You want it low: excess ceramide is the molecular link from saturated-fat overload to insulin resistance and mitochondrial failure.
In depth
The sphingolipid that accumulates under fat oversupply — the toxic lipid species that poisons insulin signaling and mitochondria.
What drives it
The Two-Way Valve reduces it.
Sphingomyelin synthase (SMS1/SMS2) consumes ceramide - it transfers a phosphocholine head group from phosphatidylcholine onto ceramide, producing sphingomyelin and diacylglycerol. Verbatim from the literature: 'phosphatidylcholine is required for converting pro-apoptotic ceramides to sphingomyelin' and 'PC is substrate of sphingomyelin synthase (EC 2.7.8.27) for the formation of SPH from (pro-)apoptotic ceramides'. This is the exact reverse of the SMASE arm: sphingomyelinase makes ceramide from sphingomyelin; sphingomyelin synthase makes sphingomyelin from ceramide. They are a reversible pair around one node, and dietary choline supplies the head group. Scope, stated: this is sequestration, not destruction - the ceramide is converted, not cleared, and sphingomyelin is itself the substrate the hydrolysis arm draws on. And the equilibrium is species-specific: total SMS knockout in mouse liver 'had a different effect on ceramide subspecies and some ceramide levels are increased and some are decreased' (PMC8022321). No human interventional evidence that feeding choline lowers a plasma ceramide has been retrieved; the CARDIA cohort (PMID 41824420) associates betaine with lower MASLD risk but does not link it to a ceramide. PMID 41824420 · PMC8022321
Innate immune sensor increases it.
PMID 21490391 — Holland/Summers 2011, j Clin Invest (jci.org/articles/view/57144) - saturated fatty acids and LPS, acting through TLR4, switch on the whole ceramide production line at once. Verbatim: LPS and saturated fatty acids, via TLR4, increase levels of transcripts encoding virtually all of the enzymes involved in de novo ceramide synthesis, including spt subunits, CerS isoforms, and Des1. This is the master upstream control: one receptor governs transcription of the entire enzyme set. Shown in rodent and cell transcriptional work. PMID 21490391
Calms inflammation increases it.
Holland/Summers 2011, J Clin Invest - the TLR4 signal reaches the ceramide enzymes through NF-kappa-B. Verbatim: This transcriptional regulation is governed by NF-kappaB, as the loss of IkappaKbeta, an upstream kinase involved in NF-kappaB activation, blocks the response. Accordingly, treatment of diet-induced-obese mice with the IkappaK inhibitor sodium salicylate selectively reduced tissue ceramides and restored insulin sensitivity. Shown in mice. This edge is why anti-inflammatory ingredients can lower ceramide at all - they act on the switch, not on the lipid. doi 10.1172/JCI57144
Ceramide first step increases it.
Serine palmitoyltransferase condenses serine with palmitoyl-CoA - the first and rate-limiting step of de novo ceramide synthesis (Wang 2017, Circulation 135:2028, PMID 28280233; unsaturated fatty acids oppose the saturated-fat drive on this step). Warning carried deliberately: SPT is not a viable therapeutic target. Inhibiting it in rodents lowers ceramide but does so by extreme diminution of all sphingolipids, including the structural ones the body requires. The selective target in this pathway is ceramide synthase, not SPT. PMID 28280233
Ceramide synthase increases it.
10.1186/1743-7075-9-71 — Ceramide synthases are a family of six isoenzymes, CerS1 to CerS6, each preferring a particular fatty-acyl chain length, which is why ceramide species differ in their consequences. CerS acts in both the de novo route (N-acylating sphinganine) and the salvage route (re-acylating sphingosine). Ceramide synthases are described in the literature as attractive drug targets for metabolic disease, in contrast to SPT. Danhe granule was reported to reduce hepatic ceramide de novo synthesis related to CerS6 and CerK in rats. doi 10.1186/1743-7075-9-71
Sphingomyelinase increases it.
