You want it quiet: the attack, not the glucose, is what ends the beta cell.

In depth

Insulitis — immune cells infiltrating the islet and attacking the beta cells inside it. This is the actual lesion of type 1 diabetes, and it runs silently for years before blood sugar ever moves.

What drives it

Immune balance reduces it.

Verbatim: 'Cytokines produced by immune cells infiltrating pancreatic islets have been implicated as one of the important mediators of beta-cell destruction in insulin-dependent diabetes mellitus' (PMID 12754418). IL-1beta and IFN-gamma are the named effectors. `immune modulation` is a broad node covering many autoimmune territories; this edge claims only the islet-directed arm. PMID 12754418

Th1 immune arm increases it.

Verbatim: vitamin D in pregnancy is hypothesized to work by 'down-regulating the T helper type 1 (Th1) pro-inflammatory cytokine responses, that possibly contribute to the destruction of beta-cells' (PMC4452099, D-Tect). Corroborated by the effector cytokines measured twice on this build: IFN-gamma and IL-17 fell with omega-3 in new-onset children, and IFN-gamma with IL-1beta was the challenge EGCG protected against. the source states the Th1 link as a hypothesis ('possibly contribute'), and the corroboration is cytokine-level, not a Th1-depletion experiment. PMC4452099

Antiviral signal increases it.

The effector cytokine that reaches the islet. Verbatim: 'Cytokines produced by immune cells infiltrating pancreatic islets have been implicated as one of the important mediators of beta-cell destruction in insulin-dependent diabetes mellitus' (PMID 12754418), IL-1beta and IFN-gamma named as the challenge pair. Corroborated twice on this build: IFN-gamma and IL-17 fell with omega-3 in new-onset T1D children, and IFN-gamma/IL-1beta was the cocktail both EGCG and curcumin protected against. `ifng` is the antiviral arm as well - IFN-gamma fights viruses and cancer. This edge claims the islet-directed consequence, not that IFN-gamma is undesirable. PMID 12754418

Coxsackievirus B3 increases it.

The leading candidate environmental trigger. Meta-analysis of 60 studies, 12,077 participants: enteroviruses associated with islet autoimmunity at OR 2.1 (95% CI 1.3-3.3; p=0.002) and with type 1 diabetes at OR 8.0 (4.9-13.0; p<0.0001); Enterovirus B specifically OR 12.7 (4.1-39.1). Scope in full: association, not proven causation - 'the demonstration of a causal relationship is missing, and is likely to remain elusive until tested in humans' (PMID 36884282). Heterogeneity was substantial (I-squared 63-85%); the authors state 'study design and quality varied'. PMID 36884282

Gut barrier reduces it.

A causal demonstration with two counterfactuals - rare at any level of evidence. Verbatim: 'breakage of the gut barrier integrity in BDC2.5XNOD mice carrying a transgenic T cell receptor (TCR) specific for a beta cell autoantigen leads to activation of islet-reactive T cells within the gut mucosa and onset of T1D' (pnas, doi 10.1073/pnas.1814558116). Counterfactual one: 'The intestinal activation of islet-reactive T cells requires the presence of gut microbiota and is abolished when mice are depleted of endogenous commensal microbiota by antibiotic treatment.' counterfactual two: 'infection with barrier-disrupting C. rodentium accelerated insulitis in NOD mice, while NOD mice failed to develop insulitis when infected with the DeltaespF strain, which cannot disrupt gut integrity.' sign inh: barrier integrity up means insulitis down. The evidence covers the causal work is murine. In humans the finding is that permeability changes precede onset - association, not proof. doi 10.1073/pnas.1814558116

Regulatory T cells reduces it.

Verbatim: 'The maturation of regulatory T cells (Tregs) expressing Foxp3 transcription factor is crucial for immune homeostasis and induction of tolerance... Numerous studies have revealed that reduced frequency OR function of FoxP3+ Tregs in NOD mice is a major susceptibility factor for T1D' (PMC7105744). The link runs inversely: Treg function up means insulitis down. murine susceptibility evidence; no supplement on this blend has been shown to raise human Treg function, so this edge explains the disease, it does not license a claim. PMC7105744

What it drives

It reduces Protects insulin cells.

The lesion itself. Verbatim: 'Over years, immune infiltration into pancreatic islets leads to beta cell damage, impairment of cell function, and destruction of beta cells' (Diabetes in America, NCBI Bookshelf NBK604182). this is the defining mechanism of type 1; it is not the mechanism of type 2, where beta-cell reserve fails without an immune siege.

It increases Type 1 diabetes.

Verbatim: 'Type 1 diabetes results from autoimmune destruction of insulin-producing beta cells' (NBK604182). Seroconversion to islet autoantibodies - IAA, GAD, IA-2, ZnT8 - marks the process years before diagnosis. insulitis is necessary but the timeline is variable; not everyone who seroconverts progresses at the same rate.

It increases Autoantibodies.

The siege leaves fingerprints. Verbatim: 'Type 1 diabetes-related autoantibodies were first described in the early 1970s... including insulin autoantibodies (IAA) and autoantibodies to glutamic acid decarboxylase (GAD), an aborted tyrosine phosphatase (islet antibody-2 or IA2), and the zinc transporter (ZnT8), all components of beta cells' (NBK604182). autoantibodies mark the process; they are not themselves proven to be the destroying agent - the effector arm is cellular.
