Leptin-downstream melanocortin signaling

The Satiety Switch

Turning it up is a licensed drug for people born with the switch broken. It is not a general obesity treatment — the agonists that reached trials in ordinary obesity largely failed.

In depth

The central appetite and pigmentation relay. Downstream of leptin, it is where the satiety signal is actually read — which is why leptin resistance shows up here rather than at the fat cell.

What drives it

Restores fullness signal increases it.

Nothing in this project has been named that often. This edge is the mechanism of the whole leptin story: leptin, which is produced in adipose tissue, binds to the leptin receptor on pro-opiomelanocortin-expressing neurons in the hypothalamus, and POMC-derived melanocortin peptides reduce food intake and body weight by acting as endogenous ligands for the brain-expressed MC4R. Leptin does not act on fat — it acts on this switch. If the downstream switch is intact, more leptin does nothing. PMID 33137293 · PMC5641599

What it drives

It reduces Supports weight management.

Setmelanotide is the first drug approved by the FDA for the treatment of severe obesity of genetic origin. Meta-analysis: 376 patients, 328 treated a mean 52 weeks, significant weight loss averaging -6.91 percent and a BMI change of -10.55 kg/m2 in adults. Phase 3 trials used a placebo-controlled withdrawal sequence - patients who responded were re-randomized to drug or placebo, which is a far stronger design than single-arm. The genetic dose-response is the part worth keeping: patients with POMC defects upstream of MC4R show significantly more weight loss than MC4R deficient patients OR obese controls, and MC4R knockout mice fail to respond entirely. The further upstream the break, the better a receptor agonist works. And the commercial limit is absolute: this is not a general obesity treatment. The review literature on this class is titled around contextualizing clinical failures. AZD2820 produced absence of weight loss in ordinary obesity, with appetite falling only at intolerable doses, and the authors write the lack of translatability between mouse studies and human clinical trials is confounding. Class safety: other MC4R agonists raised blood pressure and heart rate; setmelanotide was selected partly for not doing so. doi 10.3389/fendo.2026.1797586 · PMID 33137293 · PMC10608339

It increases Curbs appetite.

NBK574539 — drawn separately from the Melanocortin (MC4R) Signaling → Weight & Body Fat link because the trials measured them separately: setmelanotide was associated with significant weight loss and reductions in hunger scores, and the phase 3 results support setmelanotide for the treatment of obesity and hyperphagia. Hyperphagia is a named co-primary indication, not a secondary observation. The endogenous physiology matches: POMC-derived melanocortin peptides reduce food intake.

It increases Female libido.

Melanocortin agonism raises female sexual desire — proven at drug GRADE, in a diagnosed population. Kingsberg SA et al., “Bremelanotide for the Treatment of Hypoactive Sexual Desire Disorder: Two Randomized Phase 3 Trials”, Obstet Gynecol 2019 (PMID 31599840; NCT02333071 and NCT02338960). Reconnect = two identical phase 3, double-blind, placebo-controlled multicenter trials of bremelanotide subcutaneous as needed, 24 weeks, randomized 1:1. Integrated N=1202: mean change vs placebo FSFI-desire +0.35 and FSDS-DAO item 13 -0.33 (P<.0001 both). Responder rates 58.3% / 58.2% vs 36.1% / 35.4% placebo. Scope — read this before any claim rests on this edge: (1) bremelanotide is a prescription injectable drug, not a botanical — the edge shows the pathway is real, not that any supplement reproduces it; (2) the population is premenopausal women with diagnosed, acquired, generalized HSDD, not healthy women; (3) the effect sizes are small (+0.35 on FSFI-D); (4) adverse events nausea, flushing and headache each exceeded 10% in both studies; (5) funding: Palatin Technologies and AMAG Pharmaceuticals — the manufacturers.
