Calms inflamed fat tissue

Fat-tissue inflammation

The Fat That Fights Back

You want it quiet: this is a different node from neuroinflammation, kept separate because the tissue, the cells and the evidence are different — and because the direction is still disputed.

In depth

Adipose tissue inflammation — immune cells accumulating in fat, forming crown-like structures around dying adipocytes and releasing cytokines locally.

What drives it

Visceral fat increases it.

Macrophage content of perigonadal, perirenal and mesenteric (visceral) fat rose in proportion to adipocyte size and body mass, and these recruited macrophages were the dominant source of adipose TNF-alpha, iNOS and IL-6, establishing that expanding visceral fat drives adipose tissue inflammation. doi 10.1172/JCI19246 · PMID 14679176

Supports weight management increases it.

The uncontested half of a relationship whose other half is disputed. That adiposity produces adipose tissue inflammation is not in question anywhere in this literature: early during high-fat feeding, adipocytes undergo hypertrophy, releasing chemokines that induce recruitment of M1-polarized macrophages. With increased adiposity, recruited CCR2-positive macrophages form crown-like structures and overwhelm the protective effects of M2 macrophages. The chemokines ARE named: CXCL12 produced by adipocytes interacts with CXCR4 to mediate macrophage recruitment, with haptoglobin and C3a also reported. And the trigger is adipocyte death — the crown-like structures form around necrotic adipocytes, which is why the histology looks the way it does. Drawing this direction explicitly matters: it shows what obesity does to fat tissue without depending on the disputed direction between fat-tissue inflammation and insulin. One arrow is settled and drawn; the other is contested and labeled. doi 10.3389/fimmu.2023.1153915 · PMC1716210

Obesogen exposure increases it.

EDCs promote a pro-inflammatory, metabolically dysfunctional adipose phenotype, with adipose tissue inflammation as a route by which obesogen exposure translates into metabolic disease. 10.3389/fendo.2026.1782328. doi 10.3389/fendo.2026.1782328

Sodium load increases it.

Exposing adipocytes to elevated sodium chloride increased adipogenesis and lipid accumulation and raised secretion of pro-inflammatory adipocytokines including TNF-alpha, IL-6 and leptin, showing that a high salt load pushes adipose tissue toward an inflammatory state. doi 10.3390/ijms20010160 · PMID 30621146

What it drives

It reduces Insulin sensitivity.

The two are now separate, and that separation is the point of the node. The anatomy is specific and visible: in lean tissue macrophages are evenly distributed and display limited inflammatory properties; in obese tissue they gather around dying adipocytes and form crown-like structures. Macrophage abundance rises to about 41 percent of the cell population. This is a histological finding you can point at, not an inference from a cytokine level. And the intervention arm is good: a dual CCR2/CCR5 antagonist significantly reduces M1 macrophage infiltration into adipose tissue in high-fat-diet mice, as well as improving adipose tissue inflammation and insulin resistance. Converging: IL-10 overexpressed in lean-mouse macrophages protected adipocytes from TNF-alpha-induced insulin resistance, and mice with impaired M2 polarization show higher inflammation, weight gain and insulin insensitivity. The direction is disputed and the conditional carries both sides. One literature says infiltration of pro-inflammatory macrophages precedes the insulin resistance. Another says insulin resistance may drive adipose tissue inflammation by promoting the recruitment of macrophages — and in one high-fat time course insulin resistance was evident by week 4 while M1 accumulation was only mildly increased by week 10. The proposed cause arriving six weeks after the effect is not a detail. And the human data do not show the central mechanism at all. The M1-to-M2 polarization switch organises this entire field. In human adipose tissue: despite increased macrophage abundance, there was NO evidence of a phenotypic shift with increasing BMI; the M1:M2 ratio was not different between BMI below 30 and above 30, and there was NO correlation between BMI and the ratio. More macrophages, yes. The switch that is supposed to make them harmful, not demonstrated. The authors offer that single-surface-marker characterization may have been insufficient — which is a limitation, not a rescue. The arrow points at insulin sensitivity, which is a polarity-inverted node: drawn inhibitory means inflammation reduces sensitivity. doi 10.3389/fimmu.2023.1153915 · PMC8321823

It increases Adrenaline responsiveness.

Inflammation produces catecholamine resistance in obesity through activation of PDE3B by the kinases IKK-epsilon and TBK1. PMID 24368730
