Coxsackievirus B3
Cardiotropic enterovirus load↓

A short list of common viruses causes most of these cases, and they reach the heart because a protein on the muscle cell lets them in. The virus does not always leave when the illness appears to end — viral genetic material is found in a large share of people whose hearts later enlarge and weaken.
In depth
The virus family most often behind inflammation of the heart muscle.
What drives it
Viral entry receptor increases it.
The cause, and the door it comes through. This illness usually begins with a specific family of viruses and a specific protein on the heart cell that lets them in. 'enteroviruses and adenoviruses ARE cardiotropic and can directly infect cardiomyocytes by binding the coxsackievirus and adenovirus receptor (CAR), inducing necrosis AND/OR apoptosis through viral replication' (Front Cardiovasc Med 2026, doi 10.3389/fcvm.2026.1821475). The most frequently implicated agents are named: 'The most frequently implicated is PARVOVIRUS B19, followed by enteroviruses (e.g. coxsackieviruses and echoviruses) and adenoviruses.' and the virus does not always leave: viral genomes are reported 'in up to ~70% of patients with idiopathic dilated cardiomyopathy'. doi 10.3389/fcvm.2026.1821475
Antiviral signal reduces it.
The signal that fights the virus. Interferon-γ is the arm the body uses to clear a cardiotropic infection, and raising it is a measured antiviral mechanism in this exact model — an agent that 'exerts antiviral effects against CVB3 by upregulating expression of IFN-gamma mRNA' reduced virus titres, morbidity and mortality (PMID 16495755 — Astragaloside IV exerts antiviral effects against coxsackievirus B3 by upregulating interferon-gamma). But the same cytokine is elevated in the inflamed heart and is one of the markers a successful treatment lowers. Up early is protective; up late is damage. The direction depends on the phase. PMID 16495755
Cardiac scarring cells increases it.
The cell that makes the scar is also the cell the virus prefers — and that reframes the whole illness. 'We found that in addition to cardiomyocytes cardiac fibroblasts were infected by CVB3 and displayed a higher virus replication (132-fold increase) compared to cardiomyocytes (14-fold increase) between 6 and 24 hours after infection' (pmc4211177 — Cardiac Fibroblasts Aggravate Viral Myocarditis: Cell Specific Coxsackievirus B3 Replication). And they shout louder too: 'the expression of TNF-α, IL-6, MCP-1, and IFN-β was significantly increased in cardiac fibroblasts compared to cardiomyocytes OR macrophages'. ⇒ nearly ten times the viral output of the muscle cell, and more inflammatory signal. The same cell then lays down the scar that replaces the muscle the virus destroyed. It is not a late-stage bystander — it is there from the first day, amplifying the infection it will later paper over.
Antiviral sensor relay reduces it.
It blinds the alarm before the alarm can ring. '3C protease cleaves mitochondrial antiviral-signaling protein (mavs) and trif, which are important for downstream antiviral responses such as the activation of NF-κB, 2′-5′ OAS, PKR, and Mx, and caspase-dependent apoptosis' (pmc10178405 · PMID 37175422 — Role of Coxsackievirus b3-Induced Immune Responses in the Transition from Myocarditis to Dilated Cardiomyopathy and Heart Failure). And 2A does the same from another angle: it 'disrupts NF-κB activation by cleaving NFAT5 and SQSTM1'. ⇒ the cell has a sensor that would raise the antiviral defense. the virus cuts the sensor first. PMID 37175422
Muscle cell scaffold reduces it.
And here is why the virus cuts it: the scaffold is also the cage. 'cleavage of dystrophin may have a role in release of the virus from the myocyte since vIRAL infection is increased in the absence of dystrophin' (PMID 18357777 — cvb infection and mechanisms of viral cardiomyopathy). ⇒ intact scaffold holds the virus in. Cut it, and the virus gets out to infect the next cell. the damage and the escape route are the same event. that also explains the human observation that people born without working dystrophin carry extra cardiac risk from this infection. PMID 18357777

