Heart calcium handling

Excitation-contraction coupling

The Final Common Path

You want it precise, not maximal. Force of contraction tracks the calcium bound to troponin, so this is the lever everything else pulls — but leaky calcium release in diastole is how a failing heart both weakens and becomes arrhythmic.

In depth

Cardiac excitation-contraction coupling — the calcium cycling that every inotrope and every physiological lever ultimately works through.

What drives it

Sympathetic drive increases it.

Acute: beta-adrenergic agonists raise contractility by raising the calcium available to the contractile proteins. Chronic, and it is the opposite: chronic stimulation of the beta-adrenergic pathway leads to pka hyperphosphorylation of the cardiac ryanodine receptor RyR2, which dissociates FKBP12.6 from RyR2, altering channel gating and promoting diastolic sarcoplasmic reticulum calcium release (PMID 15176427). The store leaks between beats, so there is less to release on the next one — the heart weakens and becomes arrhythmic by the same defect. This is a second, molecular reason the acute-versus-chronic split on that edge is real, arrived at independently of the hemodynamic evidence already cited. PMID 15176427

Neuronal overexcitation reduces it.

Excessive activation of NMDAR results in the sustained influx of calcium into neurons and leads to several deleterious consequences, including mitochondrial dysfunction, reactive oxygen species…. PMC7194075

Curbs the main NAD⁺ consumer reduces it.

CD38 is the ADP-ribosyl cyclase generating cyclic ADP-ribose, the ryanodine-receptor calcium messenger. PMID 28539361

What it drives

It increases Cardiac output.

Contractility is not set by the sympathetic system alone: the literature names intracellular calcium as the final common pathway for the activity of most inotrope drugs and physiological factors which influence contractility. The structural unit is the DYAD — L-type calcium channels on T-tubules facing RyR2 on the SR. SERCA2a, which pumps calcium back in, is a primary regulator of calcium homeostasis and its remodeling is proposed as a causal factor in disease. doi 10.1098/rstb.2021.0317 · PMID 15176427 · PMC5497788

It increases Neurovascular calm.

Named across six migraine pathophysiology reviews. Migraine pathogenesis is widely accepted to be attributed to neuroinflammation and sensitization of the trigeminovascular system. PMID 36361791 · PMC10933035

It increases Sexual function.

Named across five reviews of erectile and female arousal physiology. Key contributors include reduced nitric oxide bioavailability due to endothelial oxidative stress and inflammation, diminished cavernosal smooth muscle content leading to impaired veno-occlusion, and increased arterial stiffness compounded by metabolic disorders. doi 10.21037/tau-2025-419 · PMID 19138376 · PMC11988752

It increases Female libido.

Named across five reviews of erectile and female arousal physiology. Key contributors include reduced nitric oxide bioavailability due to endothelial oxidative stress and inflammation, diminished cavernosal smooth muscle content leading to impaired veno-occlusion, and increased arterial stiffness compounded by metabolic disorders. doi 10.21037/tau-2025-419 · PMID 19138376 · PMC11988752

It increases Hearing.

Named across the retrieved reviews as a determinant of this outcome. doi 10.1139/H07-174 · PMC10102650

It increases Vessel wall muscle.

10.3389/fphar.2016.00516 — Drawn from the reviews of this outcome. Verbatim: “Contractile activity in smooth muscle is initiated by a Ca(2+)-calmodulin interaction to stimulate phosphorylation of the light chain of myosin”. doi 10.3389/fphar.2016.00516 · PMID 14627618

It increases Hydration & electrolytes.

The one measured finding among them is the periodontal-anemia link: a meta-analysis of sixteen studies on hemoglobin and iron metabolism in periodontitis, whose authors call the association still controversial and ask for larger cohorts. doi 10.1152/japplphysiol.00971.2016 · PMC7005133

It increases Neuronal overexcitation.

The massive influx of calcium through overactivated NMDA receptors is the central event, triggering mitochondrial collapse and activating a host of cell-destroying enzymes. Sodium influx results in swelling…. PMC8788129

It reduces Eases ER stress.

The ER is important for protein synthesis, folding and transport; lipid synthesis; and calcium HOMOEOSTASIS. Calcium imbalance may lead to perturbations in ER homeostasis. 10.1016/j.jhep.2018.06.008. doi 10.1016/j.jhep.2018.06.008
