Heart muscle inflammation
Myocarditis (heart-muscle inflammation)↓
Also searched as Myocarditis (heart-muscle inflammation).
It starts as an infection of the muscle, not merely an inflammation of it. Then the defense turns: because the virus and the heart carry look-alike markers, the immune response kills infected and uninfected muscle cells alike. That second phase, not the infection, is what the literature identifies as the harmful one. If the process does not resolve, scar replaces muscle and the loss is kept. This is a serious condition that is diagnosed and managed by a clinician; nothing here is a substitute for that care.
In depth
Inflammation of the heart muscle that runs in three phases — and the second one does the damage.
What drives it
Resists infection reduces it.
Phase one — the virus itself. The illness begins as an infection of the heart muscle, not as an inflammation of it. 'Viral entry into cardiomyocytes or endothelial cells initiates intracellular replication and direct cytotoxic injury' (Heart Failure Reviews 2026, doi 10.1007/s10741-026-10654-y — Post-viral myocarditis), and the cardiotropic viruses 'can directly infect cardiomyocytes by binding the coxsackievirus and adenovirus receptor, inducing necrosis AND/OR apoptosis through viral replication' (Front Cardiovasc Med 2026, doi 10.3389/fcvm.2026.1821475 — Myocardial inflammation in myocarditis: mechanisms and therapeutic targets). The heart muscle cell is the thing being lost, and it replaces itself poorly. That is why the damage tends to be kept. doi 10.1007/s10741-026-10654-y
Calms inflammation increases it.
Phase two — and this is the cruel part: the defense becomes the damage. 'The initial cellular and humoral immune responses may improve the outcome during phase 1; conversely, they ARE responsible for the harmful effect during phase 2. This is in part induced by molecular mimicry, which is caused by mimicked EPITOPES shared between the viral and cardiac antigens' (PMC2519249 — Acute viral myocarditis). ⇒ because the virus and the heart look alike to the immune system, it kills both: 'virus-infected (antiviral immunity) and uninfected (autoimmunity) cardiomyocytes'. The phase that does the lasting harm is not the infection. It is the response to it. PMC2519249
Eases pain increases it.

Coxsackievirus B3 increases it.
The cause, and the door it comes through. This illness usually begins with a specific family of viruses and a specific protein on the heart cell that lets them in. 'enteroviruses and adenoviruses ARE cardiotropic and can directly infect cardiomyocytes by binding the coxsackievirus and adenovirus receptor (CAR), inducing necrosis AND/OR apoptosis through viral replication' (Front Cardiovasc Med 2026, doi 10.3389/fcvm.2026.1821475). The most frequently implicated agents are named: 'The most frequently implicated is PARVOVIRUS B19, followed by enteroviruses (e.g. coxsackieviruses and echoviruses) and adenoviruses.' and the virus does not always leave: viral genomes are reported 'in up to ~70% of patients with idiopathic dilated cardiomyopathy'. doi 10.3389/fcvm.2026.1821475
Viral entry receptor increases it.

Innate immune sensor increases it.
How the body first notices — and why noticing is not harmless. 'release of PAMPs and DAMPs activates innate immune pathways' (Heart Failure Reviews 2026, doi 10.1007/s10741-026-10654-y), which run through the same master inflammatory switch that drives the inflammatory response. ⇒ the alarm that detects the virus is the same alarm that, left on, damages the muscle. That is why the second phase is the dangerous one. doi 10.1007/s10741-026-10654-y
Inflammation signal (IL-6) increases it.

Fat-tissue signal (TNF-α) increases it.