10.3390/medicina57070729 — Sphingomyelinase hydrolyzes sphingomyelin to ceramide - the second route in, independent of de novo synthesis. The isoforms are compartment-specific and must not be conflated: SMPD1 is acid and lysosomal, SMPD2 is neutral, and a secretory form acts at the outer leaflet of the plasma membrane. Dietary inulin lowered circulating ceramides in mice by suppressing neutral sphingomyelinase specifically - Smpd3 expression fell about two-fold and hepatic neutral SMase activity about three-fold. Shown in mice. No human equivalent has been retrieved. doi 10.3390/medicina57070729
Ceramide-clearing enzyme reduces it.
Ceramidase hydrolyzes ceramide to sphingosine - the clearance step, and the only arrow on this board pointing away from ceramide. Important and often misstated: this is not a simple exit. The sphingosine released is largely re-acylated by ceramide synthase straight back into ceramide - the salvage or recycling route, which accounts for a majority of sphingolipid biosynthesis. Clearance therefore depends on what happens to sphingosine next, not on ceramidase activity alone. Walnut consumption (PMID 31087608, randomized double-blind controlled inpatient crossover) reduced ceramides while raising sphingosines, which the authors read as enhanced ceramidase activity. PMID 31087608
Stops fat storage increases it.
Ceramide-metabolism review 2016, Am J Physiol Endocrinol Metab — palmitate, the product of de novo lipogenesis (fatty-acid synthase), is the substrate for de novo ceramide synthesis, so lipid oversupply drives ceramide accumulation. doi 10.1152/ajpendo.00133.2016
Activated fatty acids increases it.
'Acyl-CoAs can further enter the ceramide-biosynthesis pathway. Ceramides limit lipolysis by activating the PP2A-dependent dephosphorylation of HSL' (doi 10.3389/fphys.2022.826314). A third route from the same product to the same off-switch, and a mechanistic bridge to the Ceramide Blocker blend. doi 10.3389/fphys.2022.826314
What it drives
It increases Fat-overload cell damage.
PMC19389 — Ceramide is the messenger that carries the instruction to die when a cell is overloaded with saturated fat. Verbatim from the adiponectin-receptor work: 'Ceramidase activity is impaired in cells lacking both adiponectin receptor isoforms, leading to elevated ceramide levels and enhanced susceptibility to palmitate-induced cell death.' The reverse holds too - sulforaphane 'alleviated PA-mediated apoptosis, downregulated the expressions of cleaved caspase 3, as well as reduced ceramide contents' in the same experiment, and Ganoderma lucidum extract improved viability and decreased tunel staining in palmitate-stressed primary hepatocytes. Scope, stated: this is cell and rodent work. And the naming is deliberate - this pathway is fat-driven cell death specifically, not apoptosis in general, which the acb blend drives up as an anti-cancer aim. The two must never be conflated.
It increases Arterial plaque.
Ceramides have been detected in atherosclerotic plaques, where they facilitate the transport and entry of oxidized LDL-cholesterol into intimal cells - the initial step in atheroma formation (Adv Nutr 2024). Plasma ceramides predict cardiovascular death beyond LDL-cholesterol across three independent prospective cohorts (Laaksonen 2016, Eur Heart J 37:1967, PMC4929378), and a plasma ceramide score predicted incident myocardial infarction, stroke or cardiovascular death in the randomized PREDIMED trial (hazard ratio 2.18, 95 percent CI 1.36 to 3.49; PMID 28280233). Scope stated plainly: in humans this is association and prediction. The evidence that lowering ceramide prevents events is rodent. PMID 28280233 · PMC4929378
It reduces Insulin sensitivity.
