Muscle
Muscle (myostatin↓/IGF-1)

You want it built and kept: muscle is metabolic currency and the armor against age-related decline.
In depth
The largest metabolic organ you have, and the one most under your control. Skeletal muscle is where most glucose is disposed of — so losing it is a metabolic event, not only a strength one.
What drives it
Calms inflammation reduces it.
NF-κB activation is sufficient to induce skeletal-muscle atrophy: cachectic cytokines such as TNF-α drive NF-κB, which up-regulates the E3 ubiquitin ligase MuRF1 and accelerates ubiquitin-proteasome protein degradation (PMC7564605). This is the inflammatory route to sarcopenia. Mouse atrophy models (denervation, dexamethasone, fasting, cancer cachexia, aging) plus C2C12 myotubes treated with dexamethasone, TNF-alpha and H2O2. Growth restraint increases it.
Bodine 2001, Nat Cell Biol 3:1014 — Akt/mTOR drives skeletal-muscle hypertrophy; rapamycin blocks it and genetic activation is sufficient. PMID 11715023. It was hypothesized that constitutively raising mTORC1 activity — by muscle-specific knockout of its inhibitor TSC1 — would increase protein synthesis and produce hypertrophy. A sarcopenic phenotype was observed instead: accelerated age-related loss of muscle mass and strength. Conversely, genetic suppression of mTORC1's downstream protein-synthesis effectors delays sarcopenia in naturally aging mice (PMC9891956). And synthesis is not growth: muscle-specific Raptor knockout had NO effect on load-induced increases in protein synthesis despite ablating the hypertrophic response — the authors state the protein-synthetic response was completely dissociated from the hypertrophic response, implying a concurrent rise in protein degradation. More mTOR is not more muscle. PMID 11715023 · PMC9891956
Testosterone increases it.
Bhasin 1996, nejm 335:1 — supraphysiologic testosterone raises fat-free mass, muscle size and strength (placebo-controlled RCT). DOI 10.1056/NEJM199607043350101. And the evidence settles a question the arrow leaves open — the muscle effect is direct, not via DHT. Significant 5-alpha-reductase expression occurs in prostate but not in muscle OR bone, and the source paper is titled 'anabolic effects of testosterone are preserved during inhibition of 5-alpha-reductase'. In tissues where 5-alpha-reductase activity is low, testosterone itself is the more active androgen. Human confirmation from nature's own experiment: males with 5-alpha-reductase deficiency have normal or slightly elevated testosterone and, at puberty, their muscle growth accelerates and the penis enlarges — while prostates remain non-palpable and they do not develop baldness. Implication: blocking DHT does not cost the muscle effect. testosterone also aromatises to estradiol, which independently affects bone. doi 10.1056/NEJM199607043350101.
Training load increases it.
Resistance training is the single best-established intervention for skeletal muscle mass in existence. Sedentary lifestyle and disuse appear in nearly all of six expert sarcopenia reviews. The evidence is deep and dose-specified: a Bayesian network meta-analysis of 13 randomized trials in 711 sarcopenic older adults, plus dose-response meta-analyses giving concrete parameters — 50-53 weeks, three sessions weekly, two to three sets, seven to nine repetitions at 51-69 percent of 1RM. Older adults do respond to resistance training at whole-muscle and fiber levels. And the conditional carries three things the headline hides. Strength moves more reliably than size: findings on morphological adaptations are mixed even where strength gains are consistent. Duration beats volume: training duration is more relevant than AGE OR training volume, especially at fiber level — how long you keep going matters more than how hard. And the instrument is part of the disagreement: leg lean mass changes may not be accurate at detecting quadriceps hypertrophy, and many trials infer muscle gain from whole-body lean mass rather than measuring the muscle. The authors of the dose-response work ARE blunt about their own evidence: results must be interpreted with caution because of the poor overall methodological study quality, mean PEDro score 4.6, and considerable heterogeneity, I-squared 80 percent. On morphology they found only nine studies and call the findings preliminary. The best-established intervention in the field rests on a thinner methodological base than its reputation suggests. And it connects to the anabolic-resistance finding recorded in this pathway’s own review: anabolic resistance is a hallmark of a sedentary lifestyle at any AGE. The exercise edge and the protein edge are the same story from two directions. doi 10.1007/s40279-015-0385-9 · PMC12590801
Growth factor increases it.