Cardiac autoantibodies increases it.
The antibodies that mistake the heart for the virus — and predict what happens next. 'cross-reacting antibodies with auto-antigens have been found in patients with myocarditis and ARE indicative of progression to DCM', and cardiac auto-antibodies were shown to 'predict disease development in asymptomatic relatives' (pmc2519249 — Acute viral myocarditis). ⇒ this is the mechanism by which a past infection keeps damaging a heart long after the virus itself is gone.
Immunotherapy heart inflammation increases it.
A cause that did not exist twenty years ago, and it is the deadliest one. Cancer drugs that release the immune system's brakes can let it attack the heart. 'Inflammation of the heart muscle... resulting from ICI targeting cytotoxic T lymphocyte-associated antigen 4 (CTLA4), programmed cell death protein 1 (PD1) and PD1 ligand 1 (PDL1) is an infrequent but potentially fatal complication' (PubMed 38982146 · doi 10.1038/s41568-024-00715-5 — Immune-checkpoint inhibitor-mediated myocarditis: CTLA4, PD1 and LAG3 in the heart, Nat Rev Cancer 2024;24(8):540-553). The numbers ARE stark: 'the fatal rate is as high as 39.7%-50%, and our results indicate the fatal rate is 47.4%' (doi 10.3389/fimmu.2023.1275254 — Immune checkpoint inhibitor-associated myocarditis: a systematic analysis of case reports), and 'combination ICI regimens (anti-CTLA-4 plus anti-PD-1) carry an approximately five-fold higher risk' (doi 10.3389/fphar.2026.1845366 — Immune checkpoint inhibitor-related cardiotoxicity: a cellular pharmacology perspective). And it is growing: 'nearly 50% of all patients with cancer ARE eligible for treatment with ICIs'. doi 10.1038/s41568-024-00715-5
The Other Way In increases it.
The second door. The heart's viral receptor is not alone — cardiotropic viruses also use decay-accelerating factor (daf/CD55) alongside the coxsackie-adenovirus receptor. An agent that blocks one doorway has not necessarily closed the other. doi 10.1038/s41568-024-00715-5 · PMC12698404
Monocyte recruitment increases it.
The signal that calls in the reinforcements — and the scar cell shouts it loudest. Myricetin ameliorates the disease 'by modulating immune response and inhibiting MCP-1 expression' (doi 10.1016/j.ejphar.2023.175551 — Myricetin ameliorates experimental autoimmune myocarditis in mice by modulating immune response and inhibiting mcp-1 expression). And the source matters: 'the expression of TNF-α, IL-6, MCP-1, and IFN-β was significantly increased in cardiac fibroblasts compared to cardiomyocytes or macrophages' (pmc4211177 — Cardiac Fibroblasts Aggravate Viral Myocarditis: Cell Specific Coxsackievirus B3 Replication). ⇒ the cell that will later lay down the scar is also the loudest caller-in of inflammation. doi 10.1016/j.ejphar.2023.175551
Inflammation signal (IL-1β) increases it.
10.1007/s00395-019-0719-0 — and blocking it rescues the pump — which is the test that matters. 'inhibition of caspase-1 OR IL-1β ameliorates cardiac function and reduces the release of myocardial enzymes' (ScienceDirect Topics — Coxsackievirus B3 overview (citing Toldo et al. 2014; Wang, Gao & Xiong 2014; Wang et al. 2018)). ⇒ not a marker that merely tracks the disease. Interrupt it and the heart works better and leaks less. That is proof by blockade on a functional endpoint. doi 10.1007/s00395-019-0719-0
What it drives
It reduces Heart muscle cell.

The heart muscle cell is the thing being lost, and it replaces itself poorly. That is why the damage tends to be kept. doi 10.1007/s10741-026-10654-y
It reduces Immune balance.
It increases Cardiac scarring cells.
10.1016/j.bbrep.2025.102344 — Phase three — where a recoverable illness becomes a permanent one. 'profibrotic cytokines and mediators released during acute myocarditis in susceptible individuals activate fibroblasts and recruit new fibroblast differentiation via endothelial mesenchymal transition leading to cardiac remodeling and dilated cardiomyopathy' (Mayo/Elsevier chapter — Viral Myocarditis and Dilated Cardiomyopathy: Mechanisms of Cardiac Injury, Inflammation, and Fibrosis). Scar is not muscle. It does not contract, and once laid down it stays. This is the step that decides whether the heart recovers its function OR keeps a permanent deficit. doi 10.1016/j.bbrep.2025.102344
It increases Vessel wall adhesion.
How the immune cells get out of the blood and into the muscle. Inflamed heart vessels put out a molecular grip that catches passing immune cells and lets them cross into the tissue. The direction was proved by turning it down: 'ETBR overexpression associated with ICAM-1 downregulation leads to inflammatory attenuation in experimental autoimmune myocarditis' (doi 10.7717/peerj.16320 (pmc10607261) — et-1 receptor type b (etbr) overexpression associated with icam-1 downregulation leads to inflammatory attenuation in experimental autoimmune myocarditis), with IFN-γ, IL-12 and IL-17 falling alongside it. ⇒ fewer grips, fewer invaders, less damage. doi 10.7717/peerj.16320
It increases Th17 immune arm.
The T cell that does the damage in this illness. Of all the immune cell types, this one keeps appearing when a treatment works: myricetin 'significantly decreased the percentage of Th17 cells in spleen and plasma IL-17A level' (doi 10.1016/j.ejphar.2023.175551 — Myricetin ameliorates experimental autoimmune myocarditis in mice by modulating immune response and inhibiting MCP-1 expression), and emodin works on the same axis in the viral model. IL-12 pushes cells down this road; IL-17 is what they release when they arrive. It is the most consistently targeted step in the whole disease. doi 10.1016/j.ejphar.2023.175551
It increases Monocyte recruitment.
It increases Vessel constrictor.
The road from inflammation to a permanently enlarged heart. Quercetin 'offers cardioprotection against progression of experimental autoimmune myocarditis by suppression of oxidative and endoplasmic reticulum stress via ENDOTHELIN-1/MAPK signaling' — tested explicitly on whether it 'protects from the progression of EAM to dilated cardiomyopathy' (PMID 22145946 — Quercetin offers cardioprotection against progression of experimental autoimmune myocarditis by suppression of oxidative and endoplasmic reticulum stress via endothelin-1/MAPK signaling). ⇒ this is the step that decides whether the illness ends OR becomes permanent. PMID 22145946
It increases Immune signaling.

