In depth
Endothelial nitric-oxide synthase — the vessel-lining enzyme that makes NO to relax and open blood vessels.
What drives it
Estrogen balance increases it.
Musicki 2009, J Sex Med (PMID 19138376) — estrogen is critical for maintaining vaginal and clitoral blood flow, and its vascular effects are attributed mostly to its regulation of endothelial nitric oxide production; estrogen regulates eNOS by genomic mechanisms (increased mRNA transcription and protein synthesis) and by non-genomic mechanisms. PMID 19138376
Growth & survival signaling increases it.
Fulton 1999, Nature — Akt (downstream of PI3K) directly phosphorylates eNOS at Ser1177/1179, activating the enzyme and NO production. PMID 10376602. Fulton 1999 showed AKT directly phosphorylates eNOS and activates the enzyme, producing nitric oxide; eNOS mutated at the AKT site was resistant to phosphorylation and activation; activation-deficient AKT attenuated VEGF-stimulated NO production. Dimmeler 1999 independently showed that inhibiting PI3K/AKT, OR MUTATING serine-1177 on eNOS, prevented eNOS activation — while mimicking Ser1177 phosphorylation directly enhanced enzyme activity. Gain-of-function, loss-of-function, and site-directed mutagenesis, replicated across two labs (OMIM 164730). AKT is not one switch — its two activating phosphosites govern different substrates: selectively inhibiting Ser473 weakened FOXO1/2a and FOXO3 but did not affect GSK3-beta or TSC2, while it is THR308 phosphorylation that tracks p-eNOS Ser1177 — Ser473 exerts rare effects on eNOS (PMC8255840). Which Akt site is engaged decides which downstream arm moves. PMID 10376602 · PMC8255840
Oxidative stress reduces it.
Oxidative stress does not merely oppose nitric oxide — it turns the enzyme against itself. When eNOS uncouples it stops producing NO and starts producing superoxide: senescent endothelial cells show a fallen eNOS dimer/monomer ratio with raised superoxide that is blocked by the eNOS inhibitor L-NAME, proving the enzyme itself is the source; aged vessels show the same uncoupling with reduced bioactive NO, and it is reversed by treatment (PMC3081344). This is why an antioxidant lane and a blood-flow lane are the same lane. The mechanism, precisely: eNOS normally transfers electrons from TETRAHYDROBIOPTERIN (BH4) to L-arginine to make NO. When BH4 is limiting, eNOS uncouples and passes those electrons to oxygen instead — the enzyme then produces superoxide in place of nitric oxide. Superoxide plus NO yields peroxynitrite, which further degrades BH4: a self-amplifying vicious cycle. A second, independent route is S-GLUTATHIONYLATION of two conserved cysteines in the eNOS reductase domain. eNOS is best understood as an integrated redox hub, not a simple NO source. The finding that matters for any supplement aimed here: enhancing BH4 availability has reported mixed results clinically, and in a quantitative model a ten-fold increase in BH4 synthesis at high oxidative stress did not restore NO production or the biopterin ratio. The authors explicitly propose combination therapy — BH4 together with a reduction in oxidative stress. Feeding the substrate alone fails while ROS is high. Above a threshold the system also enters an OSCILLATORY state rather than settling. Verbatim: “Likewise, it has been postulated that NOS can also contribute to oxidative stress, because ROS can cause their uncoupling and generating O2·− instead of NO”. doi 10.1080/13510002.2021.1891808 · PMC3081344
Gut barrier increases it.
The protective arm: SCFAs ACT on endothelial cells to enhance nitric oxide generation and thus improve vasodilation, and GPR41 — the receptor carrying that effect — has expression sites mainly located in the vascular endothelium. Exogenous propionate reduced blood pressure in angiotensin-II hypertensive rats through GPR41-mediated intracellular calcium release in the endothelium. The damaging arm is the same organ producing a different molecule: TMAO can directly inhibit eNOS phosphorylation, OR indirectly modulate its activity through increased ROS, activation of NLRP3, OR decreased intracellular calcium. ROS and the NLRP3 inflammasome are both on the map, so that route can be traced. Why this link matters: part of what the gut does to blood pressure it does by acting on the endothelial enzyme, so two arrows into one outcome are partly one sequence. doi 10.3389/fcvm.2026.1884283 · PMID 36982880

Dietary nitrate increases it.
A route that does not need the enzyme - the nitrate-nitrite-NO pathway bypasses nitric oxide synthase, and works best exactly where the enzymatic route fails, in hypoxia and acidosis. Carried by the intermediary-metabolism, micronutrient and clinical reviews (PMC9967593; doi 10.3390/nu18132141; PMC12673670; doi 10.1016/j.molmet.2023.101755). Verbatim: “This appears to be mediated in part through a XOR-dependent reduction of nitrate to NO, as well as enhanced eNOS activation via increased Akt and eNOS phosphorylation”. doi 10.3390/nu18132141 · PMC9967593
Nitric oxide precursor increases it.
