Cardiac output
You want it healthy: efficient contractility sustains circulation and exertional capacity.
In depth
The heart as a pump and as a tissue that remodels. Chamber wall changes precede functional decline by years, which is why cardiac remodeling is separated from cardiac output.
What drives it
Sympathetic drive reduces it.
Cardiac autonomic review — sympathetic norepinephrine acting on myocardial β-adrenergic receptors increases heart rate (chronotropy) and contractility (inotropy). PMC2752648. SNS hyperactivity is initially a compensatory mechanism that stimulates contractility and maintains cardiac output. But chronic stimulation becomes detrimental, causing decreased cardiac function and reduced inotropic reserve through loss of cardiac beta-adrenergic receptor responsiveness. The therapy is the proof: beta-blockers and physical activity, which restore beta-AR responsiveness, improve cardiac performance and outcomes in heart failure — blocking the 'positive' input is what helps. Acute sympathetic drive supports the heart; sustained sympathetic drive damages it. PMC2752648
Heart calcium handling increases it.
Contractility is not set by the sympathetic system alone: the literature names intracellular calcium as the final common pathway for the activity of most inotrope drugs and physiological factors which influence contractility. The structural unit is the DYAD — L-type calcium channels on T-tubules facing RyR2 on the SR. SERCA2a, which pumps calcium back in, is a primary regulator of calcium homeostasis and its remodeling is proposed as a causal factor in disease. doi 10.1098/rstb.2021.0317 · PMID 15176427 · PMC5497788
Thyroid increases it.
NBK500006 — what the evidence here shows: the sources read here establish calcium handling and the beta-adrenergic pathway in detail but do not themselves demonstrate the thyroid arm. This link is drawn because the structure requires it; it is not yet evidenced to the standard of the others here, and no claim rests on it.
Calms body-wide inflammation reduces it.
the immediate phase occurs minutes after TNF exposure, is NO independent, and is mediated by sphingosine disruption of calcium transients; (doi 10.1152/ajpregu.1998.274.3.R577; doi 10.1172/JCI116834; PMC7714153; PMC6311126. doi 10.1152/ajpregu.1998.274.3.R577 · PMC7714153
Gut-brain signal increases it.
The right vagus predominantly innervates the sinoatrial node, modulating heart rate, while the left vagus more strongly influences atrioventricular conduction. Anatomically specified and lateralised. And it is directly demonstrable in a healthy person: the mammalian diving reflex occurs when sensory fibers of the vagus and trigeminal nerve ARE stimulated by facial SUBMERSION to trigger bradycardia. Respiratory sinus arrhythmia — heart rate accelerating on inhalation and decelerating on exhalation — is driven by vagal modulation, and high resting vagal tone, indexed by high-frequency HRV, is associated with lower resting heart rate and more efficient baroreflexes. This synchronisation is a critical predictor of survival in coronary heart disease — which is why HRV is a prognostic measure and not merely a wellness metric. PMC12082064
Lowers blood pressure reduces it.
Neurohormonal dysregulation with angiotensin II, endothelin-1, and sympatho-adrenergic overactivation, which occurs in heart failure, stimulates ventricular cardiomyocyte hypertrophy and aggravates the cellular damage. And this is the rare case where the intervention arm is the standard of care: the understanding of the important role of neurohormonal activation in the pathophysiology of heart failure has paved the way for the current medical treatment to improve morbidity and mortality. Inappropriate activation of the renin-angiotensin-aldosterone system is also named among the routes by which metabolic disease produces cardiac stiffening and diastolic dysfunction. Note: the same hormone system is catabolic for skeletal muscle and hypertrophic for cardiac muscle — opposite structural effects in two striated tissues, both shown on the map. doi 10.1016/j.jacc.2006.07.007 · PMC10932393
Oxidative stress reduces it.
10.3389/fphys.2016.00313. doi 10.3389/fphys.2016.00313
Cell turnover reduces it.
Cardiomyocyte apoptosis is named among the primary mechanisms, and programmed cell death - including apoptosis, pyroptosis, and ferroptosis - is one of the eight categories a 2025 review divides heart-failure mechanisms into. Why it matters more here than in most tissues: oxidative stress and chronic, low-grade inflammation ARE important contributors to the poor regenerative properties of cardiomyocytes. Heart muscle does not replace what it loses, so apoptosis here is subtractive in a way it is not in liver or gut. The atlas has one apoptosis node and the reviews distinguish apoptosis, pyroptosis and ferroptosis as separate programs with different triggers. That compression is stated. doi 10.1016/j.jacc.2006.07.007 · PMC10932393
Blood flow (nitric oxide) increases it.
An uncoupled NOS produces superoxide instead of nitric oxide — the same enzyme switching from the protective product to the damaging one, which is why this is not simply less NO. Endothelial dysfunction is listed among the eight core heart-failure mechanism categories, and in HFpEF specifically cardiovascular risk factors create an environment of oxidative stress, inflammation, and microvascular endothelial dysfunction (PMC10932393; PMC10001590; PMC11899429; PMC9894375; doi 10.1016/j.jacc.2006.07.007). REFUTED ON A SECOND READING. This link was shown by linked steps until a source was opened that does not support it: “These results demonstrate that beta3-adrenoceptor stimulation decreases cardiac contractility through activati” — The negative inotropic effect of beta3-adrenoceptor stimulation is mediated by activation of a nitric oxide synthase pathway in human ventricle. Journal of Clinical Investigation, 1998, 102(7), 1377-1384. doi 10.1016/j.jacc.2006.07.007 · PMC10932393
Tissue remodeling reduces it.
