Fasting response
The balance here is dose, not direction. 14 of the 16 lanes below are raised and 2 are deliberate reductions; the risk is not in fasting but in overdoing it. Prolonged restriction costs lean mass, lowers thyroid T3 conversion and suppresses the hormonal axes — note that hepatic IGF-1 falls during a fast even as growth hormone rises. A window is a tool; a permanent deficit is a different thing. Hormetic: periods of fasting activate AMPK, SIRT1 and autophagy and drive longevity, while chronic under-nutrition erodes the very tissues it aims to renew. The dose makes the medicine.
In depth
The nutrient-deprivation input — the scarcity signal that flips the cell from growth to repair.
What it drives
It reduces Growth restraint.
Amino-acid/mTORC1 study (PMC3908400) — withdrawal of amino acids reversibly abolishes mTORC1 signaling; nutrient deprivation (fasting) lifts the mTORC1 growth signal, a route independent of AMPK and insulin. PMC3908400. Mechanism named precisely: reduced levels of glucose and amino acids during fasting reduce mTOR activity, which inhibits protein synthesis and stimulates autophagy. It is substrate withdrawal, not an inhibitor — which is why it reverses the moment you eat. The reviewers themselves state the limit: most data come from animal studies OR short-term human trials, raising questions about generalisability to long-term clinical outcomes, and the degree to which autophagy induction produces observable improvement in human metabolic health is still not fully understood. It reduces Growth & survival signaling.
Cohen 2004, Science 305:390 — caloric restriction lowers circulating insulin and IGF-1 (≈8-fold and 1.4-fold in rodents), quieting insulin/IGF-1–PI3K-AKT signaling. PMID 15205477. Verified: reduced calories cause a cumulative decrease in insulin release, and it is insulin signaling through PI3K that drives growth -- so fasting produces a hypoinsulinaemic state with lowered PI3K/AKT tone. the reviewers frame lower cumulative PI3K/AKT exposure as something that may reduce cancer initiation or progression -- a hypothesis carried from cancer biology, not a demonstrated clinical outcome of fasting. PMID 15205477
It increases Longevity gene.
Cohen 2004, Science 305:390 — expression of the SIRT1 deacetylase is induced by caloric restriction; CR could extend lifespan by inducing SIRT1. PMID 15205477. Verified: across modalities CR and intermittent fasting consistently activate AMPK and sirtuins, inhibit mTOR and enhance autophagy, aligning with improved insulin sensitivity, lipid profile, low-grade inflammation and selected epigenetic aging measures in humans. CR-mimetics (metformin, resveratrol, rapamycin, spermidine) only partially reproduce these effects and their long-term safety and efficacy in healthy populations remain incompletely defined. PMID 15205477
It increases Fatty-acid oxidation.
Kersten 1999, J Clin Invest 103:1489 — PPARα mediates the adaptive response to fasting; fasting activates PPARα to switch the liver to fat oxidation and ketogenesis. PMID 10359558. Verified, and the mechanism is de-repression rather than direct activation: in the fed state the insulin-dependent PI3K pathway activates mTORC1, which drives nuclear localisation of NCoR1 -- the PPAR-alpha COREPRESSOR -- inhibiting PPAR-alpha activity and ketogenesis. Fasting lowers mTORC1 and S6 kinase 2, pushing NCoR1 to the cytoplasm and releasing PPAR-alpha (Cells 2020;9:1708). Fasting does not switch PPAR-alpha on; it removes the brake. PMID 10359558
It increases Ketone fuel.
Youm 2015, Nat Med 21:263 — β-hydroxybutyrate levels are elevated by starvation, caloric restriction, and the ketogenic diet as an alternative fuel. doi:10.1038/nm.3804. A threshold, not a gradient — and it is the most actionable number on this row: energy restriction for 10 to 14 hours OR more depletes liver glycogen and triggers hydrolysis of triglycerides to free fatty acids in adipocytes. Those FFAs travel to hepatocytes and are converted to the ketone bodies ACETOACETATE and beta-hydroxybutyrate. Below roughly ten hours the liver is still running on glycogen and this edge carries no flux. The same FFAs also activate PPAR-alpha. doi 10.1038/nm.3804.
It increases Cellular cleanup.
Energy deprivation activates AMPK, which phosphorylates ULK1 (Ser317) and initiates phagophore formation; a 72-hour fast raises the autophagosome marker LC3b-II in skeletal muscle (PMC6831946). Short-term food restriction also induces profound neuronal autophagy with reduced mTOR activity (DOI 10.4161/auto.6.6.12376). Measured by markers, not inferred: fasting raises AMPK phosphorylation and inhibits mTOR, producing increased expression of LC3-II, beclin-1 and ATG proteins — the standard autophagy readouts. The downstream purpose is stated too: autophagy raises free amino-acid availability to support gluconeogenesis and hold glycemia. The reviewers themselves state the limit: most data come from animal studies OR short-term human trials, raising questions about generalisability to long-term clinical outcomes, and the degree to which autophagy induction produces observable improvement in human metabolic health is still not fully understood. doi 10.4161/auto.6.6.12376 · PMC6831946
It increases Supports BDNF signaling.
