Bone density
Reads: Bone health
You want it preserved: maintaining bone density guards against fracture and frailty with age.
What this marker reads
Living tissue in permanent turnover — built by osteoblasts, dismantled by osteoclasts, with rankl and osteoprotegerin setting the balance. Density is the running total of that contest, not a fixed property.
No body publishes a range for this marker
BONE DENSITY IS NOT A BLOOD MARKER - it is a DXA measurement, and the number is a T-score, not a concentration; it belongs on a bloodwork page only if that is made explicit. The reference standard from which the T-score is calculated is, per ISCD 2023, 'the female, white, age 20-29 years, NHANES III database'; manufacturers should use NHANES III for femoral neck and total hip T-scores and their own databases for lumbar spine. Site rules (ISCD 2023): 'Use femoral neck or total hip, whichever is lowest'; 'In certain circumstances the 33% radius (also called 1/3 radius) may be utilized'; Ward's area and the greater trochanter 'should not be used for diagnosis'. ISCD states 'The WHO densitometric classification is applicable' and retains the term 'osteopenia' while preferring 'low bone mass' or 'low bone density', but it does NOT itself print the normal or low-bone-mass cut-points. HONEST GAP: no 'normal' band (commonly quoted as a T-score of -1.0 or above) is written here, because the four-category WHO table originates in the WHO Study Group report (WHO Technical Report Series 843, 1994; synopsis Kanis, Osteoporos Int 1994;4:368-381, DOI 10.1007/BF01622200) which could not be retrieved in full - the Sheffield WHO Collaborating Centre PDF now redirects, the WHO IRIS bitstream for Technical Report Series 921 (2003) errored, and neither the 2018 AHRQ evidence review, the 2019 Endocrine Society osteoporosis guideline, the 2013 IOF epidemiology recommendations nor ISCD 2023 prints the normal band. The osteopenia band above comes from the USPSTF's 2002 statement, retrieved from a web archive because the live AHRQ page has moved; it is expressed in SD below the young-adult-woman mean rather than as a labelled T-score range, and it is stated for postmenopausal women. A T-score alone does not diagnose osteoporosis in premenopausal women or men under 50, for whom ISCD uses Z-scores.
Your laboratory reads your result against its own assay, and this page does not do that for it. What is above is what a named body published and for whom; what is below is the machinery the marker reads and the formulas witnessed on it.
What moves it
Training load raises it.
Resistance training raises bone mineral density (10-15% improvement range in older adults alongside strength and lean mass; ACSM consensus statement, PMC8802999); structured exercise preserves or improves BMD in postmenopausal women. The direction holds: pooled RCTs in premenopausal women show significant gains at femoral neck (7 effect sizes, 466 participants, g=0.342, 95% CI 0.132-0.553, P=0.001, NNT=5) and lumbar spine (g=0.201, CI 0.009-0.394, P=0.04, NNT=9) (PMC3563173). But the magnitude and the certainty ARE both low, and the sources say so themselves: a 2025 systematic review and meta-analysis of RCTs concludes physical exercise has shown low efficacy and moderate evidence for gaining total bone mineral density in the elderly (Muscles Ligaments Tendons J 2025;15(3)); and a 2025 meta-analysis of bone metabolism in postmenopausal women rated certainty of evidence low for CTX and osteocalcin and very low for all remaining outcomes under GRADE, downgraded for risk of bias and imprecision (doi 10.3389/fendo.2025.1597046). And it is POPULATION- and modality-specific: resistance, impact, whole-body vibration, endurance and combined training give different site-specific results, and premenopausal, postmenopausal and elderly cohorts do not behave the same. Effect sizes are small (g~0.2-0.34). doi 10.3389/fendo.2025.1597046 · PMC8802999
Estrogen balance raises it.
Shevde 2000, PNAS — estrogen suppresses RANKL/M-CSF-induced osteoclast differentiation via an ERα-dependent mechanism, protecting bone mass. doi 10.1073/pnas.130200197
Calms inflammation lowers it.
Named across six reviews of bone remodeling and osteoarthritis pathogenesis. Bone remodeling relies on the coordinated actions of bone-resorbing osteoclasts, bone-forming osteoblasts, and osteocytes serving as mechanosensors and local remodeling coordinators. PMC10648156
Tissue renewal signaling raises it.
The Wnt/BETA-CATENIN pathway plays a crucial role in promoting the commitment of osteoblast lineage and the production of OPG. However, in osteoporosis, the activity of these pathways will be hindered by increased levels of antagonists like sclerostin and DKK1. And this has an approved drug behind it: the most well-known sclerostin inhibitor, approved by the FDA, is ROMOSOZUMAB - antisclerostin antibodies free LRP5/6, raise beta-catenin and restore Wnt signaling. An edge whose antagonist is a licensed therapy. PMC10648156
Calms body-wide inflammation lowers it.
bone remodeling is a critical physiological process that maintains skeletal integrity, repairs micro-damage, and regulates mineral homeostasis, relying on the coordinated actions of bone-resorbing osteoclasts, bone-forming osteoblasts, and osteocytes serving as mechanosensors. PMC10648156
Allergic signaling lowers it.
PMC10648156
Immune support lowers it.
A reframing worth carrying: immune cells such as T and B cells, dendritic cells and macrophages significantly influence osteoclast formation via cytokine secretion, leading to a view of bone loss as an immunological disorder as well as a skeletal disease. The concept of immunoporosis has emerged to describe bone loss associated with chronic low-grade inflammation due to aging, obesity, autoimmune disease and metabolic dysfunction. That is inflammaging arriving in the skeleton under its own name. PMC10648156
Cellular aging lowers it.
PMC10648156
Cell turnover lowers it.
In osteoporosis, osteocyte dysfunction is manifested by apoptosis, ferroptosis, and pyroptosis. These changes, together with altered secretion, lead to uncoupled remodeling, disruption of the LACUNO-CANALICULAR network and metabolic imbalances. Three distinct death programs, one apoptosis pathway on the MAP - the same simplification as on the heart. PMC10648156
Gut barrier raises it.
PMC10648156

10.1002/jbmr.3089 — This is what the symptom costs rather than what it causes physiologically. It is shown at reduced weight because no separate effect size has been reported for this arm.
