Heart & blood pressure
You want it optimized, and every lane feeding it here is a lane you want moving: healthy lipid handling, glycemic control, clot-clearing capacity, low glycation and a working endothelium. Note this is the summary node — Blood Pressure, Contractility and Cardiac Remodeling are the specific ones, each with its own drivers and its own direction.
In depth
The integrated cardiovascular picture — heart, vessels and pressure taken together, as an outcome rather than a mechanism.
What drives it
Triglycerides reduces it.
TG/ASCVD causality review 2014, JACC — Mendelian-randomization evidence supports triglyceride-rich lipoproteins as a causal contributor to atherosclerotic cardiovascular disease; their remnants penetrate the arterial wall and deposit cholesterol in plaques. doi:10.1016/j.jacc.2014.09.042. Mendelian randomization, which is genetic and therefore not confounded by lifestyle: two-sample MR across large European meta-analyses gives odds ratios for total triglycerides of 1.25 (95% CI 1.13-1.38) for coronary heart disease and 1.24 (1.11-1.38) for myocardial infarction. UK Biobank MR separately finds remnant cholesterol and triglycerides causally associated with progression from a first cardiometabolic disease to multimorbidity. Mechanism: chylomicron and VLDL remnants rapidly penetrate the arterial wall and deposit cholesterol into lesions. Three things that must travel with this number. (1) it is not settled: causality between plasma triglycerides and atherosclerotic disease remains controversial after more than four decades, and the two landmark randomized trials — strength and reduce-it — reported conflicting results. (2) the effect may not survive adjustment: the triglyceride-CHD association diminished to null when adjusted for HDL-C, and the authors draw the practical conclusion themselves — A treatment targeting a triglyceride pathway that does not also raise HDL-C may not be beneficial. Lowering the number is not the same as lowering the risk. (3) APO B is the stronger target: in the same MR, apolipoprotein B raised CHD and MI risk by 44 and 41 percent versus 25 and 24 for triglycerides — particle number beats particle content. And the effect is disease-specific: triglycerides were causal for CHD and MI but not for ischemic stroke. doi 10.1016/j.jacc.2014.09.042.

Clot formation reduces it.
Why it matters. Verbatim: 'Arterial thrombosis is the cause of myocardial infarction (MI) and stroke, while venous thrombosis (VT) leads to venous thromboembolism (VTE) and pulmonary embolism (PE)' (Eur Heart J, PMID 28039338). Attribution, no quantified risk. PMID 28039338
Reduces sugar damage reduces it.
Yang 2019 — AGE-RAGE signaling drives endothelial dysfunction and diabetic vascular injury. DOI 10.1155/2019/9570616 Verbatim: “AGEs act in the progression of CVD through the modification of extracellular and intracellular proteins and signaling cascades via AGE–RAGE pathways”. doi 10.1155/2019/9570616 · PMC9370094
Blood-sugar support reduces it.
Emerging Risk Factors Collaboration 2010, Lancet 375:2215 — diabetes confers ~2× vascular risk; glucose tracks vascular disease (102-study meta-analysis). PMID 20609967. Cardiovascular complications are predominantly responsible for the increased morbidity and mortality observed in diabetes, and oxidative stress under hyperglycemia is described as the initial link, with AGE-RAGE interaction as the common upstream pathway. this spans both macroangiopathy (aortic, coronary, cerebral, renal and peripheral atherosclerosis) and a distinct diabetic cardiomyopathy — myocardial dysfunction independent of hypertension or coronary artery disease, via cardiomyocyte apoptosis, fibrosis and contractile failure. Metabolic memory — the caveat that changes the practical claim: the literature explicitly frames non-enzymatic glycation of proteins, lipids and nucleic acids as producing a persistent effect. AGEs accumulate preferentially in tissues with low protein turnover, particularly the extracellular matrix, which is why damage outlives the exposure. Restoring glucose control does not immediately unwind what has already been cross-linked. PMID 20609967
Blood flow (nitric oxide) increases it.
Palmer, Ferrige & Moncada 1987, Nature 327:524 — endothelial NO is EDRF, driving vasodilation and lowering vascular tone. PMID 3495737. eNOS is compartmentalised, not free-floating — in cardiac myocytes essentially all eNOS co-immunoprecipitates with caveolin-3, so its activity is set by where it is tethered, and disrupting the caveolin-3/eNOS association is itself a signaling event. PMID 3495737
Calm under stress reduces it.
A search phrased in wellness terms returned four consumer health sites and two reviews about PTSD and chronic stress — no usable anxiety-specific evidence. That returned 46 cohort studies and 2,017,276 participants. The search terms were the problem, not the literature. The numbers: cardiovascular mortality RR 1.41 (1.13-1.76), coronary heart disease RR 1.41 (1.23-1.61), stroke RR 1.71 (1.18-2.50), across 222,253 people with anxiety. A second meta-analysis of 249,846 people over a mean 11.2 years gives CHD HR 1.26 and cardiac death HR 1.48, independent of demographic variables, biological risk factors, and health behaviors. And a null inside the same analysis, which is why the conditional exists: NONFATAL myocardial infarction HR 1.43, 95% CI 0.85 to 2.40, p = 0.180. The point estimate is as large as the significant ones and the interval crosses one. independent of depression, which is the non-obvious UK Biobank, 431,973 people — anxiety only HR 1.72, depression only 2.07, both 2.89, with very little evidence of multiplicative OR additive interaction. The authors recommend anxiety disorder should be considered for inclusion in CVD risk prediction and stratification. The authors’ own whether these associations ARE causal is unclear. All observational; thorough adjustment is not randomization. PMID 27324160 · PMC10377450
Arterial plaque reduces it.
The claim: atherosclerosis progresses through plaque formation involving vascular smooth muscle cells and macrophage uptake of modified LDL; these processes lead to vascular stenosis, plaque rupture, and potentially sudden death. Cerebrovascular ischemia arises from occlusive atheromatous plaques, thrombi OR A hemorrhage and can be immediately fatal. And the distinction that matters clinically is already on this pathway: the Type-I Procollagen → Atherosclerosis link was drawn because the fibrous CAP is collagen and matrix degradation turns a stable lesion into a rupture-prone one. Plaque size and plaque danger are different variables, and it is danger that produces the event. PMC12840960
Cardiac output increases it.
The end stage of most heart disease is heart failure, for which hypertension is both a cause and a risk factor, and heart failure remains a debilitating, steady and efficient killer whose molecular mechanisms remain to be fully understood. The atlas now carries the full arc on this pathway: risk factors drive remodeling, remodeling drives failure, failure drives death. doi 10.1016/j.jacc.2006.07.007 · PMC10932393