Caloric restriction raises BDNF in striatum and cortex, and intermittent fasting normalizes BDNF levels where regular feeding leaves them low (reviewed in PMC5742683, which also reports fasting raising the hippocampal p-CREB/CREB ratio). Fasting raises expression of brain-derived neurotrophic factor in neurons (NEJM 2019). The route runs through ketones and carries a condition: beta-hydroxybutyrate promotes BDNF expression in hippocampal neurons under adequate glucose supply (Hu 2018, Neuroscience 386:315) — so this is not a starvation effect but a fuel-switching one, and it depends on the Caloric Restriction / Fasting → β-Hydroxybutyrate link above firing first, which needs 10-14 hours. The reviewers themselves state the limit: most data come from animal studies OR short-term human trials, raising questions about generalisability to long-term clinical outcomes, and the degree to which autophagy induction produces observable improvement in human metabolic health is still not fully understood. PMC5742683
It increases Insulin sensitivity.
Caloric restriction improves insulin sensitivity — a core, repeatedly reviewed metabolic effect of CR (Calorie restriction, SIRT1 and metabolism, Nat Rev Mol Cell Biol 2005). The 2-year human calerie trial confirmed the underlying program in skeletal muscle: sirtuin signaling, mitochondrial biogenesis, enhanced autophagy and reduced inflammation (doi 10.1111/acel.13963). Intermittent fasting consistently improves insulin sensitivity, lipid profiles and body composition by promoting fat oxidation while maintaining lean mass — the lean-mass clause matters, because it separates fasting from simple energy deficit. Leptin sensitivity improves alongside insulin sensitivity. The reviewers themselves state the limit: most data come from animal studies OR short-term human trials, raising questions about generalisability to long-term clinical outcomes, and the degree to which autophagy induction produces observable improvement in human metabolic health is still not fully understood. doi 10.1111/acel.13963
It increases Cellular fuel.
Caloric restriction lowers mitochondrial ATP synthesis and drives accumulation of oxidized NAD⁺; the rise in AMP and NAD⁺ is what activates SIRT1 and AMPK, which then activate PGC-1α (PMC8321869). Together they form the caloric-restriction longevity chain on the map: fasting → NAD⁺ → SIRT1 → PGC-1α. Caloric restriction and exercise both stimulate the NAD+ salvage pathway through AMPK, modifying the age-dependent fall in NAD+ — so this edge runs through the AMPK → NAD⁺ link rather than acting directly. The human numbers ARE on the exercise arm and they ARE strong: skeletal-muscle NAMPT correlated negatively with AGE (r-squared 0.297, P<0.001, n=57) and VO2-peak was the best predictor of NAMPT; 12 weeks of aerobic training raised NAMPT 12 percent in young and 28 percent in older people, and resistance training raised it 25 and 30 percent — the older group gained more. Tissue-specific: in the same work, NAMPT in abdominal subcutaneous adipose was unaffected by either age or 6 weeks of HIIT. Muscle responds; fat does not. PMC8321869
It increases Energy sensing.
Cantó 2010, Cell Metab 11:213 — fasting raises the AMP:ATP ratio and activates AMPK (interdependently with SIRT1) to drive the metabolic adaptation to nutrient scarcity. Mechanism is a ratio, not a signal: during fasting the AMP:ATP ratio rises and AMPK is activated, triggering repair and inhibiting anabolic processes. AMPK is a fuel gauge reading a nucleotide ratio — nothing binds a receptor.
It increases Body clock.
The decisive experiment removes the master clock entirely: in SCN-lesioned mice, in-vivo recording of Bmal1-driven luciferase in hepatocytes showed that after only 6 days of daytime restricted feeding the liver clock had undergone a steady and complete phase shift to match the food intake rhythm — while sham-operated mice had only partially shifted in the same time. The intact SCN was actively fighting the food signal. Speed and specificity: clock-gene rhythms in liver and cerebral cortex entrain within 2 days, before food-anticipatory activity can develop, and the liver is the most rapidly and sensitively entrained tissue. And here is the finding that decides how this edge is drawn: natural food intake patterns have little synchronising effect on peripheral circadian clocks. Mimicking a mouse’s own ad-libitum pattern and shifting it by 12 hours advanced peripheral clocks by only 0-3 hours. Scheduled meals are not a zeitgeber; a long fast is. The authors name the reason: TRF is not a normal eating pattern, and it imposes non-physiologic long fasts. Glucocorticoid dependence ties it to an edge already drawn: the asymmetry in resetting speed does not occur in adrenalectomised OR glucocorticoid-insensitive mice. PMID 33158435 · PMC7182033

It increases Releases stored fat.
10.1186/s40842-022-00137-y — 'The key process in fat catabolism and the provision of energy substrate during times of nutrient deprivation (fasting) or enhanced energy demand (e.g., exercise) is the hydrolytic cleavage of stored TG, the generation of FAs and glycerol, and their release from adipocytes' (J Lipid Res). Fasting is the physiological input state this entire pathway exists to serve. doi 10.1186/s40842-022-00137-y
