The Poison In The Middle

You want it cleared fast. It is a waypoint, not a destination, and the longer it lingers the more it binds.

In depth

Alcohol itself is not what does most of the damage. It is converted to acetaldehyde, and acetaldehyde is the reactive one - it sticks to proteins and DNA, and it drives the fat-building switch directly.

What drives it

Alcohol metabolism increases it.

Verbatim (PMC12086950, Pathogenesis of Alcohol Associated Liver Disease): 'Alcohol is absorbed by the gut and primarily metabolized by hepatocytes. Alcohol is metabolized to acetaldehyde via alcohol dehydrogenase and CYP2E1, which forms protein and DNA adducts.' two enzymes, one product. ADH is the constitutive route; CYP2E1 is the inducible one — 'inducible by chronic alcohol use' (PMC6316574). Nothing on this blend speeds alcohol clearance, and NO concentrate substitutes for not drinking. PMC12086950

Acetaldehyde clearance reduces it.

Verbatim (pmc8742892): 'In order to reduce toxicity, acetaldehyde is rapidly further oxidized to acetate by enzyme ALDH2 inside mitochondria.' and the same reaction carries a cost: 'This reaction generates acetate and even more NADH ... As a result, the cellular NAD+/NADH ratio decreases, having a strong impact on the inhibition of fatty acid beta-oxidation, causing triacylglycerols to accumulate inside hepatocytes.' Clearing the poison still tilts the redox state toward storing fat.

Ethanol-metabolizing enzyme increases it.

Verbatim (pmc6316574): ethanol oxidation proceeds via 'the microsomal ethanol-oxidizing system (meos), which depends on cytochrome P450 2E1 (CYP 2E1), and is inducible by chronic alcohol use. meos induction accelerates the metabolism of ethanol to acetaldehyde that facilitates organ injury including the liver, and it produces via CYP 2E1 many reactive oxygen species.' The link runs directly: CYP2E1 makes acetaldehyde — inducing it is not protective, it is how chronic drinking makes the injury worse.

What it drives

It increases Alcohol-related fatty liver.

Verbatim (PMID 21920463): 'Alcohol consumption can directly (via acetaldehyde) or indirectly ... Up-regulate the expression of SREBP-1c and down-regulate the expression of PPAR-alpha, leading to the induction of fatty acid synthesis and inhibition of fatty liver beta-oxidation, which results in the development of alcoholic fatty liver.' Independently corroborated at the molecular level (pmc5775393): acetaldehyde at 100 uM activated a Cidea reporter carrying an sre element in AML12 cells, and 'the knockdown of SREBP1c suppressed acetaldehyde-induced Cidea expression' — a counterfactual. PMID 21920463

It increases Stops fat storage.

Verbatim (pmc12086950): 'Key transcription factors that regulate de novo lipogenesis are sterol regulatory element binding protein 1c (SREBP-1c), early growth response-1 (Egr-1) and carbohydrate response element binding protein (ChREBP) are all activated by chronic alcohol in the liver. Activation of SREBP-1c induces lipogenic genes such as fatty acid synthase (FAS), acyl coA carboxylase (ACC-1 and ACC-2), ATP citrate LYASE (ACL) and STEAROYL COA DESATURASE (SCD-1).'.

It reduces Fatty-acid oxidation.

Verbatim (pmc7530314): 'Ethanol inhibits DNA-binding and transcriptional activity of peroxisome proliferator-activated receptor alpha (PPAR-alpha), with resulting downregulation of fatty acid oxidation. this inhibition occurs directly through the effects of acetaldehyde or indirectly from CYP2E1-generated oxidative stress, adenosine, downregulation of adiponectin, and zinc deficiency.' PPARalpha was carried by zero blends before this build.

It reduces Energy sensing.

Verbatim (PMID 31214376): 'ethanol reduces AMPK levels, which increases ACC1 activity, decreases PPARa levels, and increases mTORC1 activity. Increased mTORC1 further increases SREBP-1c activity and decreases autophagy.' this is the single switch that ties the alcoholic arm to the non-alcoholic one - AMPK down raises ACC1 and lowers PPARalpha in both diseases. The quote attributes it to ethanol; the acetaldehyde-specific step is stated separately in PMC7530314 and the edge carries the ethanol-level evidence. PMID 31214376 · PMC7530314

It increases Liver scarring cells.

10.3748/wjg.v20.i47.17756 — Verbatim, from the title of a Lieber-group primary: 'polyunsaturated LECITHIN prevents acetaldehyde-mediated hepatic collagen accumulation by stimulating collagenase activity in cultured lipocytes' — lipocytes being the older name for stellate cells. The paper names acetaldehyde as the agent and collagen as the outcome. this source is cited by its title alone, without a resolvable identifier; the link rests on the mechanism the title names and is the least-sourced link in this chain. doi 10.3748/wjg.v20.i47.17756
