You want it reversed. It is the first and the most reversible stage of alcohol-related liver disease.
In depth
Fat in the liver caused by drinking rather than by diet. The end picture looks like ordinary fatty liver, but it arrives by a different road: the alcohol is burned first, so fat is not, and the burning itself switches on fat-building.
What drives it
Acetaldehyde load increases it.
Verbatim (PMID 21920463): 'Alcohol consumption can directly (via acetaldehyde) or indirectly ... Up-regulate the expression of SREBP-1c and down-regulate the expression of PPAR-alpha, leading to the induction of fatty acid synthesis and inhibition of fatty liver beta-oxidation, which results in the development of alcoholic fatty liver.' Independently corroborated at the molecular level (pmc5775393): acetaldehyde at 100 uM activated a Cidea reporter carrying an sre element in AML12 cells, and 'the knockdown of SREBP1c suppressed acetaldehyde-induced Cidea expression' — a counterfactual. PMID 21920463
Alcohol metabolism increases it.
The direct route from alcohol to alcohol-related fatty liver. Verbatim (pmc12086950): 'alcohol metabolism leads to glutathione depletion, mitochondrial damage, lipid peroxidation and innate immune responses', and the Spanish-language source (Mexican Ministry of Health): 'La esteatosis suele ser benigna, asintomática y reversible CON LA abstinencia.' Seven national bodies across ten languages state that stopping drinking is the first-line treatment, and no supplement replaces it.
What it drives
It increases Clears liver fat.
The two roads converge. Verbatim (pmc7530314, a paper comparing both diseases): 'This upregulation of SREBP-1 occurs directly from the actions of the ethanol metabolite, acetaldehyde ... Ethanol inhibits DNA-binding and transcriptional activity of PPAR-alpha, with resulting downregulation of fatty acid oxidation.' scope: alcoholic and non-alcoholic fatty liver share the SREBP-1c-up / PPAR-alpha-down endpoint, which is why one blend can address both — but the cause differs and the alcoholic form has a cause a supplement cannot remove.
It increases Tissue scarring.
Verbatim (PMC7530314): 'activated hepatic stellate cells proliferate and take on a pro-fibrotic role... activated hepatic stellate cells lay down a fibrotic extracellular matrix composed of collagens type I and III, glycoproteins, and proteoglycans.' this is the step that makes early reversal matter. Fat is reversible; the matrix is much less so. PMC7530314
It increases Liver inflammation.
The two arms converge on one histology. And the Korean kasl source: '酒精性い原因い場合をアルコール性肝炎といい、アルコールと関連がないにもかかわらず同一の病理組織所見を示す場合を非アルコール性脂肪肝炎と呼ぶ' — the alcoholic and non-alcoholic forms show the same histological picture and are distinguished only by cause. Same histology, different cause — and one of those causes is not on the label.