It increases The Other Way In.
10.3390/v13050718 — the second door. The heart's viral receptor is not alone — cardiotropic viruses also use decay-accelerating factor (daf/CD55) alongside the coxsackie-adenovirus receptor. An agent that blocks one doorway has not necessarily closed the other. doi 10.3390/v13050718
It increases Inflammatory alarm.
10.1038/s41598-018-20686-6 — this one was confirmed in actual human hearts, not only in mice. 'The presence of the NLRP3 inflammasome has been shown in endomyocardial biopsies of patients with acute myocarditis' (ScienceDirect Topics — Coxsackievirus B3 overview (citing Toldo et al. 2014; Wang, Gao & Xiong 2014; Wang et al. 2018)). Most of what is known about this illness comes from animals — this step was found in tissue taken from living patients. And the chain is timed: 'Infection with coxsackievirus B3... is associated with enhanced NLRP3 activation together with increased ASC, caspase-1 and IL-1β expression observed within 7 days from the infection in mice.'. doi 10.1038/s41598-018-20686-6
It increases The Virus’s Own Scissors.
The virus brings its own scissors. A coxsackievirus makes one long protein and must cut it into pieces — and the two blades it uses for that also cut the host. 'In CVB3 infections, viral proteases 2A and 3C can impair cellular functions by cleaving an array of host proteins' (pmc10178405 · PMID 37175422 — Role of Coxsackievirus b3-Induced Immune Responses in the Transition from Myocarditis to Dilated Cardiomyopathy and Heart Failure). These are the steps the field names as the actual druggable targets — blocking the virus at attachment, entry, uncoating, RNA or protein synthesis rather than fighting the inflammation afterwards. PMID 37175422

It increases Islet inflammation.
The leading candidate environmental trigger. Meta-analysis of 60 studies, 12,077 participants: enteroviruses associated with islet autoimmunity at OR 2.1 (95% CI 1.3-3.3; p=0.002) and with type 1 diabetes at OR 8.0 (4.9-13.0; p<0.0001); Enterovirus B specifically OR 12.7 (4.1-39.1). Scope in full: association, not proven causation - 'the demonstration of a causal relationship is missing, and is likely to remain elusive until tested in humans' (PMID 36884282). Heterogeneity was substantial (I-squared 63-85%); the authors state 'study design and quality varied'. PMID 36884282
It increases Autoantibodies.
Verbatim: 'Prospective epidemiological studies have strongly associated the persistence of enteroviruses, especially coxsackievirus B (CVB), with the appearance of islet autoantibodies and an increased risk of T1DM' (doi 10.1038/s41574-022-00688-1). The association is 'linked to autoimmune initiation (ie, islet autoantibody seroconversion) rather than to clinical progression'. initiation only - it does not predict how fast an individual progresses after seroconversion. doi 10.1038/s41574-022-00688-1
It reduces Protects insulin cells.
Verbatim: 'CVB can persist in pancreatic ductal and beta-cells, which leads to structural or functional alterations of these cells, and to a chronic inflammatory response that promotes recruitment and activation of pre-existing autoreactive T cells and beta-cell autoimmune destruction' (doi 10.1038/s41574-022-00688-1). Scope and a direct contradiction recorded: a 2025 single-cell study of primary human islets reports 'no evidence of preferential viral tropism for beta and ductal cells... with ductal cells, not beta cells, exhibiting the strongest interferon- and HLA-associated responses'. Beta-cell-selective tropism is contested. doi 10.1038/s41574-022-00688-1
It reduces Pancreatic ducts.
10.1128/msphere.00036-23 — Verbatim, and it contradicts the received view: 'Contrary to prior claims, our analysis reveals no evidence of preferential viral tropism for beta and ductal cells, instead demonstrating comparable infection rates across both endocrine and exocrine compartments. Beta, alpha, and ductal cells show the most robust transcriptional responses to infection, with ductal cells, not beta cells, exhibiting the strongest interferon- and hla-associated responses' (Cell Reports, s2211124725009829, primary human islets, single-cell). This 2025 study disagrees with the beta-cell-tropism literature cited on the CVB3 to beta-cell link. Both sides are shown on purpose. doi 10.1128/msphere.00036-23
