What drives it
Reduces scarring increases it.
Wang 2013, PLoS One — TGF-β/Smad2/3 signaling drives type I/III collagen synthesis in human dermal fibroblasts. doi:10.1371/journal.pone.0055899. TGF-beta is described as a potent inducer of collagen synthesis; in normal human dermal fibroblasts TGF-beta signaling increased collagen production, and in human cardiac myofibroblasts TGF-beta1 induced collagen synthesis, gel contraction and structural ECM remodeling. the route runs through fibroblast-to-myofibroblast transition — myofibroblasts do not generally exist in normal tissue and appear in injured, fibrotic and cancer tissue. This is a REPAIR/PATHOLOGY program, not baseline collagen maintenance. Fn14 siRNA blocked ECM gene expression even when TGF-beta1 was active, so the arrow is not sufficient on its own. doi 10.1371/journal.pone.0055899.
Matrix-degrading enzymes reduces it.
The shared substrate is structural collagen itself. Verbatim from the cathepsin comparison: 'a protease with intense collagenase activity' (PMC2951121) - MMPs are the metal-dependent collagenases of the same substrate class. PMC2951121
Vitamin C increases it.
Required cofactor for prolyl and lysyl hydroxylase. Carried by the intermediary-metabolism, micronutrient and clinical reviews. doi 10.3390/nu18132141 · PMC9967593
Copper status increases it.
Lysyl oxidase is a copper enzyme - copper sits behind collagen and elastin cross-linking, so connective tissue strength depends on it. Carried by the intermediary-metabolism, micronutrient and redox reviews (PMC9967593; doi 10.3390/nu18132141; PMC12673670; doi 10.1016/j.molmet.2023.101755). Verbatim: “Lysyl oxidase (LOX) is a copper-dependent monoamine oxidase whose primary function is the covalent cross-linking of collagen and elastin in the extracellular matrix (ECM)”. doi 10.3390/nu18132141 · PMID 42100872 · PMC9967593
Glycine calming increases it.
Collagen is roughly one third glycine by residue, and estimates of endogenous synthesis fall short of what collagen turnover plus glutathione and creatine synthesis together demand. Carried by the intermediary-metabolism, micronutrient and redox reviews (PMC9967593; doi 10.3390/nu18132141; PMC12673670; doi 10.1016/j.molmet.2023.101755). Verbatim: “Thus, increasing glycine in the diet may well be a strategy for helping cartilage regeneration by enhancing collagen synthesis, which could contribute to the treatment and prevention of osteoarthritis”. doi 10.3390/nu18132141 · PMID 30006659 · PMC9967593
Thyroid hormone conversion increases it.
Mice lacking type 2 iodothyronine deiodinase had a roughly 50% reduction in bone formation rate - the rate at which osteoblasts lay down type-I collagen matrix - caused by local intracellular T3 deficiency despite normal serum T3, showing that deiodinase-generated T3 is required to sustain type-I collagen matrix production. doi 10.1073/pnas.0911346107 · PMID 20368437
Galectin-3 increases it.
Galectin-3 is 'secreted by activated macrophages in the heart and promotes cardiac fibrosis by activating fibroblasts to produce excessive collagen and scar tissue'. Genetic evidence in both directions: 'galectin-3 knockout mice are protected from developing fibrosis in response to injury, while mice with elevated galectin-3 develop severe fibrotic changes.' Pharmacological confirmation: 'Genetic and pharmacological inhibition of galectin-3 prevents cardiac remodeling by interfering with myocardial fibrogenesis' (Circ Heart Fail 2013;6:107-117). Limit recorded: a pressure-overload study reached the opposite conclusion - 'galectin-3 loss did not affect survival, systolic and diastolic dysfunction, cardiac fibrosis, and cardiomyocyte hypertrophy' and 'galectin-3 is not a critical modulator of cardiac fibrosis' (PMID 26948424). That is transverse aortic constriction, not an injury model, and the two do not measure the same disease. The direction is right but lower is not always better. A cardiac review that separates pathology from physiology states both halves: 'Genetic deficiency of Gal-3 delays wound healing, impairs cardiac remodeling and function after AMI. On the contrary, Gal-3 deficiency shows opposite results with improved remodeling and function in other cardiomyopathies and in hypertension.' And in normal aging: 'genetic absence of Gal-3 in aged mice exacerbates pathological hypertrophy and increases fibrosis, as opposed to reduced fibrosis shown in cardiac disease.' The governing sentence: galectin-3 drives fibrogenesis 'which may favor limitation of inflammation and wound healing, unless the fibrotic process proceeds beyond tissue repair.' The node is now d:'dur' — the harm is the duration, not the protein. PMID 26948424
Cardiac scarring cells increases it.
