You want it running: robust autophagy is one of the clearest signatures of a long, healthy cellular life.

In depth

The cell's self-cleaning and recycling program — it digests worn proteins and organelles and renews the interior.

What drives it

Stress-resistance switch (FOXO) increases it.

Mammucari 2007, Cell Metab 6:458 — FoxO3 is necessary and sufficient to induce autophagy in vivo; blocking FoxO3 abolishes fasting-induced autophagosome formation. Direct binding, not correlation: ChIP-seq shows FOXO3 directly binds a network of autophagy target genes in adult neural stem and progenitor cells, and the authors describe FOXO3 as a switch that induces autophagy. Functional consequence and rescue: in the absence of FOXO activity protein aggregates accumulate in those cells, and that accumulation is reversed by TOR inhibition — loss of function, phenotype, and pharmacological rescue in one experiment. The target genes ARE named in muscle too: FOXO3 upregulates LC3, BNIP3, GABARAPL1, VPS34, ULK2 and ATG12 during atrophy. The authors state their own limit, and it matters: a number of autophagy genes in the direct FOXO3 network were not transcriptionally regulated by FOXO3 — binding is not the same as driving, and they suggest additional co-factors are required for full activation. Scope, and it cuts both ways: the same FOXO program that clears aggregates also drives muscle atrophy — FoxO raises MAFbx/atrogin-1, MuRF1, ZNF216, Mul1 and BNIP3, and FoxO3 acts through both the ubiquitin-proteasome and autophagy-lysosome systems. In the heart, FoxO3 induces reversible cardiac atrophy. Autophagy here is a catabolic program, not a purely protective one.

Fasting response increases it.

Energy deprivation activates AMPK, which phosphorylates ULK1 (Ser317) and initiates phagophore formation; a 72-hour fast raises the autophagosome marker LC3b-II in skeletal muscle (PMC6831946). Short-term food restriction also induces profound neuronal autophagy with reduced mTOR activity (DOI 10.4161/auto.6.6.12376). Measured by markers, not inferred: fasting raises AMPK phosphorylation and inhibits mTOR, producing increased expression of LC3-II, beclin-1 and ATG proteins — the standard autophagy readouts. The downstream purpose is stated too: autophagy raises free amino-acid availability to support gluconeogenesis and hold glycemia. The reviewers themselves state the limit: most data come from animal studies OR short-term human trials, raising questions about generalisability to long-term clinical outcomes, and the degree to which autophagy induction produces observable improvement in human metabolic health is still not fully understood. doi 10.4161/auto.6.6.12376 · PMC6831946

Energy sensing increases it.

Kim, Kundu, Viollet & Guan 2011, Nat Cell Biol 13:132 — AMPK directly phosphorylates ULK1 (Ser317/777), initiating autophagy. DOI 10.1038/ncb2152. Verified: AMPK promotes autophagy by phosphorylating ULK1 and by phosphorylating Raptor to relieve mTORC1's inhibitory Ser757 phosphorylation of ULK1, allowing Atg12-Atg5-Atg16L1 assembly, LC3 lipidation and autophagosome biogenesis. The prevailing view is under active challenge and the atlas should say so: a 2024 Molecular Cell paper titled 'opposing roles for AMPK in regulating distinct mitophagy pathways' states that the role of AMPK as a general ULK1 activator is not necessarily true — some AMPK sites on ULK1 are stimulatory, but the authors identified two that ARE not. AMPK phosphorylation at ULK1 Ser694 together with Ser556 is required for 14-3-3 binding, which sequesters ULK1 and prevents phagophore formation during mitophagy initiation. They propose ULK1 acts as a logic gate integrating AMPK and mTOR: under energy starvation AMPK inhibits ULK1 to prevent excess bulk autophagy, and that signal is overridden only when amino acids are absent. Their rationale: autophagy may itself be an energy-consuming pathway. The sign of this arrow is site-dependent and contested. doi 10.1038/ncb2152.

Longevity gene increases it.

Lee 2008, pnas 105:3374 — SIRT1 deacetylates Atg5/7/8, required for autophagy induction. DOI 10.1073/pnas.0712145105. Mechanism is direct deacetylation of the autophagy machinery itself: SIRT1 deacetylates FOXO3, beclin 1, atg proteins and LC3. Loss-of-function: SIRT1-silenced cells showed down-regulated autophagy in both parental and chemoresistant lines. demonstrated in HepG2 and Huh-7 hepatocellular carcinoma cell lines in a sorafenib-resistance model — a cancer-drug-resistance context, not normal tissue physiology, and SIRT1 in that setting was elevated alongside activated nf-kappa-B, which runs opposite to the suppressive relationship above. Context decides. Essential, not merely contributory: deacetylation of FOXO by SIRT1 plays an essential role in mediating starvation-induced autophagy in cardiac myocytes (Hariharan 2010, Circ Res 107:1470). But the linear model is complicated by its own opposite: the perception of a linear SIRT1-FOXO axis is complicated by findings that acetylated FoxO1 — the UNdeacetylated form — binds ATG7 in the cytoplasm and drives autophagy directly, under prolonged stress when FoxO1 dissociates from cytoplasmic SIRT2 and becomes hyperacetylated. Both acetylation states can reach autophagy by different routes. doi 10.1073/pnas.0712145105.

Growth restraint reduces it.

Kim, Kundu, Viollet & Guan 2011, Nat Cell Biol 13:132 — mTORC1 phosphorylates ULK1 Ser757, blocking autophagy initiation. DOI 10.1038/ncb2152. Two separate blocks at two stages. Initiation: mTORC1 phosphorylates ULK1 at SER757, which disrupts ULK1's ability to interact with AMPK — the brake is applied by breaking a protein-protein contact, not by destroying the enzyme (Egan 2011; Kim 2011). Maturation: mTORC1 separately phosphorylates PACER at Ser157 and WIPI2 at Ser395 to suppress autophagosome maturation. Pharmacological proof: rapamycin activates autophagy, and when mTORC1 is inhibited by nutrient lack autophagy is initiated quickly as a compensatory recovery of nutrients from the cell's own components. There is a return arrow: ULK1 phosphorylates Raptor at Ser859/Ser792 and hinders substrate docking, inhibiting mTORC1 back — a negative feedback loop that keeps mTORC1 off once autophagy has started. doi 10.1038/ncb2152.

It reduces Arterial plaque.

Autophagy, a cellular process crucial for degrading damaged components, regulates macrophage homeostasis and inflammation. Defective autophagy exacerbates inflammation, impairs cholesterol efflux, and accelerates disease progression. The specific route is unusually well specified: chaperone-mediated autophagy regulates macrophage inflammation by degrading pro-inflammatory cytokines and oxidized LDL; impaired CMA leads to accumulation of these substrates, activating the NLRP3 inflammasome. preclinical studies suggest pharmacological CMA activation may mitigate progression - preclinical is the authors’ word.
