Arterial plaque
Atherosclerosis↓
Also searched as Atherosclerosis.
It is built quietly and early, from two inputs at once — the lipid that gets retained and the inflammation that keeps the wall reactive. Lower both and you are working on the same wall from two sides.
In depth
Cholesterol-laden plaque built into the artery wall over decades, stiffening and narrowing the vessel.
What drives it
Ceramide load increases it.
Ceramides have been detected in atherosclerotic plaques, where they facilitate the transport and entry of oxidized LDL-cholesterol into intimal cells - the initial step in atheroma formation (Adv Nutr 2024). Plasma ceramides predict cardiovascular death beyond LDL-cholesterol across three independent prospective cohorts (Laaksonen 2016, Eur Heart J 37:1967, PMC4929378), and a plasma ceramide score predicted incident myocardial infarction, stroke or cardiovascular death in the randomized PREDIMED trial (hazard ratio 2.18, 95 percent CI 1.36 to 3.49; PMID 28280233). Scope stated plainly: in humans this is association and prediction. The evidence that lowering ceramide prevents events is rodent. PMID 28280233 · PMC4929378
Lowers cholesterol increases it.
Ference 2017, Eur Heart J 38(32):2459-2472 (doi 10.1093/eurheartj/ehx144) -- EAS Consensus Panel appraised over 200 prospective cohorts, Mendelian randomization studies and randomized trials in more than 2 million participants and concluded LDL causes atherosclerotic cardiovascular disease, and log-linearly with cumulative exposure. Contested: Okuyama 2018, Pharmacology 101(3-4):184-218 (PMID 29353277) is a published critical review disputing that consensus. LDL alone is an incomplete panel. Remnant cholesterol carries causal effect on CAD/MI largely independent of LDL-C, and Lp(a) correlates with lesions more strongly than LDL-C once LDL-C is below 104 mg/dL. doi 10.1093/eurheartj/ehx144 · PMID 29353277
Calms inflammation increases it.
Atherosclerosis is a chronic inflammatory disorder, not only lipid deposition. CANTOS proved it causally: canakinumab, which neutralizes IL-1β downstream of NF-κB/NLRP3, significantly reduced recurrent cardiovascular events in prior-MI patients independent of any lipid lowering (PMC5857062; mechanism review PMC11989677). Note: the same trial showed no effect on cardiovascular or all-cause mortality and a higher rate of fatal infection — it establishes the pathway, not a treatment. Mechanism at the initiating step: NF-kappa-B drives endothelial expression of the leukocyte adhesion molecules E-selectin, VCAM-1 and ICAM-1, plus the chemokines MCP-1, IL-8 and CCL20 — which recruit circulating monocytes into the arterial intima. Loss-of-function: inhibiting NF-kappa-B with a super-repressor IkB-alpha blunted all signs of endothelial activation; in vivo, agents that augment endothelial NF-kappa-B and raise ICAM-1/VCAM-1 accelerated atherosclerosis in apoE-knockout mice. A published controversy that must travel with this edge — A journal of clinical investigation perspective is titled 'NF-kappa-B: pivotal mediator OR innocent bystander in atherogenesis?' The causal-versus-correlative question was posed in the title of a major review and is not closed. The adhesion molecules ARE not interchangeable: knockout work found a major role for VCAM-1 but not ICAM-1 in early atherosclerosis (J Clin Invest 2001;107:1255-1262), while P-selectin or ICAM-1 deficiency substantially protected apoE-deficient mice. And VCAM-1 expression MIGRATES as disease progresses — predominating in endothelial cells in early lesions and shifting to intimal cells later. Stage decides which molecule matters. PMC5857062
Blood flow (nitric oxide) reduces it.
