Immunotherapy heart inflammation
Checkpoint-inhibitor myocarditis↓
Also searched as Checkpoint-inhibitor myocarditis.
This is the most lethal form of the illness. Modern immunotherapy works by taking the safety catches off the immune system so it will attack a tumor — and in a small number of people it attacks the heart instead. Reported death rates run from roughly four in ten to one in two, and the risk is about five times higher when two such drugs are combined. It usually appears within the first weeks of treatment. Anyone receiving cancer immunotherapy should discuss any supplement with their oncology team before taking it.
In depth
Heart-muscle inflammation caused by cancer drugs that release the immune system’s brakes.
What drives it
Immune checkpoint reduces it.
A cause that did not exist twenty years ago, and it is the deadliest one. Cancer drugs that release the immune system's brakes can let it attack the heart. 'Inflammation of the heart muscle... resulting from ICI targeting cytotoxic T lymphocyte-associated antigen 4 (CTLA4), programmed cell death protein 1 (PD1) and PD1 ligand 1 (PDL1) is an infrequent but potentially fatal complication' (PubMed 38982146 · doi 10.1038/s41568-024-00715-5 — Immune-checkpoint inhibitor-mediated myocarditis: CTLA4, PD1 and LAG3 in the heart, Nat Rev Cancer 2024;24(8):540-553). The numbers ARE stark: 'the fatal rate is as high as 39.7%-50%, and our results indicate the fatal rate is 47.4%' (doi 10.3389/fimmu.2023.1275254 — Immune checkpoint inhibitor-associated myocarditis: a systematic analysis of case reports), and 'combination ICI regimens (anti-CTLA-4 plus anti-PD-1) carry an approximately five-fold higher risk' (doi 10.3389/fphar.2026.1845366 — Immune checkpoint inhibitor-related cardiotoxicity: a cellular pharmacology perspective). And it is growing: 'nearly 50% of all patients with cancer ARE eligible for treatment with ICIs'. doi 10.1038/s41568-024-00715-5

It increases Heart muscle protein.
The same self-antigen, reached by a completely different road. 'Mechanistic studies further implicate autoreactive T cells targeting cardiac α-MYOSIN heavy chain (α-MyHC), a contractile protein uniquely expressed in cardiac muscle, as central mediators... In preclinical models, immunization with α-MyHC peptides induces autoimmune myocarditis' (pmc12698404 · doi 10.3389/fimmu.2025.1677984 — Overview of immune checkpoint inhibitor associated myocarditis mechanisms diagnostics and treatment). ⇒ this is the exact protein used to create the laboratory autoimmune model. A virus can expose it, a cancer drug can unmask it, and a syringe can inject it — and the heart suffers the same way in all three cases. The downstream is shared too: 'dual blockade of PD-1 and CTLA-4 induces a pro-inflammatory phenotype in cardiac tissues through upregulation of NF-κB, NLRP3 inflammasome components, and MyD88'. doi 10.3389/fimmu.2025.1677984