10.3390/cells10071716 — phase three — where a recoverable illness becomes a permanent one. 'profibrotic cytokines and mediators released during acute myocarditis in susceptible individuals activate fibroblasts and recruit new fibroblast differentiation via endothelial mesenchymal transition leading to cardiac remodeling and dilated cardiomyopathy' (Mayo/Elsevier chapter — Viral Myocarditis and Dilated Cardiomyopathy: Mechanisms of Cardiac Injury, Inflammation, and Fibrosis). Scar is not muscle. It does not contract, and once laid down it stays. This is the step that decides whether the heart recovers its function OR keeps a permanent deficit. doi 10.3390/cells10071716
What it drives
It increases Skin.
Dermal-aging review 2019, Int J Mol Sci — dermal collagen provides the skin's mechanical support and elasticity; collagen loss drives dermal atrophy and wrinkling. PMC6540032
It reduces Cardiac output.
Hypertrophy and fibrosis ARE significant characteristics of pathological cardiac remodeling, contributing to heart failure, and metabolic abnormalities activate pro-fibrotic pathways resulting in cardiac stiffening and diastolic dysfunction. A causal caveat the sources make themselves: these studies associate cardiac lipid accumulation with cardiac fibrosis, but a causal relationship has not been demonstrated. The fibrosis-to-dysfunction link is solid; some of the upstream routes into fibrosis are not. doi 10.1016/j.jacc.2006.07.007 · PMC10932393
It reduces Arterial plaque.
Extracellular matrix remodeling by metalloproteases is named among the processes driving plaque progression and vulnerability, and macrophages induce inflammation progression via interleukin-1-beta, TNF, ROS, and metalloproteases. This is the edge that separates plaque size from plaque danger: the fibrous cap is collagen, and matrix degradation is what turns a stable lesion into a rupture-prone one. A map that only tracked lipid accumulation would rank plaques by the wrong variable. PMC5666716
It reduces Oxidative stress.
10.3389/fbioe.2022.908033 — The heart-failure and atherosclerosis reviews both discuss fibrosis and oxidative stress in the same breath: oxidative stress and chronic, low-grade inflammation ARE important contributors, and metabolic abnormalities activate pro-fibrotic pathways resulting in cardiac stiffening. Sign and direction ARE both less settled than the source count suggests, and this is exactly the known false-positive mode: twelve sources touching both nodes is not twelve sources claiming a relation. Drawn inhibitory on the reading that established fibrotic tissue sustains oxidative stress rather than resolving it, but the dominant documented direction in these reviews runs the Oxidative Stress (ROS) → Type-I Procollagen link. doi 10.3389/fbioe.2022.908033
It increases Muscle.
Loss of fibronectin-rich ECM, coupled with fibrotic changes and adipocyte infiltration, disrupts satellite cell QUIESCENCE. The matrix is not scaffolding - it is the stem cell niche, and losing it costs the repair capacity as well as the structure. doi 10.3390/ijms26146740 · PMC13074595
It reduces Cellular aging.