A healthy endothelium resists plaque; an activated one initiates it. Oxidized LDL triggers endothelial cells to up-regulate adhesion molecules and chemokines, recruiting the monocytes that become foam cells (EAS Consensus Panel, Eur Heart J 2020;41:2313; PMC11989677). Endothelial dysfunction is therefore the first step, and endothelial NO tone is what opposes it. Convergent finding, and the mechanism is specific: NO inhibits leukocyte adhesion to the vessel wall by interfering with CD11/CD18 binding and by suppressing its expression — and leukocyte adherence is an early event in atherogenesis, which is why NO protects against onset. NO also inhibits DNA synthesis, mitogenesis and proliferation of vascular smooth muscle cells (antiproliferative effects mediated mostly by cGMP), and is a potent inhibitor of platelet aggregation and adhesion. Three separate anti-atherogenic actions, not one. Two distinct output mechanisms, not one: NO acts both through soluble guanylate cyclase / cGMP and through S-NITROSYLATION of reactive protein thiols — an arrow labeled 'NO' collapses two different signaling systems. Localisation changes the output: plasma-membrane-targeted eNOS drives sGC/cGMP signaling more than Golgi-tethered eNOS, so where the enzyme sits alters what it does. The enzyme has preconditions: functional eNOS requires dimerisation, L-arginine substrate and the BH4 cofactor; without BH4 it uncouples and makes superoxide instead of NO (see the Oxidative Stress (ROS) → Endothelial (eNOS) link). And a null result worth carrying: selective endothelial CaMKII inhibition in two transgenic models left systolic and diastolic blood pressure, heart rate, plasma NO and NO-dependent vasodilation unchanged (PMC5653296) — not every proposed upstream regulator of eNOS matters in vivo. Inflammatory alarm increases it.
The population fact: CLONAL hematopoiesis of indeterminate potential affects over 10 percent of individuals older than 70 and confers A 1.5- to 2-fold increased risk of coronary heart disease and all-cause mortality, independent of traditional cardiovascular risk factors. Independent of the traditional factors is the phrase that makes this a separate driver rather than a restatement of the lipid arm. The mouse work names the mechanism: bone marrow reconstitution with TET2-deficient cells increases atherosclerotic plaque size, an effect mediated by NLRP3 inflammasome activation and downstream increases in interleukin-1-beta and interleukin-6. And then the human confirmation, which is the best thing on this pathway: in an exploratory analysis of the CANTOS trial, patients with TET2 CHIP had reduced major adverse cardiovascular event rates when treated with a monoclonal antibody neutralizing IL-1-beta. The mouse mechanism did not merely describe something — it predicted which humans would respond to a drug, and the prediction held in a 10,061-patient randomized trial. Two mutations, two inflammatory profiles, two different drug responses, matching. A paradox recorded rather than smoothed: Jak2-V617F is an activating mutation associated with over a 12-fold increase in incident coronary events — yet deletion of Jak2 in various cell types also paradoxically leads to increased atherosclerosis. Gain and loss of function in the same kinase both make it worse, which no simple sign accommodates. The atlas can express the inflammasome but not the somatic clonal expansion upstream of it, so this edge carries the mechanism while the cause sits outside the map. doi 10.1016/j.jaccao.2025.05.013 · PMC11058018
Oxidative stress increases it.
LDL particles ARE retained beneath the endothelium and undergo oxidative modification under conditions of oxidative stress and myeloperoxidase catalysis, leading to the formation of oxidized LDL. The order matters and is the whole point: retention comes first, oxidation second. Native LDL sitting in the wall is not the lesion — ox-LDL is. That is why the Lipid Metabolism → Atherosclerosis link and this edge are drawn separately rather than collapsed. PMC5666716
Innate immune sensor increases it.
Reviews describe upstream signals converging on core inflammatory hubs, such as the toll-like receptor 4 / nuclear factor-kappa-B pathway and the NLRP3 inflammasome. The proximate trigger is specified: OX-LDL binds to LOX-1 on endothelial cells, activating the NF-kB signaling pathway and subsequently inducing the expression of adhesion molecules. PMC5666716
Limits fat uptake increases it.
Infiltrated monocytes differentiate into macrophages, which together with vascular smooth muscle cells uptake OX-LDL via scavenger receptors such as CD36 and SR-A1. This leads to the formation of foam cells and expansion of the lipid necrotic core. Scavenger receptors ARE unregulated by cholesterol load, unlike the LDL receptor — which is why the macrophage does not stop eating, and why the foam cell forms at all. PMC5666716
Calms body-wide inflammation increases it.
modified lipids activate inflammatory cells in the intima, producing chemokines and cytokines such as TNF, interleukin -1, -4, and -6, and interferon-gamma, which activate other leukocytes, endothelial cells, and adhesion molecules. PMC5666716
Cellular cleanup reduces it.
