Blocks the hormone that shrinks follicles
You want it low at the follicle: DHT is the primary hormonal driver of pattern hair loss, and both SHBG and 5α-reductase inhibition restrain it.
In depth
Dihydrotestosterone — the potent 5α-reductase product that binds the dermal-papilla androgen receptor and drives follicular miniaturization.
What drives it
Hormone-binding protein reduces it.
Free-hormone-hypothesis review 2022, Cell Mol Life Sci — SHBG binds circulating androgens (testosterone and, with even higher affinity, DHT), lowering the free bioactive fraction that reaches the follicle. doi 10.1007/s00018-022-04562-1
Testosterone increases it.
5α-reductase-inhibitor review 2023, Expert Opin Pharmacother — 5α-reductase converts testosterone to DHT, which binds the dermal-papilla androgen receptor ~5× more avidly and is the primary hormonal driver of androgenetic alopecia. doi:10.1080/14656566.2023.2280630. 5-alpha-reductase converts testosterone to DHT, which binds the androgen receptor with greater potency. Two isoforms with different territories: type 1 in liver and skin (systemic DHT, skin physiology); type 2 in prostate, seminal vesicles and hair follicles — which is why type 2 dominates benign prostatic hyperplasia and androgenic alopecia. Finasteride inhibits type 1; dutasteride inhibits both. conversion is tissue-dependent, not systemic — individual variation in 5-alpha-reductase and aromatase activity creates real heterogeneity in response to identical testosterone doses. doi 10.1080/14656566.2023.2280630.
5-alpha-reductase increases it.
5-alpha-reductase catalyzes the conversion of testosterone to dihydrotestosterone — the enzyme produces the androgen, so the edge is activating. This is the node the botanical evidence actually addresses: saw palmetto and pumpkin seed oil (via beta-sitosterol and plant sterol esters) inhibit the enzyme, and DHT falls as a consequence. An ingredient therefore works by inhibiting 5-alpha reductase, and the benefit to hair arrives through a double negative: inhibit the enzyme, lower DHT, release the follicle. saw palmetto is not only a 5AR inhibitor — a human hair-follicle organ-culture study reports it "promotes hair growth through mechanisms that extend beyond 5-alpha reductase inhibition", so this edge carries one route of several. Prager 2002, J Altern Complement Med 8:143-152 — randomized, double-blind, placebo-controlled trial of botanically derived inhibitors of 5-alpha-reductase in androgenetic alopecia ; Evron 2020, Skin Appendage Disorders (UC Irvine) — systematic review, 5 RCTs + 2 prospective cohorts, saw palmetto in alopecia.
What it drives
It reduces Hair.
Androgenetic-alopecia mechanism review 2025, Ann Dermatol — DHT binding the dermal-papilla androgen receptor drives progressive follicular miniaturization (suppressing Wnt/β-catenin and inducing TGF-β), shortening anagen. doi:10.5021/ad.25.042. The sensitivity is local, not systemic — which is why baldness is patterned. Elevated DHT and androgen receptor are named the causal factors for androgenetic alopecia, and three things are simultaneously higher in dermal papilla cells from balding scalp and beard than elsewhere: type II 5-alpha-reductase, androgen receptor, and the AR coactivator HIC-5/ARA55. The same circulating androgen therefore produces follicle loss in one scalp region and beard growth in another, because the receiving tissue differs, not the hormone. doi 10.5021/ad.25.042.
It reduces Growth factor.
DHT suppresses dermal-papilla IGF-1 production (Zhao 2011, Growth Horm IGF Res, PMID 21839661) by lowering cgrp / androgen-receptor signaling — an androgen route into follicle miniaturization. The intermediate is named and it is a microRNA: the androgen receptor directly promotes transcription of miR-221, which suppresses IGF-1 expression, inactivating mapk in dermal papilla cells and PI3K/akt in dermal sheath cells. In aga patients miR-221 correlated positively with AR and negatively with IGF-1, and miR-221 suppressed hair growth and dpc/dsc proliferation (PMC10367565). Therapeutic link: IGF-1 expression in follicular dermal papillae correlates with the therapeutic efficacy of finasteride — the marker tracks who responds to the drug. PMID 21839661 · PMC10367565
It reduces Tissue renewal signaling.
