The Virus’s Own Scissors
The virus makes one long protein and must cut it into working pieces. The same blade also cuts the host — the scaffold that holds the muscle cell together, the machinery that makes new protein, and the recycling system that clears damage. It is one of the two steps the field names as a real drug target.
In depth
A viral enzyme that cuts host proteins the heart muscle cell depends on.
What drives it
Coxsackievirus B3 increases it.
The virus brings its own scissors. A coxsackievirus makes one long protein and must cut it into pieces — and the two blades it uses for that also cut the host. 'In CVB3 infections, viral proteases 2A and 3C can impair cellular functions by cleaving an array of host proteins' (pmc10178405 · PMID 37175422 — Role of Coxsackievirus b3-Induced Immune Responses in the Transition from Myocarditis to Dilated Cardiomyopathy and Heart Failure). These are the steps the field names as the actual druggable targets — blocking the virus at attachment, entry, uncoating, RNA or protein synthesis rather than fighting the inflammation afterwards. PMID 37175422
Blood flow (nitric oxide) reduces it.
A defense the body already has — and it works by jamming the virus's scissors. 'nitric oxide inhibits dystrophin PROTEOLYSIS by coxsackieviral protease 2A through S-NITROSYLATION: a protective mechanism against enteroviral cardiomyopathy' (Badorff C, Fichtlscherer B, Rhoads RE, Zeiher AM, Muelsch A, Dimmeler S, et al. — Nitric oxide inhibits dystrophin proteolysis by coxsackieviral protease 2A through S-nitrosylation: a protective mechanism against enteroviral cardiomyopathy. Circulation 2000;102(18):2276-2281). ⇒ S-NITROSYLATION means attaching a chemical group to the enzyme's active site so it cannot cut. The body's own signaling molecule disables the blade that would cut the muscle cell's scaffold. And the direction is disease-specific: the same molecule is anti-lipolytic in human fat tissue, where raising it reduces fat release. Helpful here, unhelpful there. The sign belongs to the tissue, not to the molecule.
What it drives
It reduces Muscle cell scaffold.
It cuts the scaffold that holds the muscle cell together — and that is a mechanical route to an enlarged heart, not an inflammatory one. 'protease 2A inhibits host cell protein synthesis, cleaves host protein dystrophin, and may induce cardiomyopathy and cardiomyocyte apoptosis' (doi 10.1007/s10495-006-0013-0 — Chau DHW et al. Coxsackievirus B3 proteases 2A and 3C induce apoptotic cell death through mitochondrial injury and cleavage of eIF4GI, Apoptosis 2007;12:513-524). It was shown directly in heart cells: 'dystrophin and eIF4G-1 ARE cleaved during coxsackievirus B3 infection of cultured myocytes' — rat neonatal ventricular cardiomyocytes, by western blot (Nat Med — Enteroviral protease 2A cleaves dystrophin: evidence of cytoskeletal disruption in an acquired cardiomyopathy). ⇒ dystrophin is the same protein whose inherited loss causes muscular dystrophy with heart failure. The virus produces an acquired version of a genetic disease. doi 10.1007/s10495-006-0013-0
It reduces Cellular cleanup.
It also disables the cell'S recycling crew. 'CVB3 protease 2A cleaves atg4a' and 'cleavage of atg4a was associated with a loss of autophagy function of the truncated cleavage fragment' (PMC9502984 — Coxsackievirus Protease 2A Targets Host Protease ATG4A to Impair Autophagy). ⇒ autophagy is how a cell clears damaged parts and some pathogens. The virus cuts the enzyme that runs it. PMC9502984
It reduces The Handbrake the Virus Releases.
And this is the cruellest trick in the whole MAP: the virus releases the brake on the cell's self-destruct. 'both 2A and 3C are required for cleavage of CARD8, which normally functions to inhibit activation of CASP1 and pyroptosis' (pmc10178405 · PMID 37175422 — Role of Coxsackievirus b3-Induced Immune Responses in the Transition from Myocarditis to Dilated Cardiomyopathy and Heart Failure). ⇒ CARD8 is a handbrake. Cutting it lets the inflammatory cell-death machinery run. the bursting cell death is not an accident of the infection — the virus actively unlocks it. PMID 37175422

It reduces Protein synthesis hub.
Proof of the hardest kind: put the virus's scissors in a heart with NO virus, and the heart fails. 'transgenic mice with cardiac-specific localization of protease 2A develop severe cardiac dysfunction and dilated cardiomyopathy. These findings suggest that inhibiting protease 2A activity might be an effective method to treat CVB3 infection' (pmc11125614 — Caboxamycin Inhibits Heart Inflammation in a Coxsackievirus b3-Induced Myocarditis Mouse Model). ⇒ NO infection. NO immune attack. Just the enzyme — and the heart enlarges and weakens anyway. It also cuts the cell's protein-making machinery: an agent that works here 'substantially decreased CVB3 VP1 production and cleavage of translation factor eIF4G1'.