IL-12 pushes cells down this road; IL-17 is what they release when they arrive. It is the most consistently targeted step in the whole disease. doi 10.1016/j.ejphar.2023.175551
It increases Follicle oxidative stress.
The oxidative burden, and it moves with the dose. In the eam model, treatment 'induced a significant decrease of PROOXIDANTS production (O2⁻, H2O2, and TBARS) and improved antioxidant defense system via increase in GSH, SOD, and CAT... with medium and high dose being more effective than low dose' (pmc8901324 — Melissa officinalis l. Supplementation Provides Cardioprotection in a Rat Model of Experimental Autoimmune Myocarditis). ⇒ a redox shift alongside a recovery of pump function.
It increases Calms histamine response.
The allergy cell turns out to matter here — and the proof is deletion, not correlation. 'mast cell deficient mice developed less pronounced myocardial necrosis and cellular infiltration induced by encephalomyocarditis virus, and the histamine H1-receptor antagonist improved survival of mice and improved histological changes' (doi 10.1186/1476-9255-7-39 (pmc2922108) — Matsumori a, Yamamoto k, Shimada m. Cetirizine a histamine H1 receptor antagonist improves viral myocarditis, j Inflamm 2010). ⇒ take the cell away and the disease is milder. That is the strongest form of causal evidence there is. And it led somewhere practical: 'mast cell deficiency had beneficial effects in the viral myocarditis model, and anti-allergic agents prevented viral myocarditis' (doi 10.1007/s00059-012-3692-z — Matsumori a. Lessons learned from experimental myocarditis, Herz 2012) — an ordinary oral antihistamine improved survival in this illness. doi 10.1186/1476-9255-7-39
It increases Immune cell summons.
The summons that calls t cells into the heart — and blocking it saved lives without touching the virus. An engineered decoy version 'had significantly reduced heart/body weight ratio and serum CK-MB level, increased survival rate and improved heart histopathology'. And the mechanism is explicit: 'this therapeutic effect was not attributable to an enhanced viral clearance, but to a blunted Th1 immune response' (pmc3062568 — Direct Gene Transfer with ip-10 Mutant Ameliorates Mouse cvb3-Induced Myocarditis by Blunting Th1 Immune Responses). ⇒ you can save the heart without killing more virus. the immune response, not the infection, is doing the damage at this stage.
It increases The Gate the Immune Cells Come Through.
The gateway that lets T cells in. Blocking it 'alleviated cardiac pathological injury and downregulated the percentage of CD45+CD3+ T cells'; 'the infiltration of activated T cells (CD69+) and macrophages (F4/80+) in the myocardium also decreased' (PMC10221111 — Inhibitor of CD147 Suppresses T Cell Activation and Recruitment in CVB3-Induced Acute Viral Myocarditis). ⇒ A second, independent way to stop the invasion — not by chasing the virus, but by shutting the door the immune cells come through. PMC10221111