Hypertrophy and fibrosis ARE significant characteristics of pathological cardiac remodeling, contributing to heart failure, and metabolic abnormalities activate pro-fibrotic pathways resulting in cardiac stiffening and diastolic dysfunction. A causal caveat the sources make themselves: these studies associate cardiac lipid accumulation with cardiac fibrosis, but a causal relationship has not been demonstrated. The fibrosis-to-dysfunction link is solid; some of the upstream routes into fibrosis are not. doi 10.1016/j.jacc.2006.07.007 · PMC10932393
Insulin sensitivity increases it.
In the early stages of diabetic cardiomyopathy, metabolic disturbances such as defective insulin signaling, excessive circulating insulin, impaired glucose uptake, elevations in myocardial fatty acid uptake, and mitochondrial dysfunction promote cardiac morphological and functional changes. The route is specified three ways: hyperglycemia, insulin resistance, and hyperinsulinemia have all been associated with calcium overload; insulin resistance is detrimental to mitochondria via excessive ROS production and altered cardiomyocyte calcium handling; and it can increase FFA flux through CD36. This is heart disease that begins before any coronary narrowing, which is precisely what a map built around atherosclerosis alone would miss. doi 10.1016/j.jacc.2006.07.007 · PMC10932393
Blood pressure reduces it.
Prolonged stress induced by risk factors, including aging, diabetes, and hypertension, has deleterious effects, triggering alterations in key signal transduction and metabolic pathways and causing pathological hypertrophy, fibrosis, and apoptosis. The initial response induces cardiomyocyte growth and survival, angiogenesis, antioxidant generation, and mitochondrial biogenesis — all good. It is sustained stimulation that turns it. Same pathway, opposite outcome, and duration is the variable. doi 10.1016/j.jacc.2006.07.007 · PMC10932393
Mood increases it.
Depression increases the risk of heart disease at RR 1.81, the same magnitude as its effect on overall mortality (RR 1.81). Most meta-analyses have been based on longitudinal studies using depressive symptom checklists which pick up many subthreshold depression cases - the exposure is broader than clinical diagnosis. PMC3661358
Arterial plaque reduces it.
Hypertension and atherosclerosis reviews name cardiac complications including coronary artery atherosclerosis, myocardial ischemia and myocardial hypertrophy and its complications, and the end stage of most heart disease is heart failure. Structurally this completes a loop already half-drawn: the Blood Pressure → Atherosclerosis link and the Blood Pressure → Contractility link both exist, and this edge says the two targets are not independent - hypertension reaches the myocardium both directly through pressure load and indirectly by narrowing its supply. The atlas draws one cardiac node covering both ischemic and hypertensive heart disease; that compression is stated. PMC12840960
What it drives
It increases Physical performance.
Mitochondria and PGC-1-alpha are both peripheral factors, and the literature is explicit that they are not the ceiling for whole-body exercise: central circulatory factors mainly restrict VO2MAX when performing an all-round exercise, while peripheral factors might be dominant in regional exercise. A review is titled cardiac output limits maximal oxygen consumption. The practical reading: mitochondrial support addresses the factor that is not limiting for whole-body aerobic capacity in a trained person — it becomes limiting only for small-muscle-mass or regional work, where peak oxygen uptake is thought to be limited by oxygen extraction and mitochondrial oxidative capacity. Quantified in humans: in 26 women spanning VO2max 32-62 mL/kg/min, a 1 liter higher blood volume at VO2max was associated with a 16.2 mL higher maximal stroke volume (r=0.63, P<0.001) and 2.5 L/min higher maximal cardiac output (r=0.56, P<0.01); stroke volume correlated with left ventricular muscle mass and end-diastolic diameter at every intensity. Why heart rate cannot simply rise to compensate: heart rate is the main determinant of myocardial oxygen consumption, and ventricular filling and myocardial blood flow would be compromised by further increases, since myocardial flow is blunted by contraction and occurs principally during DIASTOLE. Ceiling: VO2max is approximately 50 percent genetically determined and 50 percent trainable. doi 10.1016/j.scispo.2021.02.001 · PMC12053887
It increases Heart & blood pressure.
The end stage of most heart disease is heart failure, for which hypertension is both a cause and a risk factor, and heart failure remains a debilitating, steady and efficient killer whose molecular mechanisms remain to be fully understood. The atlas now carries the full arc on this pathway: risk factors drive remodeling, remodeling drives failure, failure drives death. doi 10.1016/j.jacc.2006.07.007 · PMC10932393
It increases Kidney.
Cardiorenal syndrome - the two organs fail together and each accelerates the other. The reverse arm is at least as well documented and is not drawn: hypertension is the second most frequent cause of chronic kidney disease. Drawing only heart-to-kidney shows half of a two-way relationship. doi 10.1016/j.jacc.2006.07.007 · PMC10932393
It increases Muscle.
Cardiac cachexia. It connects to the IGF-1 to muscle link, which rests partly on the finding that IGF1 transgenic mice ARE resistant to muscle atrophy induced either by angiotensin treatment OR in a mouse model of cardiac cachexia. The same cachexia model supports both links. doi 10.1016/j.jacc.2006.07.007 · PMC10932393
It increases Gut-brain signal.
Every link on this pathway inherits one taVNS produced opposite effects split by baseline RMSSD. High baseline tone - heart rate and TNF-alpha significantly increased and vagally mediated HRV significantly decreased versus sham. The same intervention helped one group and harmed the other. Diagnostic stratification failed where biological stratification worked: NO difference in any outcome between MDD participants and controls. doi 10.3390/biomedicines13030700 · PMC12689627
It increases Hydration & electrolytes.
The one measured finding among them is the periodontal-anemia link: a meta-analysis of sixteen studies on hemoglobin and iron metabolism in periodontitis, whose authors call the association still controversial and ask for larger cohorts. doi 10.1152/japplphysiol.00971.2016 · PMC7005133