Autophagy, a cellular process crucial for degrading damaged components, regulates macrophage homeostasis and inflammation. Defective autophagy exacerbates inflammation, impairs cholesterol efflux, and accelerates disease progression. The specific route is unusually well specified: chaperone-mediated autophagy regulates macrophage inflammation by degrading pro-inflammatory cytokines and oxidized LDL; impaired CMA leads to accumulation of these substrates, activating the NLRP3 inflammasome. preclinical studies suggest pharmacological CMA activation may mitigate progression - preclinical is the authors’ word. PMC5666716
Calms histamine response increases it.
Atherosclerosis is characterized by the retention of lipids and inflammatory cells such as macrophages, T lymphocytes, and mast cells in damaged arterial wall, the intima. Mast cells ARE the least secure element here: they appear in a list of infiltrating cell types without a mechanism, effect size or intervention in what was retrieved. Drawn because the observation is direct and repeated; flagged because it is a presence claim, not a causal one. PMC5666716
Tissue remodeling reduces it.
Extracellular matrix remodeling by metalloproteases is named among the processes driving plaque progression and vulnerability, and macrophages induce inflammation progression via interleukin-1-beta, TNF, ROS, and metalloproteases. This is the edge that separates plaque size from plaque danger: the fibrous cap is collagen, and matrix degradation is what turns a stable lesion into a rupture-prone one. A map that only tracked lipid accumulation would rank plaques by the wrong variable. PMC5666716
Blood pressure increases it.
Arterial hypertension is named directly among the mechanisms in a review titled for atherosclerosis, hypertension and coronary artery disease together. This link carries the systemic half of that story. PMC5666716
Immune support increases it.
The intima retains macrophages, T lymphocytes, and mast cells, and reviews name immune cell recruitment and activation and metabolic reprogramming within immune cells among the core processes driving plaque progression and vulnerability. Partly overlaps the Neuroinflammation → Atherosclerosis link, drawn alongside it: that edge carries the cytokine arm, this one the cellular arm. The split is defensible but not sharp, and is stated rather than hidden. PMC5666716
What it drives
It reduces Heart & blood pressure.
The claim: atherosclerosis progresses through plaque formation involving vascular smooth muscle cells and macrophage uptake of modified LDL; these processes lead to vascular stenosis, plaque rupture, and potentially sudden death. Cerebrovascular ischemia arises from occlusive atheromatous plaques, thrombi OR A hemorrhage and can be immediately fatal. And the distinction that matters clinically is already on this pathway: the Type-I Procollagen → Atherosclerosis link was drawn because the fibrous CAP is collagen and matrix degradation turns a stable lesion into a rupture-prone one. Plaque size and plaque danger are different variables, and it is danger that produces the event. PMC12840960
It reduces Cardiac output.
Hypertension and atherosclerosis reviews name cardiac complications including coronary artery atherosclerosis, myocardial ischemia and myocardial hypertrophy and its complications, and the end stage of most heart disease is heart failure. Structurally this completes a loop already half-drawn: the Blood Pressure → Atherosclerosis link and the Blood Pressure → Contractility link both exist, and this edge says the two targets are not independent - hypertension reaches the myocardium both directly through pressure load and indirectly by narrowing its supply. The atlas draws one cardiac node covering both ischemic and hypertensive heart disease; that compression is stated. PMC12840960
It reduces Sexual function.
The clinical corollary that makes this pathway worth tracking: ED may function not only as a complication of T2DM but also as an early clinical marker of cardiometabolic disease, with penile vascular changes paralleling vascular abnormalities seen in coronary and peripheral arteries. Smaller vessels fail earlier, so the symptom arrives before the heart attack. doi 10.3390/jcm15041621 · PMID 33923709 · PMC11988752

It reduces Female libido.
Named across five reviews of erectile and female arousal physiology. Key contributors include reduced nitric oxide bioavailability due to endothelial oxidative stress and inflammation, diminished cavernosal smooth muscle content leading to impaired veno-occlusion, and increased arterial stiffness compounded by metabolic disorders. doi 10.21037/tau-2025-419 · PMID 19138376 · PMC11988752