The expert sarcopenia reviews named IGF-1 decline as a mechanism; IGF-1/MGF is also named as a primary anabolic route. The protective arm is excellent and the title says it: muscle-specific expression of IGF-1 blocks angiotensin II-induced skeletal muscle wasting. IGF1 transgenic mice ARE resistant to atrophy from angiotensin OR cardiac cachexia, and local IGF1 injection is sufficient to block disuse atrophy. Mechanistically measured: marked increase in mTOR and p70S6K phosphorylation, and the angiotensin-induced fall in phospho-Akt completely blocked. And the conditional is the commercially important half: IGF-1 is not what makes muscle grow under load. Overload hypertrophy was unchanged in mice overexpressing a dominant-negative IGF1 receptor. Take the receptor out of action and loading still works. A second null, in the same disease the positive result claims: in hindlimb-unloaded MLC/mIgf-1 mice, surprisingly, IGF-1 overexpression was unable to counteract the loss of muscle weight and the decrease of fiber cross-sectional area. Local injection blocks disuse atrophy in one report and transgenic overexpression fails to in another. Both are recorded. And a baseline oddity: MLC/mIgf-1 mice have muscle hypertrophy not accompanied by increased Akt phosphorylation at rest — the textbook intermediate sits flat while the outcome moves (doi 10.1172/JCI22324; doi 10.1152/physiol.00041.2007; PMC3670938; PMC3143906). Lowers blood pressure reduces it.
Angiotensin II induces skeletal muscle wasting — the effect that muscle-specific IGF-1 was built to block. Angiotensin infusion reduces phospho-Akt in muscle, which disinhibits FoxO and drives the MuRF1/Atrogin-1 ubiquitin-proteasome program, and IGF1 transgenic mice ARE resistant to muscle atrophy induced by angiotensin treatment. What this adds: the renin-angiotensin system is usually read as purely cardiovascular. It is also a catabolic driver of muscle, which means a hormone system managed for blood pressure has a body-composition cost. The evidence is rodent infusion models and transgenic rescue; the human arm is inference from ACE-inhibitor observational data and is not claimed here. doi 10.1172/JCI22324 · PMC7564605
Cardiac output increases it.
Cardiac cachexia. It connects to the IGF-1 to muscle link, which rests partly on the finding that IGF1 transgenic mice ARE resistant to muscle atrophy induced either by angiotensin treatment OR in a mouse model of cardiac cachexia. The same cachexia model supports both links. doi 10.1016/j.jacc.2006.07.007 · PMC10932393
Mitochondrial energy increases it.
Mitochondrial dysfunction is particularly prominent in aging muscle - PGC-1-alpha expression decreases by 40-60 percent, leading to loss of oxidative phosphorylation capacity and accumulation of reactive oxygen species, triggering lipid peroxidation and mtDNA damage. A quantified magnitude, which is rare in this literature. doi 10.3390/ijms26146740 · PMC13074595
Oxidative stress reduces it.
Named among the core mechanisms across six sarcopenia reviews: protein metabolism, mitochondrial dysfunction, chronic inflammation, and neuromuscular junction degeneration, with regulation of inflammation primarily targeting inhibition of NF-kB signaling and elevated PGC-1-alpha OR PAX7 expression reversing mitochondrial OR satellite cell dysfunction. doi 10.3390/ijms26146740 · PMC13074595
Calms body-wide inflammation reduces it.
the core mechanisms of sarcopenia lie in protein homeostasis imbalance: the molecular scissors gap between anabolic decline (mTORC1 signal resistance) and catabolic hyperactivity (UPS/autophagy overactivation). doi 10.3390/ijms26146740 · PMC13074595
Clears worn-out cells reduces it.