Named across five inflammaging and hallmarks-of-aging reviews as part of the shared mechanism set that links molecular damage to organ decline: nutritional perception dysregulation and inflammatory aging ARE the core key elements linking upstream molecular damage to downstream organ function decline. doi 10.1016/j.molmet.2023.101755 · PMC11233824
It increases Kidney.
Named across five reviews of the cardiovascular-renal-hepatic-metabolic syndrome as part of the shared mechanism set: ectopic fat accumulation triggered by metabolic reprogramming, oxidative stress and inflammation induced by energy overload, modified lipids, UREMIC toxins, and senescence, as well as insulin resistance pathways activated by pro-inflammatory factors and lipotoxic products, collectively exacerbate simultaneous hepatic and renal injury. (doi 10.1038/s41581-026-01066-0; doi 10.3390/ijms26146962; PMC11725408; PMC11906131; PMC11764088; PMC12257864). REFUTED ON A SECOND READING. It increases Bone health.
Named across six reviews of bone remodeling and osteoarthritis pathogenesis. Bone remodeling relies on the coordinated actions of bone-resorbing osteoclasts, bone-forming osteoblasts, and osteocytes serving as mechanosensors and local remodeling coordinators. PMC10648156
It reduces Joint wear.
The subchondral bone is as involved as the cartilage: an abnormal expression of OPG and RANKL has been described in osteoarthritis osteoblasts, which is responsible for abnormal bone remodeling and decreased mineralization. And the direction splits by subtype: low osteoarthritis osteoblasts show decreased OPG and increased RANKL, while high osteoarthritis osteoblasts show increased OPG and reduced RANKL. Two phenotypes, opposite ratios, one node here. The authors are blunt: osteoarthritis pathogenesis is still poorly understood and there ARE NO treatments available to repair degraded cartilage and altered bone. PMC10648156
It increases Liver.
Named across six gut-liver axis reviews. The main alteration is dysbiosis - A decrease in beneficial bacteria, an increase in potentially harmful microbes, and a loss of overall microbial diversity - with intestinal barrier integrity determining the levels of microbial metabolites, ENDOTOXINS and ammonia that reach the liver and systemic circulation. (doi 10.1038/s41522-022-00352-6; doi 10.3389/fimmu.2025.1638197; PMC11265038; PMC12608333; doi 10.1016/j.jhep.2019.10.003). REFUTED ON A SECOND READING. It reduces Clears blocked nose & sinuses.
This is the same axis as eczema, and it splits the same way: type 2 endotypes are IL-4/IL-5/IL-13-driven and eosinophilic, while non-type-2 endotypes ARE driven by increased Th1 and Th17 responses, resulting in elevated neutrophil levels. Two endotypes, two cell types, two nodes. And the intervention evidence is unusually direct: dupilumab (IL-4Ralpha inhibitor) reduces polyp burden and restores smell, while MEPOLIZUMAB (anti-IL-5) and OMALIZUMAB (anti-IgE) address specific endotypes - three biologics, three targets, three endotype-matched responses. It reduces Urate clearance.
The architecture is unusually clean and worth stating: gout has two separable stages. Stage one is metabolic - urate underexcretion through renal and gut mechanisms is the major mechanism for hyperuricemia in most people, via URAT1, GLUT9 and ABCG2. Stage two is immunological - MSU crystals function as damage-associated molecular patterns which, upon phagocytosis by macrophages, activate the NLRP3 inflammasome through a two-step process: priming via NF-kB signaling and activation via lysosomal rupture, potassium efflux, and mitochondrial ROS. And the two stages come apart in people: not all individuals with hyperuricemia show deposition of MSU crystals OR develop gout. Local factors decide - reduced joint temperature, low pH, high sodium concentration, and preexisting joint damage. A urate number is not a diagnosis. The treatment target is explicit: long-term lowering of serum urate to less than 360 MICROMOL/L leads to crystal dissolution and ultimately to suppression of flares. PMID 39712248 · PMC8268734
