher affinity, DHT), lowering the free bioactive fraction that reaches the follicle. doi 10.1007/s00018-022-04562-1
It reduces Sexual function.
SHBG does not change how much testosterone you make — it changes how much of it can ACT, which is why this pathway runs to function and not to Total Testosterone. In men roughly 70% of circulating testosterone is bound with high affinity to SHBG, 20-30% to albumin, and only about 1-2% circulates free; SHBG is the principal determinant of the split between bound and free, and the free-hormone hypothesis holds that only the unbound fraction can cross the membrane and act (PMC8318754, Endocrine Reviews PMC6287254). So a high SHBG can produce a functional deficiency on a perfectly normal total-testosterone result — which is exactly why expert panels recommend measuring free testosterone whenever SHBG is disturbed by obesity, diabetes, aging, thyroid or liver disease. It reduces PCOS.
SHBG binds circulating androgen; low SHBG raises free testosterone and is a consistent feature of PCOS. PMC9967593
It increases Women's hormone balance.
The genitalia and lower urinary tract share common estrogen receptor function. The receptor change is the estrogen receptors alpha and beta ARE present in the vagina, vestibule, urethra and bladder TRIGONE - while both receptors can be found in premenopausal women, only estrogen-beta receptors can be detected in postmenopausal women. The tissue does not merely see less hormone; it loses a receptor subtype. And a pH loop with a clinical endpoint: estrogen maintains vaginal glycogen, lactobacilli convert it to lactic acid holding pH at 3.5-5.0; loss of estrogen is responsible for the reduction of Lactobacillus, changing the vaginal fluid to an alkaline pH of 5.0 OR above, which promotes overgrowth of gram-negative rod fecal flora, inducing vaginal infection and UTI. Scale and underdiagnosis: GSM affects 40-54 percent of postmenopausal women and 15 percent of premenopausal women, and around 60 percent of these women have never had a diagnosis of GSM before. doi 10.3389/frph.2021.779398 · PMC10692865

It increases Men's hormone balance.
The finding that unifies all three nodes is that the same androgen does opposite things in different tissues: testosterone acts directly, and indirectly through intracellular conversion to dihydrotestosterone by 5-alpha reductase, as in prostate cells, and separately through estrogen effect when converted to estradiol by aromatase. Raise testosterone and you also raise DHT and estradiol - which is why the same intervention supports male hormonal health and can enlarge the prostate. And the clinical data shows both arms at once: in 183 men over 50 with BPH, testosterone was significantly lower in patients with severe LUTS (285.94 ng/dL) than mild (544.09 ng/dL) - so low testosterone tracks with worse urinary symptoms, while DHT drives the gland. That is not a contradiction to resolve, it is two different molecules. Bidirectionality is explicit: the relationship between hypogonadism and metabolic disorders is bidirectional, and a vicious circle between the two components has been documented. PMC6669361

SHBG's highest-affinity ligand is DHT; higher SHBG lowers free androgen available to prostate tissue. doi 10.3390/ijms21218191