Tissue renewal increases it.
Impaired satellite cell function is named across the reviews, and sarcopenic muscle shows loss of fibronectin-rich ECM, coupled with fibrotic changes and adipocyte infiltration, disrupting MuSC QUIESCENCE and self-renewal. doi 10.3390/ijms26146740 · PMC13074595
Insulin sensitivity increases it.
Insulin, another important anabolic hormone, is also involved through a similar pathway. However, myocyte sensitivity to insulin is significantly reduced during aging, which may be related to the deposition of ectopic lipids such as ceramides in muscle, impaired mitochondrial function, and decreased endothelial function. Three named causes of the resistance, and all three are nodes here. doi 10.3390/ijms26146740 · PMC13074595
Estrogen balance increases it.
Age-related hormonal changes, including reductions in estrogen and testosterone, may activate myostatin, exacerbating muscle loss. It is the negative regulator the whole anti-sarcopenia drug field targets, and the map cannot currently express it. doi 10.3390/ijms26146740 · PMC13074595
What it drives
It increases Insulin sensitivity.
The type 2 diabetes reviews state: decreased peripheral glucose uptake (mainly muscle) combined with augmented endogenous glucose production ARE characteristic features of insulin resistance. Muscle is the main disposal site, so muscle mass is a determinant of whole-body glucose handling - not just a consequence of it. This closes a loop: the Insulin Sensitivity → Skeletal Muscle Mass link is drawn (the anabolic arm, with myocyte sensitivity falling with age), and the Skeletal Muscle Mass → Insulin Sensitivity link is drawn here. Losing muscle worsens glucose handling, which worsens the anabolic signal, which loses more muscle. It is reinforcing - both arms positive - and is shown as a loop rather than as two arrows. PMC13074595
It increases Physical performance.
And the dissociation is the point, not a caveat: the sarcopenia reviews state that nutritional interventions preserve lean mass but fail to consistently restore function independently. So mass drives performance without determining it - which is why EWGSOP diagnostic criteria require low muscle strength as the primary criterion and treat low mass as confirmatory. The map already carried the other half of this: the Mitochondrial Function → Aerobic Capacity link distinguishes central from peripheral limits. This is the peripheral-mass arm. doi 10.3390/ijms26146740 · PMC13074595
It increases Recovery.
Satellite-cell capacity and ECM integrity determine repair after exertion or injury, and the sarcopenia set records loss of fibronectin-rich ECM disrupting MuSC QUIESCENCE and self-renewal. The weakest of the three: no retrieved source measures muscle mass predicting recovery rate as an endpoint. Drawn on the shared satellite-cell mechanism, which is an inference from a common substrate rather than a demonstrated effect. doi 10.3389/fcell.2025.1590524
It increases Urinary health.
The genitalia and lower urinary tract share common estrogen receptor function. The receptor change is the estrogen receptors alpha and beta ARE present in the vagina, vestibule, urethra and bladder TRIGONE - while both receptors can be found in premenopausal women, only estrogen-beta receptors can be detected in postmenopausal women. The tissue does not merely see less hormone; it loses a receptor subtype. And a pH loop with a clinical endpoint: estrogen maintains vaginal glycogen, lactobacilli convert it to lactic acid holding pH at 3.5-5.0; loss of estrogen is responsible for the reduction of Lactobacillus, changing the vaginal fluid to an alkaline pH of 5.0 OR above, which promotes overgrowth of gram-negative rod fecal flora, inducing vaginal infection and UTI. Scale and underdiagnosis: GSM affects 40-54 percent of postmenopausal women and 15 percent of premenopausal women, and around 60 percent of these women have never had a diagnosis of GSM before. doi 10.3389/frph.2021.779398 · PMC10692865
It increases Wound healing.
The one measured finding among them is the periodontal-anemia link: a meta-analysis of sixteen studies on hemoglobin and iron metabolism in periodontitis, whose authors call the association still controversial and ask for larger cohorts.
