Tissue scarring

You want it stopped early. Fat is reversible; scar is much less so.
In depth
When injury keeps repeating, stellate cells lay down collagen where working tissue used to be. Scar does not do any of the jobs a liver does, and past a point it does not come back.
What drives it
Alcohol-related fatty liver increases it.
Verbatim (PMC7530314): 'activated hepatic stellate cells proliferate and take on a pro-fibrotic role... activated hepatic stellate cells lay down a fibrotic extracellular matrix composed of collagens type I and III, glycoproteins, and proteoglycans.' this is the step that makes early reversal matter. Fat is reversible; the matrix is much less so. PMC7530314
Pancreatic stellate cells increases it.
The same cell type, named in both organs. The pancreatic stellate cell is the pancreatic counterpart; the hepatic stellate cell is the liver one and lays down the same collagens I and III (PMC7530314). This edge asserts a shared cell biology, not that a pancreatic driver treats liver scar. No driver crosses this edge without its own hepatic evidence. PMC7530314
Energy sensing reduces it.
Verbatim (PMID 31214376): 'Ethanol reduces AMPK levels, which increases ACC1 activity, decreases PPARa levels, and increases mTORC1 activity. Increased mTORC1 further increases SREBP-1c activity and decreases autophagy.' The link runs inversely: AMPK activity opposes the cascade that ends in scar. This is an upstream mechanistic link, not a demonstration that activating AMPK reverses established fibrosis in a person. PMID 31214376
Acetaldehyde clearance reduces it.
A knockout counterfactual, not an association. Verbatim (PMC6050510): 'ALDH2 deficiency accentuated CCl4-induced liver fibrosis in mice, accompanied by increased expression of collagen 1alpha1, alpha-SMA and TIMP-1. Moreover, ALDH2 knockout triggered more ROS generation, hepatocyte apoptosis and impaired mitophagy after CCl4 treatment. In cultured HSC-T6 cells, ALDH2 knockdown by transfecting with lentivirus vector increased ROS generation and alpha-SMA expression in an in vitro hepatocyte fibrosis model using TGF-beta1.' Two independent loss-of-function tests - whole-animal knockout and cell-line knockdown - both worsening fibrosis. the model is carbon tetrachloride, not alcohol; the pathway is validated, the alcohol route to it is inferred from the shared acetaldehyde chemistry and is not claimed here. PMC6050510
Liver inflammation increases it.
Verbatim (pmc7530314, comparing NAFLD and ald): 'activated hepatic stellate cells lay down a fibrotic extracellular matrix composed of collagens type I and III'. And the German-language source gives the reversibility boundary: 'Eine Leberzirrhose (Fibrose-Stadium F4) gilt ... als NICHT MEHR VOLLSTÄNDIG REVERSIBEL. ALLE STADIEN DAVOR ... KÖNNEN SICH ... ZURÜCKBILDEN.' scope: pivens showed vitamin e did not cross this edge — 'improvement in fibrosis was not observed.' Nothing on this blend has been shown to reverse established fibrosis in a person.
Liver scarring cells increases it.
Verbatim (PMC7530314): 'activated hepatic stellate cells lay down a fibrotic extracellular matrix composed of collagens type I and III'. And the PPC mechanism runs exactly here: Cao Q, Mak KM, Lieber CS, 'DILINOLEOYLPHOSPHATIDYLCHOLINE prevents transforming growth factor-BETA1-mediated collagen accumulation in cultured rat hepatic stellate cells' (doi 10.1067/mlc.2002.121853). That PPC paper is a reference-list string, recorded not cited; the Hepatic Stellate Cells → Liver Fibrosis link itself rests on. doi 10.1067/mlc.2002.121853 · PMC7530314
Reduces scarring increases it.
10.1186/s12931-025-03110-2 — 10.3390/cells8111419 — The direct link from TGF-β to hepatic fibrosis. TGF-β is a cytokine that signals fibrosis throughout the body — in hair, bone, heart, skin, lung and sexual tissue — and the fibrosis shown here is its hepatic arm. Liver stiffness is the clinical measurement of this output, not a pathway of its own. Limitation: lowering liver stiffness in people has not yet been shown for the liver blends. doi 10.1186/s12931-025-03110-2
Fluke burden increases it.
Periportal pipestem fibrosis is the defining hepatic lesion of chronic schistosomiasis. PMID 19825105
What it drives
It increases Liver scarring.
10.3389/fphar.2014.00167 — Nbk482419 — the reversibility boundary, and it is the most important line on this MAP. German-language source (fettleber.org): 'Eine leberzirrhose (fibrose-stadium F4) gilt ... als NICHT MEHR VOLLSTÄNDIG REVERSIBEL. ALLE STADIEN DAVOR (F0-F3) KÖNNEN SICH ... ZURÜCKBILDEN.' Arabic-language source gives the rate: 20-30% of heavy drinkers reach cirrhosis. Korean kasl gives the pace: heavy drinking carries roughly 2% per year to cirrhosis. The evidence covers nothing on this blend has been shown to reverse established cirrhosis in a person, and pivens showed vitamin e did not even improve fibrosis ('improvement in fibrosis was not observed'). NO copy may imply otherwise. doi 10.3389/fphar.2014.00167
It increases Liver-tissue risk.
The number that answers 'why does stiffness matter', and it is measured in fatty liver disease specifically. PMC8472221, a liver-stiffness-based HCC risk model in NAFLD (defined as CAP >=238 dB/m), median follow-up 64.6 months: 'In multivariate analysis, AGE >=60 years (HR = 9.1), platelet count <150 (HR = 3.7), and liver stiffness >=9.3 kPa (HR = 13.8) were independent predictors.' AUC for predicting HCC at 2, 3 and 5 years: 0.948, 0.947, 0.939. A dose ladder in a second population (PMID 38061410, HCV cirrhosis post-SVR): 'The adjusted annual risk of HCC was 2.03% with LSM <10 kPa, 2.48% at 10-14.9 (aHR 1.71), 3.22% at 15-19.9, 5.07% at 20-24.9 (aHR 2.55), and 5.44% at >=25 kPa (aHR 3.03; P <.0001).' And a floor: 'patients with LSM <5 kPa in the absence of diabetes had a low risk of HCC in which surveillance could be discontinued.' Scope, and it is the whole point for this blend: stiffness is a measurement of fibrosis, not a separate biology - which is why NO 'stiffness' node was built. And the Russian-language source found A 4,520-person NHANES elastography analysis in which coffee was associated with lower liver stiffness but not steatosis - the two readings come off the same machine and can move independently. Nothing on this blend has been shown to lower liver stiffness in a person. PMID 38061410 · PMC8472221
It increases Portal pressure.
The pressure begins before cirrhosis does. Baveno VII introduced 'compensated advanced chronic liver disease (cACLD)' precisely because 'cirrhosis implies a pathological (invasive) diagnosis' — the concept 'was put forward based on non-invasive tests that would predict the development of complications of cirrhosis', and Baveno VII 'leveraged the use of liver stiffness measurement to categorize the degree of portal hypertension' (PMC11299547). this edge asserts that advanced fibrosis raises portal pressure before the cirrhosis threshold; the earlier MASLD source notes 'the risk of liver-related complications due to CSPH can already manifest in an earlier stage of MASLD prior to cirrhosis'.
