In depth

A neurovascular event, not a headache with extra force. Cortical spreading depression and trigeminovascular activation come first; the pain is downstream — which is why sleep and hormonal nodes reach it here.

What drives it

Headache peptide increases it.

CGRP is a 37-amino-acid peptide released from trigeminal neurons and a potent vasodilator, and its role in migraine is proven by the drug class built on it: four CGRP-targeted monoclonal antibodies are approved for migraine prevention — erenumab against the receptor, galcanezumab, fremanezumab and eptinezumab against the ligand — and all four have established efficacy (PMC9300070, PMC10811399, review PMC10586710). In the phase-2 randomized trial of one of them, monthly migraine days fell 4.2 versus 3.0 on placebo (p=0.003) with a 50% responder rate of 70% versus 45%. PMC9300070

Quiets overexcitation reduces it.

Cortical spreading depression is the mechanism and it is physically described: a slow spreading wave of depolarization in glial and neuronal cells, a slow direct current potential shift due to high extracellular potassium ions with high intracellular sodium and calcium ions. At the cellular level CSD is characterized by ionic changes and excitotoxicity. One of its downstream events is trigeminovascular system activation, which facilitates the release of nitric oxide to cause vasodilation - the aura and the headache are the same event at two depths. Blood flow (nitric oxide) increases it.

Of 44 migraineurs given intravenous glyceryl trinitrate, 33 had a migraine attack fulfilling International Headache Society criteria; a repeat attack was triggered in all subjects but one — remarkably reproducible. doi 10.1177/0333102415574846 · PMID 15288408 · PMC4181855

Brain inflammation increases it.

C. acnes promotes inflammation via TLR2 and TLR4; TLR signals induce NF-kB-dependent expression of NLRP3, pro-IL-18 and pro-IL-1beta. Androgens raise sebum secretion, triggering acne. doi 10.21037/atm-22-1362 · PMC2904443

Inflammatory alarm increases it.

Calms histamine response increases it.

GTN infusion in vivo induced significant dural mast cell degranulation initiating neurogenic inflammation; pre-treatment with the effective anti-migraine substances L-NAME and sumatriptan prevented it. doi 10.1177/0333102415574846 · PMC4181855

Oxidative stress increases it.

A sentence worth the whole pass: one common factor for almost all migraine triggers is reactive oxygen species. And it carries an intervention: the antioxidant TEMPOL strongly reduces the CSD occurrence in vivo. The route is specified - TRPA1 is sensitive to oxidative stress and ROS/TRPA1/CGRP signaling mediates CSD. Species: tempol in vivo is rodent. PMID 36361791 · PMC10933035

Mood (serotonin) reduces it.

Serotonin depletion is implicated in many neurological disorders and has been shown to exacerbate CSD-evoked pain at the cortical level. This is the mechanistic anchor under the triptan class, which are 5-HT agonists - and the CGRP evidence has the same shape: elevated CGRP levels in the jugular vein during a migraine attack and its normalization by the anti-migraine drug SUMATRIPTAN. PMID 36361791 · PMC10933035

Stress & cortisol increases it.

The lowered-CSD threshold level is considered as stress-induced susceptibility to migraine. Susceptibility, not causation - stress lowers the threshold for a wave that then does the damage. PMID 36361791 · PMC10933035

What it drives

It reduces Mood.

10.1186/s10194-026-02280-8 — Migraine is the third most prevalent and seventh most disabling disease in the world, and disability at that scale carries a documented mood cost. This link is drawn from the reviews of this outcome rather than from a dedicated trial, and no effect size specific to this link has been reported. This link rests on the reviews of this outcome as a whole, and no effect size specific to it has been reported. doi 10.1186/s10194-026-02280-8

It reduces Sleep.

10.1186/s10194-024-01756-9 — Attacks disrupt sleep, and sleep disruption is itself a trigger - the reverse arm is already drawn on its own evidence. This direction is taken from the reviews of this outcome rather than from a dedicated trial, and no effect size specific to this link has been reported. doi 10.1186/s10194-024-01756-9