The two enzymes compete for it - arginase activity, which rises with age and inflammation, diverts arginine away from nitric oxide. Citrulline raises plasma arginine more effectively than arginine does, by bypassing first-pass arginase. Carried by the intermediary-metabolism, micronutrient and redox reviews. doi 10.3390/nu18132141 · PMC9967593

What it drives
It increases Female libido.
Musicki 2009, J Sex Med (PMID 19138376) — review of endothelial nitric oxide synthase regulation in female genital tract structures: the NO/cyclic-GMP pathway has a primary role in regulating clitoral and vaginal blood flow and smooth-muscle relaxation during sexual arousal, and estrogen's effect on genital blood flow is attributed largely to its regulation of eNOS. PMID 19138376
It reduces Blood pressure.
Endothelial NO tonically lowers blood pressure in humans (Hypertension, DOI 10.1161/01.hyp.0000252425.06216.26) — NO-synthase inhibition raised mean BP by 6 mmHg in intact subjects and by 21 mmHg under autonomic blockade, isolating the endothelial contribution; eNOS-knockout mice run ~20 mmHg higher. Convergent eNOS-derived NO governs vascular tone, and eNOS is protective against hypertension; NO stimulates vasodilation via cGMP in smooth muscle cells. Two distinct output mechanisms, not one: NO acts both through soluble guanylate cyclase / cGMP and through S-NITROSYLATION of reactive protein thiols — an arrow labeled 'NO' collapses two different signaling systems. Localisation changes the output: plasma-membrane-targeted eNOS drives sGC/cGMP signaling more than Golgi-tethered eNOS, so where the enzyme sits alters what it does. The enzyme has preconditions: functional eNOS requires dimerisation, L-arginine substrate and the BH4 cofactor; without BH4 it uncouples and makes superoxide instead of NO (see the Oxidative Stress (ROS) → Endothelial (eNOS) link). And a null result worth carrying: selective endothelial CaMKII inhibition in two transgenic models left systolic and diastolic blood pressure, heart rate, plasma NO and NO-dependent vasodilation unchanged (PMC5653296) — not every proposed upstream regulator of eNOS matters in vivo. doi 10.1161/01.hyp.0000252425.06216.26 · PMC5653296

It reduces Reduces clotting.
The endothelial brake. Verbatim: 'Platelet adhesion to collagen fibrils and endothelial cell matrix was inhibited completely by NO but only partially by prostacyclin' (PMID 2825688). In vitro on isolated platelets; the NO-deficiency link to arterial thrombosis is association, not a trial. PMID 2825688
It increases Heart & blood pressure.
Palmer, Ferrige & Moncada 1987, Nature 327:524 — endothelial NO is EDRF, driving vasodilation and lowering vascular tone. PMID 3495737. eNOS is compartmentalised, not free-floating — in cardiac myocytes essentially all eNOS co-immunoprecipitates with caveolin-3, so its activity is set by where it is tethered, and disrupting the caveolin-3/eNOS association is itself a signaling event. PMID 3495737
It reduces Arterial plaque.
A healthy endothelium resists plaque; an activated one initiates it. Oxidized LDL triggers endothelial cells to up-regulate adhesion molecules and chemokines, recruiting the monocytes that become foam cells (EAS Consensus Panel, Eur Heart J 2020;41:2313; PMC11989677). Endothelial dysfunction is therefore the first step, and endothelial NO tone is what opposes it. Convergent finding, and the mechanism is specific: NO inhibits leukocyte adhesion to the vessel wall by interfering with CD11/CD18 binding and by suppressing its expression — and leukocyte adherence is an early event in atherogenesis, which is why NO protects against onset. NO also inhibits DNA synthesis, mitogenesis and proliferation of vascular smooth muscle cells (antiproliferative effects mediated mostly by cGMP), and is a potent inhibitor of platelet aggregation and adhesion. Three separate anti-atherogenic actions, not one. PMC11989677
It increases Sexual function.
