Fasting insulin
Reads: Fasting insulin
You want it low: low fasting insulin marks an insulin-sensitive, metabolically healthy state.
What this marker reads
Fasting insulin — a sensitive marker of insulin resistance; chronically high levels signal metabolic strain.
No body publishes a range for this marker
No professional body publishes a fasting-insulin reference interval, and the reason is itself published: insulin immunoassays are not standardised. Measured against isotope-dilution mass spectrometry, assay deviation ran from −298.2 to +302.6 pmol/L. A single number here would be true only of one analyser.
Your laboratory reads your result against its own assay, and this page does not do that for it. What is above is what a named body published and for whom; what is below is the machinery the marker reads and the formulas witnessed on it.
What moves it
Fat-burning hormone lowers it.
Adiponectin/insulin PNAS study — globular adiponectin improves insulin sensitivity (suppressing hepatic glucose output, raising muscle glucose uptake), lowering fasting insulin and glucose. doi 10.1073/pnas.1922169117
Insulin sensitivity lowers it.
As insulin sensitivity falls, the pancreas compensates by secreting more — which is why fasting insulin rises long before fasting glucose does, and why it is the earlier warning (PMC11906131, PMC9255858). Note, and it matters: the arrow is not settled in one direction. Hyperinsulinemia is conventionally read as the compensation for resistance, but evidence shows chronically elevated insulin can itself drive resistance — fasting insulin correlates negatively with insulin-receptor mRNA in human skeletal muscle across >450 samples, and prolonged insulin exposure attenuates AKT and ERK signaling in myotubes. Treat this as a reinforcing loop, not a one-way street. PMC11906131
Fat burning lowers it.
insulin, fasting insulin, type 2 diabetes and blood lipids share one literature: heart, kidney, liver, and metabolic diseases share interlinked pathological mechanisms, including inflammation, oxidative stress, insulin resistance, and endothelial health issues. These four overlap by nature: fasting insulin is the level, insulin is the signaling, type 2 diabetes is the diagnosis and blood lipids are the substrate. The same driver legitimately reaches all four, and a reader should not mistake four arrows for four independent findings. doi 10.1038/s41581-026-01066-0 · PMC11725408

What it moves
It lowers Women's hormone balance.
Measured prospectively in the Study of Women's Health Across the Nation: perimenopausal fasting insulin and BMI showed complementary but distinct associations with the menopausal transition, and elevated insulin predicted earlier and more prolonged vasomotor symptoms while also tracking with higher testosterone (PMID 40963750). Fasting insulin remained an independent predictor of hot flashes in models accounting for fasting glucose — so this is the insulin itself, not merely the glycemia behind it. PMID 40963750
It lowers Hormone-binding protein.
Clinically the inverse relationship is consistent — low SHBG is a recognized marker of hyperinsulinemia, and interventions that lower basal insulin raise SHBG alongside improved insulin sensitivity and androgen profiles (PMC12829335, PMC7663738). But the honest nuance, stated because it changes what may be claimed: in HepG2 hepatocytes cultured with high glucose, insulin alone did not alter SHBG production OR hnf-4α levels (Selva et al., PMID 17992261). The insulin association appears to run through hepatic lipogenesis rather than through insulin acting directly on the SHBG promoter — so lowering insulin plausibly raises SHBG by removing the lipogenic drive, not by a direct effect. PMID 17992261 · PMC12829335
It raises Stops fat storage.
Insulin is the signal that tells the liver to make fat: hepatic insulin resistance together with dietary monosaccharides raises delivery to the liver and stimulates SREBP-1c, which drives hepatic DE novo lipogenesis (PMC7663738). This is the step that then lowers HNF-4α and SHBG downstream — the same chain, one link earlier. PMC7663738
It lowers Kidney.
Chronic hyperinsulinemia activates pro-growth and pro-fibrotic pathways, resulting in vascular dysfunction, inflammation and fibrosis. This is the insulin level doing damage, separately from the resistance - which is why fasting insulin exists as a node distinct from insulin, and this edge is the clearest case for that split anywhere on the map. doi 10.1038/s41581-026-01066-0 · PMC11725408
