The Balance here is Between Goals — and between people. Raising it helps eight things: muscle mass and recovery, bone density, sexual function, urinary function, male fertility and hormonal health, and desire in women. It costs seven: DHT rises, and with it prostate load, acne and follicular oxidative stress; in women it pushes toward pcos and hormonal imbalance. This is the one node Where the Right Answer Genuinely Differs by sex. For a man past fifty the balance usually favors building it; for a woman the same move carries the androgen-excess costs. The map cannot pick for you, and it should not pretend to.

In depth

Serum testosterone — the anabolic signal behind muscle, drive and libido in men. The evidence shows a finding that overturns the common story: suppression under stress is centrally mediated — the brain down-regulating the axis — 'despite intact gonadal capacity'. The organ is rarely the problem.

What drives it

Reproductive axis increases it.

HPG-axis endocrinology review — pituitary LH stimulates testicular Leydig cells to synthesize testosterone. PMID 3109367. the axis is a closed feedback loop -- rising testosterone inhibits GnRH at the hypothalamus and LH at the pituitary, while inhibin selectively suppresses FSH only. Raising one arm is opposed by the loop. PMID 3109367

Sleep increases it.

For testosterone production, you need at least 3 hours of uninterrupted sleep, which is about the time of your first REM episode. Waking up throughout the night could affect hormone production. The three-hour figure is the least secure element here: the underlying primary work was not retrieved. Drawn because the sleep-testosterone link is consistently reported across the peer-reviewed set; the specific threshold is not carried by this edge. doi 10.1016/j.smrv.2011.05.001 · PMC3132857

Follicle-stimulating hormone increases it.

LH binds Leydig cell receptors, raising cAMP and driving StAR and CYP11A1 expression. PMC9113536

DHEA precursor pool increases it.

Upstream of both sex-hormone arms, which makes supplementation unpredictable - direction depends on tissue enzyme expression, age and sex. Carried by the intermediary-metabolism, micronutrient and clinical reviews. doi 10.3390/nu18132141 · PMC9967593

Zinc status increases it.

Weaker evidence - deficiency lowers testosterone; repletion in replete men does not raise it. Carried by the intermediary-metabolism, micronutrient and redox reviews. doi 10.3390/nu18132141 · PMC9967593

Obesogen exposure reduces it.

Obesogens act as anti-androgens and anti-estrogens and interfere with leptin, adiponectin and thyroid hormones. The flavor enhancer MSG may induce its adipogenic effects by impairing secretion of…. 10.1093/endo/bqaa024. doi 10.1093/endo/bqaa024

Prolactin reduces it.

Gonadotropin withdrawal in hyperprolactinemia produces hypogonadotropic hypogonadism. PMID 3917455 · PMC5686678

What it drives

It increases Nerve protection.

It increases Blocks the hormone that shrinks follicles.

5α-reductase-inhibitor review 2023, Expert Opin Pharmacother — 5α-reductase converts testosterone to DHT, which binds the dermal-papilla androgen receptor ~5× more avidly and is the primary hormonal driver of androgenetic alopecia. doi:10.1080/14656566.2023.2280630. 5-alpha-reductase converts testosterone to DHT, which binds the androgen receptor with greater potency. Two isoforms with different territories: type 1 in liver and skin (systemic DHT, skin physiology); type 2 in prostate, seminal vesicles and hair follicles — which is why type 2 dominates benign prostatic hyperplasia and androgenic alopecia. Finasteride inhibits type 1; dutasteride inhibits both. conversion is tissue-dependent, not systemic — individual variation in 5-alpha-reductase and aromatase activity creates real heterogeneity in response to identical testosterone doses. doi 10.1080/14656566.2023.2280630.

It increases Follicle oxidative stress.

Androgen/DPC-ROS study 2013, BMB Rep — dihydrotestosterone (androgen) increases reactive-oxygen-species generation in hair-follicle dermal papilla cells, mediating downstream TGF-β1 induction. PMC4133876

It modulates Urinary health.

Gormley 1992, NEJM — benign prostatic hyperplasia is androgen-dependent; testosterone converted to DHT via 5α-reductase drives prostate enlargement and urinary-obstruction symptoms, reversed by finasteride. doi:10.1056/NEJM199210223271701. The earlier note here rested on a patent claiming DHT tracks urinary symptoms while blood testosterone does not. A POST-HOC analysis of the randomized reduce trial — 3,009 asymptomatic and 2,145 symptomatic men — found NO evidence that serum testosterone OR serum dihydrotestosterone was related to LUTS incidence in asymptomatic men (P=0.9 and P=0.4) or to LUTS progression in symptomatic men (P=0.9 and P=0.7), with results alike in both placebo and dutasteride arms (all P-interaction >=0.3). Neither serum androgen predicts urinary symptoms (J Urol doi 10.1097/JU.0000000000003738; PMC10842939). Yet blocking DHT works, which is the whole point: 5-alpha-reductase inhibitors reduce serum and intraprostatic DHT — dutasteride by more than 90% — causing involution of prostate tissue, and randomized placebo-controlled trials show reduced LUTS, increased maximum urinary flow, and lower absolute risk of urinary retention and BPH surgery over 24 months (PMID 14627182; PMC5407420; finasteride: prostate volume -18% vs +14% on placebo over 4 years). The reconciliation is the circulating androgen level is not the variable — intraprostatic DHT and tissue volume ARE. A blood test does not predict who has symptoms; lowering DHT inside the gland still relieves them. Implication for this edge: it should be read as running through prostate tissue DHT, not through serum testosterone. doi 10.1056/NEJM199210223271701. · PMID 14627182 · PMC10842939

It increases Sexual function.