Erection is a nitric-oxide event, and the proof is a drug class built on it: NO released in the corpus cavernosum activates guanylate cyclase, raising cGMP and relaxing the smooth muscle so blood can enter. PDE5 inhibitors do nothing but prevent the breakdown of that cGMP — sildenafil has no direct relaxant effect of its own — and they have been first-line therapy for erectile dysfunction since their introduction (PMC6003652, PMC4563010). Testosterone matters, but it acts largely by stimulating NO synthase; the endothelium is where the mechanism lands. The NO-cGMP axis characterization earned the 1998 NOBEL PRIZE in physiology OR medicine and is the mechanistic basis of every PDE5 inhibitor. Pharmacology confirms the direction: PDE5 inactivates cGMP and terminates the relaxation; inhibiting PDE5 preserves cGMP and enhances erection. Human trial, and it runs both ways: 21 men with ED given sildenafil on demand for 12 weeks improved IIEF-EF from 17.9 to 26 (P<0.05) and showed higher eNOS protein afterwards, alongside falls in hs-CRP and sICAM-1 (Int J Impot Res doi 10.1038/ijir.2009.51). endothelial NO is prerequisite but not alone — neuronal NOS contributes in parallel, and age-related ED also involves lost cavernosal smooth-muscle content and impaired veno-occlusion, which more NO cannot fix. doi 10.1038/ijir.2009.51 · PMC6003652
It increases Urinary health.
The same NO/cGMP pathway relieves the urinary symptoms, which is why one drug class treats both: PDE5 inhibition and the consequent change in NO/cGMP signaling relaxes prostatic smooth muscle and reduces urethral pressure, and lower urinary tract symptoms secondary to benign prostatic hyperplasia is now an approved indication for PDE5 inhibitors (PMC4563010, PMC6003652). The overlap is not coincidence — in men scheduled for surgical management of LUTS/BPH, 36% with moderate and 94% with severe symptoms also had erectile dysfunction. PMC4563010
It reduces Sympathetic drive.
the confound, in the authors’ own words: systemic infusion of nitric oxide synthase inhibitors increases blood pressure due to inhibition of endothelial NOS, resulting in activation of the arterial baroreflex and subsequent inhibition of central sympathetic outflow. Block NO, pressure rises, the baroreflex fires, and sympathetic activity falls — the opposite of the direct effect, and large enough to hide it. A Circulation study confirms the trap: L-NMMA raised mean arterial pressure by about 10 percent and had NO detectable effect on muscle sympathetic nerve activity. The solution is to pick a nerve bed the baroreflex does not control: to overcome this confounding inhibitory influence, the investigators directly measured skin sympathetic nerve activity, which is not under BARORECEPTOR control. Same drug, same subjects, different nerve, and the masking disappears. The result: pharmacological NOS inhibition causes a robust and sustained increase in central sympathetic outflow in humans. Nitric oxide is described as a key signaling molecule involved in the tonic restraint of central sympathetic outflow from the brainstem. And the anatomy is localized with a disease model: in fructose-fed insulin-resistant rats, the paraventricular nucleus showed reduced NO content and reduced nNOS and eNOS protein alongside raised plasma norepinephrine, and the authors conclude the reduction of NO generation in the PVN caused weakened sympathoinhibition — proposing this as a mechanism for the sympathetic overdrive of metabolic syndrome, diabetes and obesity. What this changes on the MAP: the Endothelial (eNOS) → Blood Pressure link and the Sympathetic → Blood Pressure link were drawn as independent parallel drivers. They are not independent — eNOS restrains the sympathetic arm as well as dilating the vessel, so part of what the Endothelial (eNOS) → Blood Pressure link carries runs through the Sympathetic → Blood Pressure link. the human work is acute pharmacological blockade in healthy normotensive volunteers; the PVN localisation is rodent. doi 10.1161/01.CIR.96.11.3897 · PMC2770160
It increases Hair.
Androgen-receptor-mediated paracrine signaling, mainly TGF-beta, from dermal papilla cells induces apoptosis of microvascular endothelial cells in the dermal papilla of balding scalp. The androgen does not merely shrink the follicle — it kills the follicle’s blood vessels, causing vascular calcification, degeneration of the microvascular network, and an ischemic and hypoxic pilosebaceous unit that suppresses stem cell activity. Why the conditional matters commercially: the vascular loss is downstream of the androgen, not a parallel cause. Raising scalp perfusion in a follicle whose papilla vasculature has already been apoptotically pruned is treating the effect. The authors put it as a clinical rule: early anti-AR treatment is better than late. And it reinforces itself — microcirculatory dysfunction can induce perifollicular fibrosis, and persistent low perfusion drives follicles into catagen. doi 10.1016/j.jid.2022.01.005 · PMC12380480
It increases Cardiac output.
An uncoupled NOS produces superoxide instead of nitric oxide — the same enzyme switching from the protective product to the damaging one, which is why this is not simply less NO. Endothelial dysfunction is listed among the eight core heart-failure mechanism categories, and in HFpEF specifically cardiovascular risk factors create an environment of oxidative stress, inflammation, and microvascular endothelial dysfunction (PMC10932393; PMC10001590; PMC11899429; PMC9894375; doi 10.1016/j.jacc.2006.07.007). REFUTED ON A SECOND READING.