Holland 2007, Cell Metab 5:167 — the sphingolipid ceramide is the common intermediate linking saturated fat and glucocorticoids to insulin resistance; inhibiting ceramide synthesis restores glucose tolerance. PMID 17339025. Two independent and fully separable mechanisms, named to the residue: ceramides block AKT by (1) PP2A dephosphorylating its activating residues, and (2) PKC-zeta phosphorylating an inactivating T34 residue inside the AKT PH domain, which blocks AKT translocation rather than its phosphorylation. Downstream, GLUT4 translocation fails in muscle and adipose and hepatic gluconeogenesis is inadequately suppressed. Human, with clamp: in visceral and subcutaneous adipose from 439 subjects, CerS6 expression correlated positively with BMI, body fat and hyperglycemia and inversely with insulin sensitivity measured by EUGLYCAEMIC-HYPERINSULINAEMIC clamp — the reference method, not a surrogate index. Three limits that the sources state themselves. (1) much of the mechanism rests on artefact-prone tools: the early cultured-cell work used cell-permeable short-chain ceramides with cytotoxic properties, often at unphysiologic concentrations. (2) the PP2A arm fails its in-vivo test: pharmacological inhibition of PP2A in rats paradoxically worsened hepatic insulin resistance despite activating AKT — the opposite of what the model predicts. (3) which arm dominates depends on the cell: the PKC-zeta route predominates in L6 myotubes and in primary human muscle cells, whereas PP2A predominates in C2C12. And the causal link is explicitly unsettled: an enzyme generating ceramide close enough to the insulin receptor to suppress it directly has not yet been identified. Chain length decides the sign: long-chain C16:0/C18:0 ceramides track myocardial infarction, heart failure and cardiovascular mortality, while very-long-chain C22:0/C24:0 are neutral OR potentially protective. 'Ceramide' is not one thing. PMID 17339025
It reduces Mitochondrial energy.
Gudz 1997, J Biol Chem — cell-permeable ceramide directly inhibits mitochondrial respiratory-chain complex III, collapsing oxidative phosphorylation and generating ROS. PMID 9305864. Mechanism is electron diversion, not enzyme inhibition: ceramides impair mitochondrial function by diverting electrons in the electron transport chain into reactive oxygen species instead of using them for oxidative phosphorylation, diminishing the organelle's ability to generate ATP — the same electrons become damage instead of energy. Depletion rescues it: blocking ceramide synthesis with myriocin in high-fat-fed mice improved mitochondrial performance — increased oxygen consumption, increased citrate synthase activity, and preserved PGC-1 expression. The rescue was not clean: effects on lipid oxidation were unclear — long-chain acylcarnitines rose (suggesting incomplete oxidation) while short-chain fell. Ceramides also alter membrane permeability directly rather than acting purely as signals. PMID 9305864
It increases Master phosphatase.
A cross-blend link that runs the helpful way. 'ceramides limit lipolysis by activating the PP2A-dependent dephosphorylation of HSL' (doi 10.3389/fphys.2022.826314). Acyl-CoAs made from released fat 'can further enter the ceramide-biosynthesis pathway'. ⇒ lowering ceramide releases this brake, which is a genuine mechanistic bridge to the Ceramide Blocker blend. Not a clinical claim; a mechanism. doi 10.3389/fphys.2022.826314
It increases Sphingosine-1-phosphate.
The sphingolipid ladder continues one rung further: the same backbone that becomes the harmful lipid can instead be phosphorylated into this messenger. The two ARE opposites in effect and neighbors in chemistry, which is why the rung matters. PMC11975168
It increases The Phorbol-Proof Kinase.
A dose that reverses itself, and it matters because another product in this range targets this molecule. 'low concentrations of ceramide were observed to transiently stimulate PKC zeta activity in vitro and in vivo, whereas high doses of ceramide lead to inhibition of PKC zeta' (PMID 10348660 — Atypical PKC zeta is activated by ceramide). ⇒ A little opens this kinase; a lot closes it. And the fat-release route does not run through ceramide at all: 'neither was ceramide increased by S1P, nor was S1P-mediated lipolysis affected by the ceramide synthase inhibitor FUMONISIN B1' (PMC11975168 — Intracellular S1P Induces Lipolysis Through Direct Activation of PKC Zeta). The two are neighbors in chemistry and separate in action. PMID 10348660 · PMC11975168