Corona 2014 meta-analysis, J Sex Med — testosterone supplementation improves erectile function and male sexual response in hypogonadal men. PMID 24697970. Testosterone regulates brain regions involved in sexual motivation including the hypothalamus; lower testosterone is associated with decreased libido, reduced sexual activity and satisfaction (Cureus systematic review). Again DHT-independent: maintenance of libido and sexual behavior does not require conversion of testosterone to DHT, and men with 5-alpha-reductase deficiency develop normal libido at puberty. only 1-5 percent of circulating testosterone is free and physiologically active — 95-98 percent is bound to SHBG and albumin, so total testosterone is the wrong measure. Normal range in healthy men is stated as 264-916 ng/dL, a nearly four-fold spread. PMID 24697970

It increases Muscle.

Bhasin 1996, nejm 335:1 — supraphysiologic testosterone raises fat-free mass, muscle size and strength (placebo-controlled RCT). DOI 10.1056/NEJM199607043350101. And the evidence settles a question the arrow leaves open — the muscle effect is direct, not via DHT. Significant 5-alpha-reductase expression occurs in prostate but not in muscle OR bone, and the source paper is titled 'anabolic effects of testosterone are preserved during inhibition of 5-alpha-reductase'. In tissues where 5-alpha-reductase activity is low, testosterone itself is the more active androgen. Human confirmation from nature's own experiment: males with 5-alpha-reductase deficiency have normal or slightly elevated testosterone and, at puberty, their muscle growth accelerates and the penis enlarges — while prostates remain non-palpable and they do not develop baldness. Implication: blocking DHT does not cost the muscle effect. testosterone also aromatises to estradiol, which independently affects bone. doi 10.1056/NEJM199607043350101.

It increases PCOS.

Hyperandrogenism is one of the defining features of the syndrome: PCOS is characterized by an overabundance of androgens alongside chronic ovulatory dysfunction, and hyperandrogenemia in turn aggravates insulin resistance — the two reinforce each other. PMC9832677

It increases Growth restraint.

Activation of Akt leads to phosphorylation and activation of downstream molecules including mTOR and p70S6K, resulting in an increase in protein synthesis. And the same kinase does the other half: furthermore, Akt activation leads to phosphorylation and inhibition of FoxO transcription factors, which ARE required for upregulation of the ubiquitin ligases MuRF-1 and MAFbx, resulting in a decrease in protein degradation. Muscle mass is a balance of two rates and testosterone moves both of them the same way through one node. That is why the effect on lean mass is reliable even where the effect on function is not — see the physical-performance findings, where testosterone moves body composition and fails on functional performance. Causal in vivo: wire-induced immobilization raised Atrogin-1, MuRF1, F4/80 and IL-6 (p < 0.001) while muscular androgen receptor expression significantly decreased and myostatin rose. Castration made the AR loss and the muscle damage worse, and both were completely ameliorated by testosterone supplementation and AR upregulation. Remove the hormone, worse; restore it, fixed. doi 10.1038/s41598-025-95115-6 · PMID 22101547 · PMC4987228

It reduces Reproductive axis.

NBK526128 — This is the return arm of the axis. Negative feedback is the defining property of the HPG axis: without it, the axis would produce a hormone and never switch itself off. The claim is plain: testosterone from the testes exerts negative feedback effects on FSH and LH production by negatively modulating production of GnRH in the hypothalamus, and testicular steroids have major feedback actions directly at the hypothalamus to inhibit the secretion of GnRH. Steroid hormones have a much greater inhibitory effect on LH production than on FSH.

It increases Bone health.

Named across six reviews of bone remodeling and osteoarthritis pathogenesis. Bone remodeling relies on the coordinated actions of bone-resorbing osteoclasts, bone-forming osteoblasts, and osteocytes serving as mechanosensors and local remodeling coordinators. PMC10648156

It increases Female libido.

Named across five reviews of erectile and female arousal physiology. Key contributors include reduced nitric oxide bioavailability due to endothelial oxidative stress and inflammation, diminished cavernosal smooth muscle content leading to impaired veno-occlusion, and increased arterial stiffness compounded by metabolic disorders. doi 10.21037/tau-2025-419 · PMID 19138376 · PMC11988752

It increases Recovery.

10.1186/s40798-024-00703-x — Recovery is assembled from the sarcopenia and VO2max literatures; none of the papers cited studied recovery itself. doi 10.1186/s40798-024-00703-x

It reduces Women's hormone balance.

The genitalia and lower urinary tract share common estrogen receptor function. The receptor change is the estrogen receptors alpha and beta ARE present in the vagina, vestibule, urethra and bladder TRIGONE - while both receptors can be found in premenopausal women, only estrogen-beta receptors can be detected in postmenopausal women. The tissue does not merely see less hormone; it loses a receptor subtype. And a pH loop with a clinical endpoint: estrogen maintains vaginal glycogen, lactobacilli convert it to lactic acid holding pH at 3.5-5.0; loss of estrogen is responsible for the reduction of Lactobacillus, changing the vaginal fluid to an alkaline pH of 5.0 OR above, which promotes overgrowth of gram-negative rod fecal flora, inducing vaginal infection and UTI. Scale and underdiagnosis: GSM affects 40-54 percent of postmenopausal women and 15 percent of premenopausal women, and around 60 percent of these women have never had a diagnosis of GSM before. doi 10.3389/frph.2021.779398 · PMC10692865