Leirós 2012 (Br J Dermatol, PMID 22283397) — in dermal papilla cells from androgenetic-alopecia patients, androgen treatment lowered the cytoplasmic/total β-catenin ratio and raised gsk-3β activity, i.e. Paired human biopsies, same 15 men: balding frontal follicles showed decreased Wnt10a and LEF1 mRNA versus their own non-balding occipital follicles, while tgf-beta signaling rose — the two move in opposite directions in the same scalp. A companion finding states hair follicle stem cell differentiation is inhibited through cross-talk between Wnt/beta-catenin and androgen signaling in dermal papilla cells from aga patients. PMID 22283397
It increases Reduces scarring.
Androgen-induced TGF-β1 from dermal papilla cells is an established route of follicular growth inhibition in AGA co-culture models (AGA transcriptome review, PMC8484716); the DHT–androgen-receptor complex up-regulates TGF-β1, IL-6 and DKK-1 while down-regulating Wnt agonists. Demonstrated in COCULTURE, not inferred: with AR-overexpressing human dermal papilla cells from AGA scalp cocultured with normal human keratinocytes, the androgen R1881 suppressed keratinocyte growth through androgen-inducible TGF-beta-1. The founding paper is titled 'androgen-inducible TGF-BETA1 from balding dermal papilla cells inhibits epithelial cell growth: A CLUE to understand paradoxical effects of androgen on human hair growth' (Inui 2002). Human tissue confirmation: TGF-beta-1 protein was elevated in balding follicles versus non-balding occipital follicles from the same 15 men (Lu 2016). PMC8484716
It increases PCOS.
Named across five reviews of the cardiovascular-renal-hepatic-metabolic syndrome as part of the shared mechanism set: ectopic fat accumulation triggered by metabolic reprogramming, oxidative stress and inflammation induced by energy overload, modified lipids, UREMIC toxins, and senescence, as well as insulin resistance pathways activated by pro-inflammatory factors and lipotoxic products, collectively exacerbate simultaneous hepatic and renal injury. doi 10.1038/s41581-026-01066-0 · PMC11725408
It reduces Women's hormone balance.
The genitalia and lower urinary tract share common estrogen receptor function. The receptor change is the estrogen receptors alpha and beta ARE present in the vagina, vestibule, urethra and bladder TRIGONE - while both receptors can be found in premenopausal women, only estrogen-beta receptors can be detected in postmenopausal women. The tissue does not merely see less hormone; it loses a receptor subtype. And a pH loop with a clinical endpoint: estrogen maintains vaginal glycogen, lactobacilli convert it to lactic acid holding pH at 3.5-5.0; loss of estrogen is responsible for the reduction of Lactobacillus, changing the vaginal fluid to an alkaline pH of 5.0 OR above, which promotes overgrowth of gram-negative rod fecal flora, inducing vaginal infection and UTI. Scale and underdiagnosis: GSM affects 40-54 percent of postmenopausal women and 15 percent of premenopausal women, and around 60 percent of these women have never had a diagnosis of GSM before. doi 10.3389/frph.2021.779398 · PMC10692865

It reduces Men's hormone balance.
The finding that unifies all three nodes is that the same androgen does opposite things in different tissues: testosterone acts directly, and indirectly through intracellular conversion to dihydrotestosterone by 5-alpha reductase, as in prostate cells, and separately through estrogen effect when converted to estradiol by aromatase. Raise testosterone and you also raise DHT and estradiol - which is why the same intervention supports male hormonal health and can enlarge the prostate. And the clinical data shows both arms at once: in 183 men over 50 with BPH, testosterone was significantly lower in patients with severe LUTS (285.94 ng/dL) than mild (544.09 ng/dL) - so low testosterone tracks with worse urinary symptoms, while DHT drives the gland. That is not a contradiction to resolve, it is two different molecules. Bidirectionality is explicit: the relationship between hypogonadism and metabolic disorders is bidirectional, and a vicious circle between the two components has been documented. PMC6669361

It increases Pattern hair loss.
Testosterone is necessary for the development of male pattern baldness. DHT binds the androgen receptor in dermal-papilla cells and drives the follicle miniaturization that is the central molecular cause of pattern hair loss. This edge names the condition rather than the outcome, so the trials that studied AGA specifically have a node that matches what they measured. Garza 2012, Sci Transl Med 4(126):126ra34.
