Inner-ear hair-cell function
Also searched as Inner-ear hair-cell function.
Outer hair cells do not regenerate. Every exposure that kills them is permanent, which makes this one of the few places on the map where prevention is the only lever that exists.
In depth
Auditory function — the health of the cochlear hair cells that convert sound into signal.
What drives it
Oxidative stress reduces it.
Loud noise damages hearing by oxidising it: a transient, intense burst of reactive oxygen species is detectable in the cochlea immediately after noise exposure, and the resulting lipid peroxidation destroys the fragile outer hair cells — which do not regenerate (Henderson et al., Ear Hear 2006;27:1-19, PMID 16446561; PMC8066810). Shown three ways in one experiment: inhibiting SIRT3 aggravated oxidative hair-cell damage in noise-exposed mice, SIRT3 overexpression significantly attenuated that injury, and the protection was abolished by blocking SOD2 — placing the damage squarely on mitochondrial redox balance (PMC8637754). Note: antioxidants protect in animal models and some clinical settings, but there is still no established drug treatment for noise-induced hearing loss (PMC9750774). Loss-of-function, twice and independently: in-vivo N-acetylcysteine reduced noise-induced ROS formation (measured as 4-hydroxynonenal and 3-nitrotyrosine) and reduced AMPK-alpha activation in outer hair cells; separately, NOX3 knockout reduced superoxide production, inhibited outer hair cell apoptosis and improved hearing, and p22phox-deficient mice showed diminished excitotoxic cochlear damage. Note: outer hair cells do not regenerate in mammals, so this damage is cumulative and not recoverable. PMID 16446561 · PMC8066810
Mitochondrial energy increases it.
Inherited mutations in mtDNA, acquired alterations to mtDNA, mitochondrial overactivation, calcium imbalance, and ototoxicity all contribute to mitochondrial dysfunction in cochlear cells. This dysfunction, in turn, leads to oxidative stress and subsequent hearing impairment. doi 10.1139/H07-174 · PMC10102650

Cellular aging reduces it.
The prevalence gradient is the argument: acquired sensorineural hearing loss is found in 5-8 per 1,000 children, 33 percent of adults 65-74, and over 80 percent of adults 85 and over. And a limit the reviewers state: there is still a lack of information on the underlying mechanisms of redox homeostasis and molecular redox networks in hair cells. doi 10.1139/H07-174 · PMC10102650

Cellular fuel increases it.
Mitochondrial antioxidants, modulators of mitochondrial function, calcium channel blockers, and mtDNA mutations that affect mitochondrial activity in the auditory system have emerged as critical factors in hearing protection. The reviewers state their own gap: there is still a lack of information on the underlying mechanisms of redox homeostasis and molecular redox networks in hair cells - so these are protective-target candidates, not demonstrated interventions. The clinical fact that makes protection the only strategy: mammalian cochlear hair cells do not regenerate, which is why SNHL is described as the most common type of permanent hearing impairment. doi 10.1016/j.mito.2025.102063 · PMC10102650
